I have already met with you or informed you about your child’s Tier I - Personal Education Plan.
My grade level team has met and discussed the progress that has been made. During that meeting
we developed a Tier II plan in order to better meet the needs of your child. Part of this process
includes having a vision and hearing screening completed by the school nurse. I have included a
form that you must sign and check if you give permission for the screenings. By allowing us to
complete the screening, we can rule out if either of those are problems your child may be
experiencing.
