- 3. Premed • Orthopedic Premed Clinics are held every Thursday and Friday, 10am to 1pm, maximally 5 patients each, seeing patients for elective operation in near future (within 1-4 weeks)

Premed Flow • Patient planned for Elective Op-during clinic • Premed Screening(fill up form)-during clinic – Need further optimization • Refer medical outpatient – Optimized with controlled premorbidity • Patient put on premed date, Op date • Patient given written info on blood taking date, premed date, op date • FBC/RP/Coag/FBS IMR form, CXR PA erect form given(forms in bilik 5) • Blood taking date – Venue Bilik 7, One week before premed date, Friday 8am, fasted overnight – SN draw blood at bilik 7, CXR taken to give SN • Premed Date • Op date

Premed • Premed date – Pt come 8am, ECG, V.signs taken, interview by HO, clerked in premed clerking sheet – Pt brought up to Ward 11 at 10am-1pm, r/v by anaest • Pass – Proceed to finalize/verify Premed Clerking sheet by MO, countersign first page • Fail – Optimize pt accordingly/Refer to medical for optimization – New Premed Date, KIV new op date

- 7. TBP/ORATIS/JPA/SOCSO form • Centralize applications in organized system
- 8. Applications Flow • Patient needing applications identified-clinic • Patients Folder attached with memo, placed in Box 1 – Memo contain pt name/ic/type of applic./phone/requesting surgeon/written by/date • Thursday PM (end of clinic day) box 1 collected by SN, distribute to MOs • 5 working days to settle the applications – Forms/quotations/letterhead etc • Put back box 2 • SN in charge collect from Box 2, approach requesting surgeon to sign, approach HOD to sign, despatch

TBP • Quotation x 3 (1 real, 2 dummy), Borang C, Social welfare referral for socioeconomic assessment, Letterhead • TBP application will be collected by SN in charge from box B, and to be countersign by requesting surgeon and HOD • Estimated time of approval 4months • Eligible – financially dependent, no other source of funding, need of the operation after assessment by requesting surgeon
- 11. SOCSO • Quotation x1, letterhead • SOCSO application will be collected by SN in charge from box B, countersign by requesting surgeon, and pt will be called by SN to collect the forms, pt to self submit to SOCSO unit in charge • Eligible – pt under SOCSO, work related injury

ORATIS • Quotation x1, letterhead, jpa form(sign by pt during clinic when decided for application),card pesara (photocopy double side + phone number) • Application will be collected by SN in charge from box B, sign by requesting surgeon, pass to MA Sofian to fax over to ORATIS centre • Estimated approval time 1 week (implant less than RM10k)2 months for implant more than RM10k • Eligible – Patient with Kad Pesara, retire from feneral departments (not including state departments) • URGENT ORATIS application need to call up ORATIS centre and request – as short as 3 days (0355103148)
- 13. JPA • Quotation x1, letterhead, jpa form • Application will be collected by SN in charge from box B, countersign by requesting surgeon, pt will be called up to collect and submit to own department in charge
