Part 1: (250 words Maximum)
For this discussion, first research human services jobs in your
local area or in an area (city or state) of interest to you across
three settings of your choice. The settings may include any of
the following:
Treatment or rehabilitation center
Hospital
Outpatient health clinic
Home health
Hospice
Then summarize the role and responsibilities described in the
three different job postings you discovered.
Finally, in response to your peers, highlight any differences
between the jobs you highlighted and consider what may
account for these differences (location, setting, and so on).
Part 2: (250 words Maximum)
Overview:
Before beginning the assignment, you should be familiar with
the three public health intervention levels. The following case
(Case Attached)
follows a localized outbreak of hepatitis A and the actions
taken by local citizens, governmental organizations, and a
coalition of hospitals. As you read, identify specific parties and
their actions/interventions to address the crisis. Using the table
below, identify the following:
·
What:
the action/intervention taken
·
Who:
the organization or person undertaking the action/intervention
·
Whether the action/intervention was
primary, secondary, or tertiary
.
Instructions
Identify at least three interventions from the case above, using
no more than 10 or so words (without an explanation). For each
instance, identify the action, the person or organization
performing the action, and the type of intervention it is. There
are eight separate interventions identified, and two are
completed as examples (numbers 4 and 8). The order of your
entries does not matter.
Grading: You will receive 4 points for each correct cell/answer,
up to a total of 36 point
s.
·
Primary interventions involve activities aimed at reducing
factors that lead to health problems.
·
Secondary interventions involve early detection of and
intervention in the potential development of a public health
problem.
·
Tertiary intervention is focused on treatment of an existing
health problem to lessen its effects and prevent further
deterioration and recurrence.

Part 3: (250 words Maximum)
Prompt:
You will examine public health interventions published in
scholarly literature for how they addressed your selected public
health issue.
Specifically, the following
critical elements
must be addressed:
III.
Public Health Interventions
A.
Analyze
at least two published public health interventions used to
address the issue, describing both the interventions and the
theoretical basis of each. In other words, what were the
intervention strategies used to address this problem and what
was the rationale (the thinking behind the strategy, why the
strategy works) for those actions, according to your resource?
B.
Explain how the selected interventions reflect primary,
secondary, and/or tertiary intervention
strategies
for public health prevention.
C.
Explain how the interventions
address
and/or mitigate the social determinants of the issue (health and
healthcare disparities associated with each). Remember that
social determinants are the conditions in which people are born,
grow, live, work, and age.
Use the
Public Health Interventions Worksheet Below
to enter your descriptions. Your instructor will review your
work and provide feedback and possibly suggestions. Note that
the same worksheet will also be used for completing Milestone
Four in Module Five.
For use in completing Milestone Three (
items in blue
)

Part 4:
Inpatient Assessment and the Interdisciplinary Team (Sections
II and III)
Prompt:
Create a draft of Sections II and III of your final project, the
collaborative care guide.
II.
Inpatient Assessment
A. In a narrative format, provide an overview—using a
strengths-based focus—to identify the
inpatient client needs
represented in the context of the case.
1. What are the central physiological, emotional, social, and
environmental issues facing this client?
2. How will her strengths help her make any needed transitions?
3. Does trauma impact this client? How?
B. What
potential complexities
could the client encounter during her inpatient stay? How will
these complexities impact the client?
C. What types of
strategies
could you employ in this setting to ensure that the client is not
re-traumatize
D. Develop effective strategies to address the client’s
inpatient financial needs
. How can you best facilitate access to recommended services?
1. What resources might be available in her community to fill
unmet financial needs?
2. Are there state or federal resources available to assist this
client?
III.
The Interdisciplinary Team
A. Based on the case study, ideally
who should serve
on the interdisciplinary team tasked with developing the client’s
care plan? Why?
B. Analyze the
role
of each member of the interdisciplinary team in relation to the
following:
1.
Improving patient care
. For instance, you might consider the role of a team member
who would have day-to-day interaction with the client
throughout the client’s hospital stay.
2.
Improving patient outcomes
. For instance, you might consider the role of a team member
who would be responsible for developing cost-effective
outcomes.
C. What strategies should be implemented by the team to
ensure the patient understands
the terms and outcomes of the post-discharge plan?
Guidelines for Submission:
Your draft of Sections II and III should be about 2 pages in
length and should use double spacing, 12-point Times New
Roman font, one-inch margins, and APA formatting.

  • 1. Part 1: (250 words Maximum) For this discussion, first research human services jobs in your local area or in an area (city or state) of interest to you across three settings of your choice. The settings may include any of the following: Treatment or rehabilitation center Hospital Outpatient health clinic Home health Hospice Then summarize the role and responsibilities described in the three different job postings you discovered. Finally, in response to your peers, highlight any differences between the jobs you highlighted and consider what may account for these differences (location, setting, and so on). Part 2: (250 words Maximum) Overview: Before beginning the assignment, you should be familiar with the three public health intervention levels. The following case (Case Attached) follows a localized outbreak of hepatitis A and the actions taken by local citizens, governmental organizations, and a coalition of hospitals. As you read, identify specific parties and their actions/interventions to address the crisis. Using the table below, identify the following: · What: the action/intervention taken · Who: the organization or person undertaking the action/intervention
  • 2. · Whether the action/intervention was primary, secondary, or tertiary . Instructions Identify at least three interventions from the case above, using no more than 10 or so words (without an explanation). For each instance, identify the action, the person or organization performing the action, and the type of intervention it is. There are eight separate interventions identified, and two are completed as examples (numbers 4 and 8). The order of your entries does not matter. Grading: You will receive 4 points for each correct cell/answer, up to a total of 36 point s. · Primary interventions involve activities aimed at reducing factors that lead to health problems. · Secondary interventions involve early detection of and intervention in the potential development of a public health problem. · Tertiary intervention is focused on treatment of an existing health problem to lessen its effects and prevent further deterioration and recurrence. Specifically, the following critical elements must be addressed: III. Public Health Interventions A. Analyze at least two published public health interventions used to address the issue, describing both the interventions and the theoretical basis of each. In other words, what were the intervention strategies used to address this problem and what was the rationale (the thinking behind the strategy, why the strategy works) for those actions, according to your resource? B. Explain how the selected interventions reflect primary, secondary, and/or tertiary intervention strategies for public health prevention. C. Explain how the interventions address and/or mitigate the social determinants of the issue (health and healthcare disparities associated with each). Remember that social determinants are the conditions in which people are born,
