With the February 15th Open Enrollment deadline just a few weeks away,
Freeway Insurance Health Agents will be available to sign-up customers so that
they can avoid paying the penalty: 2% of a person’s income or $325 per adult/
$162.50 per child, whichever is more.

5. What Should I bring To Enroll?
To help complete the enrollment process
individuals should bring the following:
• Social security card
• Residency identification
• Recent W-2 form
