Tegrital CR Tablets (Generic Carbamazepine ER Tablets)

Tegrital CR Tablets (Generic Carbamazepine ER Tablets) belongs to
a group of medicines called anticonvulsants and is used to to
control epilepsy, a condition in which there are repeated seizures
(fits). There are many different types of seizures, ranging from mild
to severe.
This medicine is also used to control sudden, repeated attacks of
facial pain, known as trigeminal neuralgia, to control mania, a
mental condition with episodes of overactivity, elation or irritability,
to control bipolar mood disorder where periods of mania alternate
with periods of depression, to control the pain associated with
painful diabetic neuropathy, to control diabetes insipidus, a
condition where there is excessive thirst and urination and to
control the symptoms of alcohol withdrawal syndrome.

Active Ingredient
The active ingredient present in Tegrital CR tablets is Carbamazepine.
Each Tegrital CR tablet contains 200 mg, 300 mg or 400 mg of
Carbamazepine.

Carbamazepine Tablets – Brand Names
Carbamazepine tablets are available under
various drug trade names that include

Tegretol, Tegretol XR, Tegrital, Tegrital CR,
Carbadac, Carbatol, Carmaz, Fobigone, Gemtol,
Gemtol CR, Mazetol, Mazetol SR, Versitol,
Versitol Rtd, Versizur, Zen, Zen Retard, and
Zeptol.

Uses of Tegrital CR Tablets
Tegrital CR Tablets (Generic Carbamazepine ER Tablets) are used to treat
certain types of seizures (epilepsy). It is also used to relieve pain due to
trigeminal neuralgia (tic douloureux) and in the treatment of bipolar
disorder (manic-depressive illness).
Carbamazepine works in the brain and nervous system to control seizures,
pain, and bipolar disorder. This medicine is an anticonvulsant.
Carbamazepine is not effective in controlling absence, myoclonic or atonic
seizures, and does not prevent the generalization of epileptic discharge.
Moreover, exacerbation of seizures may occasionally occur in patients with
atypical absences.

Tegrital CR Tablets Dosage
The dosage of Tegrital CR tablets (Generic Carbamazepine ER Tablets) is
given below:
Epilepsy: The starting dose is 100 mg to 200 mg once or twice daily; the
dosage should be slowly raised until an optimum response is obtained
which is generally at 400 mg 2 to 3 times daily. In some patients 1600
mg or even 2000 mg daily may be necessary.
Acute Mania And Maintenance Treatment Of Bipolar Affective
Disorders: Dosage range is about 400 to 1600 mg daily, the usual dosage
being 400 to 600 mg daily given in 2 to 3 divided doses. In acute mania,
the dosage should be increased rather quickly, whereas small dosage
increments are recommended for maintenance therapy of bipolar
disorders in order to ensure optimal tolerability.

The dosage of Tegrital CR tablets (Generic Carbamazepine ER Tablets) is
given below:
Alcohol-Withdrawal Syndrome: The average dosage is 200 mg 3 times daily. In
severe cases, it can be raised during the first few days (e.g. to 400 mg 3 times
daily). At the start of treatment for severe withdrawal manifestations, Tegrital CR
should be given in combination with sedative-hypnotic drugs (e.g.
Clomethiazole, Chlordiazepoxide). After the acute stage has abated, Tegrital CR
can be continued as monotherapy.
Trigeminal Neuralgia: The starting dosage of 200 to 400 mg should be increased
slowly daily until freedom from pain is achieved (normally at 200 mg 3 to 4
times daily). The dosage should then be gradually reduced to the lowest possible
maintenance level. Maximum recommended dose is 1200 mg/day.

The dosage of Tegrital CR tablets (Generic Carbamazepine ER
Tablets) is given below:
Trigeminal Neuralgia: When pain relief has been obtained, attempts
should be made to gradually discontinue therapy, until another
attack occurs.
Painful Diabetic Neuropathy: The average dosage: 200 mg 2 to 4
times daily.
Diabetes Insipidus Centralis: The average dosage for adults: 200 mg
2 to 3 times daily.

Tegrital CR Tablets Contraindications
Tegrital CR tablets are contraindicated in patients with a
hypersensitivity to Carbamazepine or any other component of this
medication.
This medicine should also not be administered to patients with a
history of previous bone marrow depression, known sensitivity to any
of the tricyclic compounds, such as Amitriptyline, Desipramine,
Imipramine, Protriptyline, Nortriptyline, etc.
Likewise, on theoretical grounds its use with monoamine
oxidase(MAO)inhibitors is not recommended. Before administration of
Tegrital CR, MAO inhibitors should be discontinued for a minimum of
14 days, or longer if the clinical situation permits.

Tegrital CR Tablets Contraindications
Coadministration of Carbamazepine and Nefazodone may result in
insufficient plasma concentrations of Nefazodone and its active
metabolite to achieve a therapeutic effect. Coadministration of
Carbamazepine with Nefazodone is contraindicated.

Storage Instructions
Tegrital CR Tablets (Generic Carbamazepine
ER Tablets) have to be stored at controlled
room temperature i.e. from 20°C to 25°C (68°F
to 77°F) with excursions permitted between
15° to 30°C (59° to 86°F).
Keep this as well as all other medicines away
from children and pets.

Warnings
Drug Reaction with Eosinophilia and Systemic Symptoms: Monitor
for hypersensitivity. Discontinue if another cause can not be
established.
Suicidal Behavior and Ideation: Monitor for depression, suicidal
thoughts or behavior, and/or any unusual changes in mood or
behavior.
Embryofetal Toxicity: Advise women of child-bearing potential of
possible risks to the fetus.
Abrupt Discontinuation and Risk of Seizure: Taper the dose when
discontinuing treatment.
Hyponatremia: Consider discontinuing Tegrital CR tablets in patients
with significant symptomatic hyponatremia.

Warnings
Cognitive and Motor Impairment: Advise patients not to drive or
operate machinery until they have gained sufficient experience on
Tegrital CR tablets to gauge whether it adversely affects these
activities.
Liver Damage: Monitor liver function. Discontinue Tegrital CR
tablets with aggravated liver dysfunction or active liver disease.
Hepatic Porphyria: Avoid Tegrital CR tablets use in patients with
hepatic porphyria: can cause acute episodes of porphyria.

Tegrital CR Tablets Side Effects
Most common Carbamazepine (Tegrital CR tablets)
side effects with an incidence rate of 5% or more and
two times placebo were dizziness, somnolence,
nausea, vomiting, ataxia, constipation, pruritus, dry
mouth, asthenia, blurred vision, and speech disorder.

Overdose
The presenting signs and symptoms of overdosage usually involve the central nervous,
cardiovascular, and respiratory systems, as well as the adverse drug reactions mentioned
under side effects. Relapse and aggravation of the symptomatology on the 2nd or 3rd day
after overdose, due to delayed absorption, should be anticipated.
Central Nervous System: CNS depression, disorientation, depressed level of consciousness,
tremor, restlessness, somnolence, agitation, hallucination, coma, blurred vision, nystagmus,
mydriasis, slurred speech, dysarthria, ataxia, dyskinesia, abnormal reflexes (slowed /
hyperactive), convulsions, psychomotor disturbances, myoclonus, opisthotonia,
hypothermia / hyperthermia, flushed skin/cyanosis, EEG changes.
Respiratory System: respiratory depression, pulmonary edema.
Cardiovascular System: tachycardia, hypotension/hypertension, conduction disturbance with
widening of QRS complex, syncope in association with cardiac arrest.
Gastrointestinal System: nausea, vomiting, delayed gastric emptying, reduced bowel motility.
Musculoskeletal System: There have been some cases which reported rhabdomyolysis in
association with carbamazepine toxicity.

Overdose
Renal Function: urinary retention, oliguria or anuria; fluid retention, and
water intoxication.
Laboratory Findings: hyponatremia, hypokalemia, leukocytosis, reduced
white cell count, metabolic acidosis, hyperglycemia, glycosuria, acetonuria,
increased muscle creatine Phosphokinase.
There is no known specific antidote to Carbamazepine (Tegrital CR tablets).
Evacuate the stomach, with an emetic or by gastric lavage and then
administer activated
charcoal. Delay in evacuating the stomach may result in delayed absorption,
leading to relapse during recovery from intoxication.
Hemodialysis is the effective treatment modality in the management of the
Carbamazepine overdose.

Overdose
Vital signs, including electrocardiogram to detect any cardiac
arrhythmias or conduction defects, should be watched and
symptomatic treatment should be administered as required.
Hyperirritability or convulsions should be appropriately managed
by standard medical care.
Hyponatremia should be appropriately managed by standard
medical care.
Shock (circulatory collapse) should be treated with supportive
measures, including intravenous fluids, oxygen, and corticosteroids.
Charcoal hemoperfusion has been recommended.

Tegrital CR tablets During Pregnancy
Carbamazepine (Tegrital CR Tablets) has been classified by the US FDA as
Pregnancy Category D.
Carbamazepine can cause fetal harm when administered to a pregnant woman.
Epidemiological data suggest that there may be an association between the use of
Carbamazepine during pregnancy and congenital malformations, including spina
bifida.
There have also been reports that associate Carbamazepine with developmental
disorders and congenital anomalies (e.g., craniofacial defects, cardiovascular
malformations, hypospadias and anomalies involving various body systems).
Developmental delays based on neurobehavioral assessments have been reported. In
treating or counseling women of childbearing potential, the prescribing physician
will wish to weigh the benefits of therapy against the risks.
