Angina, Heart failure and dyslipidaemia
1. Anti-anginal drugs
and
Drugs used in Heart failure
Dr. Pravin Prasad
M.B.B.S., MD Clinical Pharmacology
Lecturer, Lumbini Medical College
6 March, 2019 (22 Falgun 2075), Wednesday
2. By the end of the class BSN 1st year
students will be able to:
â˘Classify drugs used in the treatment of:
âAngina pectoris
âHeart failure
âHyperlipidaemia
â˘Explain the mechanism of action, indications and
adverse effects of:
âNitrates
âDigoxin
3. Angina Pectoris: Introduction
â˘Condition in which there is an inadequate supply of
blood and oxygen to a portion of the myocardium
â˘Spectrum of diseases that includes:
âAngina Pectoris
âMyocardial Infarction
âChronic ischaemic heart disease with heart
failure
âSudden cardiac death
5. Nitrates
â˘Includes Nitroglycerine, Isosorbide mononitrate/
dinitrate
â˘Acts by:
âDecreasing cardiac work in stable angina
â˘Dilatation of veins â peripheral pooling of
blood â decreased preload
â˘Dilatation of arteries â decreased afterload
âIncreasing blood supply in variant angina
â˘Dilates larger coronary arteries
6. Nitrates: Indications
â˘Angina Pectoris:
âClassical as well as variant
âTerminating an attack as well as prophylaxis
⢠Acute Coronary Syndrome
⢠Congestive Heart Failure and acute Left
Ventricular Failure
⢠Biliary colic, Esophageal spasm
⢠Cyanide poisoning: sodium nitrate
11. Digoxin
â˘Includes: digoxin, digitalis
â˘Also known as cardiac glycosides
âExtracted from plants
â˘Acts by:
âIncreasing force of contraction
âDecreasing heart rate
â˘Better emptying of heart (Cardiac output
increased)
13. Digoxin: Mechanism of Action
â˘Binds to and inhibits Na+ K+ ATPase of myocardial
fibres
âProgressive accumulation of Na+ and Ca2+
intracellularly
âExtra Ca2+ loaded into sarcoplasmic reticulum
âIncreased release of Ca2+ subsequently
âIncreased force of contraction
14. Digoxin: Indications
â˘Congestive Heart Failure
â Most effective in patients with dilated heart and
low ejection fraction
â CHF with AF in need of ventricular rate control:
continuous digoxin
â˘Cardiac arrhythmias
âAtrial Fibrillation, Flutter
âParoxysmal Supraventricular Tachycardia (PSVT)
16. Drugs used in Dyslipidaemia
â˘Also known as hypolipidaemic drugs
âDecreases the blood lipids (bad cholesterol) level
â˘Commonly prescribed in patients with:
âDyslipidaemia
âHypertension
âDiabetes mellitus
âIschaemic heart disease
18. Conclusion
â˘Nitrates are used for the termination of attack of
angina pectoris
âActs by decreasing preload and afterload in
stable angina
âActs by dilating coronary vessels in variant
angina
â˘Digoxin is used for the treatment of heart failure
âActs by increasing the cardiac output
