CONTENTS
Fiscal Year 2014 has been a year of excellence and
growing connection to the communities we serve.
We increased our clinical impact through various service expansions, including Optometry
at our prominent Mondawmin Health Center location and increased specialized services
such as pediatric dental screenings. Our Behavioral Health, Substance Abuse, and HIV
services continue to be thought-leaders and among the most robust programs in the
Baltimore region. We are also excited to expand our footprint as we are in the planning
stages of opening a new health center at the Hebrew Orphan Asylum in the Rosemont
community, partnering with the Coppin Heights Community Development Corporation.
With community always at the forefront of what we do, THC has extended its
reach to even more community partners with like missions and values and the
same zeal to serve. As a participant in the West Baltimore Health Enterprise Zone
through the West Baltimore Care collaborative, we affected positive change and
clinical outcomes, fighting cardiovascular disease in our city. We continued with
our various giving campaigns, including our Turkey Dinner Giveaway to families
in area partnering schools and the Holiday Gift Drive for our patients.
Fiscal Year 2014 still had its challenges. As we endured one of the most eventful winters
our region has seen in decades, our patients were less likely to come into our centers for
care on those heavy snow days and this yielded more closures than expected. We also
experienced additional clinic down time from the transition of our Electronic Medical
Records, a necessary investment and project as we continue to keep efficiency and
quality at the heart of our operations. While these factors did impact our operations
we still saw over 37,000 patients and can report positive net income for FY2014!
We are encouraged about our work this past fiscal year and even more excited at the
additional lives we will continue to touch through our efforts. While the health care
environment remains one of constant change, Total Health Care will be steadfast in its
mission to provide excellent medical services to the people and communities we serve,
ensuring that all who walk through our doors will find a permanent medical home.

CESH (Center of Emotional and Social Health)
Mental Health Services are just as important as physical well-being for total
health. That’s why Total Health Care is committed to improving the mental
health and quality of life for all members of the community. We provide
high-quality services to those with physical barriers and behavior and
mental health issues, working to improve the way they feel about everyday
life. Our services comprise a variety of areas beginning with mental health
screenings and including group and individual mental health counseling,
nutritional assessment and therapy, and preventive health education.
Outreach services identify and assist with navigation into mental healthcare
treatment. Our team consists of licensed psychiatrists, psychologists, social
workers, counselors, psychiatric nurses and public health administrators.
Alcohol and Substance Abuse Treatment Services
One invaluable program is Total Health Care’s alcohol and substance
abuse treatment services. Overuse of alcohol, drugs or other
substances is considered abuse, and Total Health Care has programs
to help fight this community concern. The Total Health Care Substance
Abuse Program is a state-certified, outpatient program that treats the
whole person. This includes physically, psychologically, socially and
spiritually. Our substance abuse case management services link on-
site primary care, mental health, housing, and counseling services.
HIV Medical and Mental Health Care
Support group sessions provide additional support to patients
with challenges to fully understanding their HIV condition by
linking them to the full spectrum of HIV services available. Case
managers help patients navigate services within and outside of
Total Health Care so they can receive the total care they need to
manage the disease and live productive and fulfilling lives.

Patient-directed care is the core of Total Health Care. To help our
patients direct their care, we developed our Patient-Centered
Medical Home. It’s not a building, a house, or a hospital. It
is a coordinated team of medical experts who make sure
patients are their healthiest. We provide the team of experts
and specialists to ensure the best care is available, and
together with the patient, we develop a care plan based on that
individual’s needs. The ultimate decisions are the patient’s.
The care team gains access for patients to all needed services.
This includes doctor visits, shots, testing, follow-up visits,
home care services, medication reviews and discounts,
and educational information like nutrition and self-care.
This ensures patients are in control of their care.
Patient Centered Medical Home

Total Health Care’s Department of Pharmacy does more than
dispense medications. We care for our patients, making sure that
the medications they are prescribed are safe and the best choice
to deliver maximum results. We monitor dosages and coordinate
with the prescribing physician as well as our database to ensure
the right medication is chosen based on the patient’s medical
history. We provide patients with drug and disease information,
medication therapy management programs and counseling services.
In addition, our drug preparation and distribution services give
the patient increased accessibility to customized medications.
Patients choose from on-site retail pharmacy centers at six
convenient Total Health Care locations, where they find the
medications they need in name brand and generic forms. Licensed
pharmacists are always on-site to provide prescriptions quickly,
usually within 20 minutes, and counseling for patients who just
want a question answered or a medication side effect explained.
Our youngest patients often need the most attentive care. That’s why
only board-certified pediatricians and family practitioners care for
children at Total Health Care. Parents have peace of mind and children
feel comfortable, whether they visit for flus and illnesses, asthma and
allergies, skin diseases, sprains or strains. We also provide essential
services to the community for minors like wellness physicals and
vaccinations, confidential visits and contraceptive management. Our
specialists have worked with children from birth through adolescence,
and they become like family. Many children choose to remain with our
pediatricians until their early 20s or as long as insurance permits.
At Total Health Care, our pediatric services are provided in
seven locations. Late hours, on-call services, same-day sick
appointments as well as routine care are all available in our offices.
Our specialty services include nutrition, diet and obesity, mental
health and social work, sports physicals, asthma and allergy
testing. It’s all here for our patients, in one place, at one time.

a smile, especially for children.
Total Health Care is one of the city’s few community healthcare
organizations committed to dental health for children. We
provide pediatric dental screenings that include preventive care
services and a focus on oral health education with dental literacy
a priority for all ages. Diagnosis and treatment of cavities and
dental conditions are handled at our centers and, if necessary,
our dentists work in collaboration with our medical providers.
Total Health Care hosts several dental screening events each
year to improve the oral health of the community’s youngest
members. Plans are underway to expand our efforts to open
additional dental sites with a focus on our pediatric patients.

Total Health Care continues to expand its services to provide
comprehensive care for the community. In fall 2015, Total Health
Care will open its ninth center, in the former Hebrew Orphan’s
Asylum, located in Baltimore’s Rosemont neighborhood. The full-
service center will offer enhanced pediatric and dental services
and provide area residents with health education and preventive
health programs to further improve the health of the community.
The historic building’s foundation has been stabilized to improve
safety and provide structural improvements. Total Health Care
is pleased to operate within a gem of the community, which was
built in 1874 as a Jewish orphanage. The mansion, which later
housed West Baltimore General Hospital from 1923 through
1945 and the Lutheran Hospital of Maryland from 1945 to
1989, is listed on the National Register of Historic Places.
At the City Center
In Full Sight
Responding to the community’s needs is paramount for
Total Health Care. So it was only natural that optical services
became a part of the health care organization’s expanded
offerings for patients who reside or work in the area.
Our board-certified optometrists and ophthalmologists work to
make sure the eyes are just as healthy as the body. Services
provided include comprehensive eye exams with and without
refraction, glaucoma screenings and diabetic retinal evaluations.
Our centers also offer a wide variety of eyeglass and sunglass
frames that are fitted by our specially trained opticians. We
offer lightweight, bi-focal, progressive, transition and digitally
constructed lenses. We also provide custom-fitted contact lenses
maximizing comfort for our patients. It’s all part of our commitment
to full service and comprehensive care for our patients.

Our network operates at optimum speed, enabling clinicians to access essential
data quickly. This also allows them to easily stay abreast of the latest techniques
and best practices. System cell phones and laptops enable communication
around the clock.
Our equipment is technology-based, using the most advanced technology
available today. Tests are performed more easily, results are provided more
quickly and the data is more detailed. Consistent upgrades enable us to remain
on the cutting edge of patient care.

employed in a variety of ways to optimize our
operations and further improve patient care.

HIGHLIGHTS

The Health Enterprise Zone is a collaborative effort between local
healthcare providers, Bon Secours Baltimore Health System,
and West Baltimore Primary Care Access Collaborative. Partners
in the Health Enterprise Zone are working to expand access to
primary care for those living within it. This affords the creation
of a community health infrastructure to serve residents in West
Baltimore, a community facing some of the highest disease
burden rates and worst social determinants of health care in
Maryland. One of the program’s successes is the result of our
Community Health Workers’ (CHWs) dedication — they are the
experts that knit medical experience with patient lifestyle habits
to create healthier living for individuals and families. Based upon
need, available resources, and success, the outreach of the
Health Enterprise Zone will expand to assist more persons. The
ultimate goal is to foster self-responsibility for good health.

Total Health Care is a member of the Maryland Maternity Access
Coalition, which aims to create a Maryland Birth Injury Fund to
compensate families and help care for children with permanent
birth injuries. The benefits are to include nursing care, medical
equipment and specialty care. The fund would be paid for by
hospitals, OB/GYN physicians, and medical liability insurance
companies to help families and protect access to quality maternity
health care and eliminate the need for costly litigation. Recent polls
indicate strong support for the fund. Total Health Care is committed
to providing care for all members of the community in need.

Maternity Access Coalition

All citizens have the right to receive healthcare services regardless
of their ability to pay. Many community members do not know
where to go or how to find healthcare services. To help, Total
Health Care employs Community Health Workers to assist
community members in gaining access to the care they need.
Community health workers strive to match Baltimore residents with
the specific healthcare services they need. They assist community
members throughout the healthcare spectrum, from guiding them with
healthcare applications to serving as navigators connecting them to
case managers and clinicians, helping community members understand
the need for healthcare prevention services and staffing health fairs
to test for serious health conditions. Respect, professionalism and
support are words community members use to describe their efforts.
Moms Clubs at Total Health Care are clinic-based prenatal education
support groups open to female patients at 34 weeks or less gestation.
The program is designed to help pregnant women connect with
important information and resources. Guided by the Baby Basics
curriculum, which provides pregnant women with comprehensive
and easy-to-read prenatal materials, the Moms Clubs empower them
to effectively navigate the healthcare system and become better
advocates for themselves and their families. Each participating
woman receives a Baby Basics pregnancy guide and planner and
agrees to attend six weekly sessions of the program at the health
center. Participating women also receive one-on-one care coordination
from THC’s Community HealthCorps members to address their
individual care needs. Moms Club at Total Health Care was developed
in partnership with Community HealthCorps Maryland, B’More
for Healthy Babies Upton/Druid Heights and Promise Heights.
In addition to clinic-based Moms Clubs, THC periodically facilitates
school-based Young Moms Clubs for pregnant teens in partnership
with Renaissance Academy and the Teen Pregnancy Prevention
Initiative at the Baltimore City Health Department. Young Moms
Club utilizes the Baby Basics curriculum, yet also incorporates family
planning, career planning, and other age-appropriate topics.

A friend suggested Emmanuel Bartholomew contact Total Health
Care for primary care services. Without insurance, he was hesitant
about what he could pay. Emmanuel had diabetes; his sugar was
a whopping 530 and his weight was up to 255 pounds. He could
have been in a diabetic coma if it were not for his love of walking.
Despite his initial hesitancy, Emmanuel followed through on his
friend’s suggestion and connected with his local Total Health Care
center. THC’s Dr. Samuel Williams got Emmanuel on medication,
referred him for podiatry and ophthalmologic care, and put him on an
exercise regimen. Emmanuel also met with a nutritionist, who helped
him record his meals, watch his portions, and improve his diet.
Total Health Care worked with Emmanuel to price his care
based on what he could afford while simultaneously working
to enroll him in insurance. Now with insurance, he can get
his medication refill for just $14 and his TRU strips for $15.
Everyone he’s worked with has been wonderful, he says.
“Without them I don’t know where I’d be now,” says
Emmanuel, whose blood sugar is down to 105 and his
weight an improved 235. “I’m a patient for life.”

Victor Parker first saw Dr. Arethusa Kirk when he was
new to the area. He was just eight years old and needed
a checkup. He was nervous about seeing a new doctor,
but he soon realized there was no reason for concern.
Dr. Kirk was friendly and made him feel at home.
Today, at 18, he considers her a friend. They have a
good relationship and he trusts her with his health.
“She always makes me feel important,” he says.
“Everything I have to say is important to her and she
makes sure I understand everything we talk about.”
Victor also credits the staff with making him feel comfortable.
He laughs and jokes with them while waiting for Dr. Kirk.

Tiffany Groover, M.D., saw a patient at the Men’s Health Center on
North Avenue.
He returned from the restroom looking pale, clammy and ill. His
sugar level was through the roof, and his diastolic pressure was too
low to measure.
She felt like she was back in her residency training in the Intensive
Care Unit.
“He was close to cardiac arrest and dying and needed a defibrillator
in his heart,” says Dr. Groover. “If I had let him walk away without
being cautious, he probably would not have survived.” Dr. Groover
has seen the patient frequently since and he’s doing much better.
Getting patients the care they need is the main reward for
Dr. Groover. Most are not that extreme, but they come in and tell
her they’ve quit smoking or their medical condition is under control
because they are taking their medications daily.
Dr. Groover chose Total Health Care for her medical practice because
of THC’s commitment to improving the health of the community. She
trained to care for the underserved, and she thrives on caring for the
Baltimore community. Her stellar reputation in the community for
providing preventive care, education and care for chronic illnesses
for men of all ages has seen her practice grow.
“We truly care for everyone,” she says. “Everyone needs care.”

“…I realized this
is where I need to

the need is.”
She remembers being just eight years old, and her mother was
quite ill. The young girl told her mom to lay down. She would take
care of her. Since that moment, Teonya Perry-Barbour wanted
to be a nurse. “I’ve always been the one to patch everyone up,”
she says. That little girl who started out patching up dolls, and
relatives, and playing nurse with her cousins grew up and never
gave up on her goal. She started as a Geriatric Nursing Assistant
then progressed up the career ladder to Certified Nursing Assistant
then LPN in a float pool and ultimately earned her RN. “I’ve always
loved nursing; it interested me to take care of people, nurture
people,” she says. “It was my calling.” Teonya joined Total Health
Care in 2011, and today serves as clinical coordinator at the
Immediate Care Center on Division Street. She’s the friendly face
who greets and triages patients, making sure to get them what’s
needed. She manages the center and oversees an LPN and three
medical assistants. Her impact is great and she can see the
results of her labor, as a recent patient can attest. The woman
presented with serious health issues including hypertension.
With no job and no health insurance, she could no longer see
her primary care physician. Teonya pulled her aside and the
woman burst into tears. She calmed her down and talked to her.
Teonya got her back to see the physician and the medications
she needed. The woman was grateful. “That’s when I realized
this is where I need to be, this is where the need is,” she says.

For two years, Total Health Care has partnered with Promise
Heights to care for the Upton/Druid Heights area of Baltimore.
Promise Heights, a Promise Neighborhood administered by the
U.S. Department of Education and operated by the University of
Maryland School of Social Work, aims to improve the educational,
social, health, and economic opportunities of children from birth
to young adulthood in the West Baltimore neighborhood.
Total Health Care is committed to improving the health of
the area and is partnering with Promise Heights to build a
pipeline of services for children from before birth through
college. Among our offerings are prenatal services, health
education and screenings, pediatric services, parenting
workshops, moms clubs and physician-led classes.
“They are a community institution and the people in the
community really trust them,” says Bronwyn Mayden,
executive director of Promise Heights. “They develop strong
relationships with people in the community. It makes a lot of
sense for us to work with an organization that is well-respected,
does good work and is invested in the community.”

That’s why we’ve been working since 1968 to close healthcare disparities by
providing comprehensive, quality care to people living in some of Baltimore’s
lowest income and most challenging neighborhoods. Our Mission has been and
remains focused on improving the quality of life these individuals experience.
At Total Health Care,YOU Matter…
•	 You help us offer reduced-fee, sliding payment scales for patients who could not otherwise
afford medical care, substance abuse treatment, or mental health services. Some of these
folks remain uninsured; others are insured yet can’t afford co-pays and deductibles.
•	 You make sure we can provide free of charge medical care and supportive services
for patients most in need. Because of you, these folks do not have to choose
between food and medicine, or rent money and a visit to the doctor.
•	 You enable care for which no other funding exists – dentures so that an elderly patient
can eat regular food, counseling for the uninsured woman escaping an abusive
relationship and suffering from depression, salaries for community health workers
able to visit families in their homes to help make sure treatment and prescription
instructions are understood and to offer any assistance needed in following them.
•	 You provide desperately needed resources in the city’s most impoverished areas so that we
can reduce and eliminate barriers to health and well-being. Donor gifts pay for the teaching of

  • 3. Fiscal Year 2014 has been a year of excellence and growing connection to the communities we serve. We increased our clinical impact through various service expansions, including Optometry at our prominent Mondawmin Health Center location and increased specialized services such as pediatric dental screenings. Our Behavioral Health, Substance Abuse, and HIV services continue to be thought-leaders and among the most robust programs in the Baltimore region. We are also excited to expand our footprint as we are in the planning stages of opening a new health center at the Hebrew Orphan Asylum in the Rosemont community, partnering with the Coppin Heights Community Development Corporation. With community always at the forefront of what we do, THC has extended its reach to even more community partners with like missions and values and the same zeal to serve. As a participant in the West Baltimore Health Enterprise Zone through the West Baltimore Care collaborative, we affected positive change and clinical outcomes, fighting cardiovascular disease in our city. We continued with our various giving campaigns, including our Turkey Dinner Giveaway to families in area partnering schools and the Holiday Gift Drive for our patients. Fiscal Year 2014 still had its challenges. As we endured one of the most eventful winters our region has seen in decades, our patients were less likely to come into our centers for care on those heavy snow days and this yielded more closures than expected. We also experienced additional clinic down time from the transition of our Electronic Medical Records, a necessary investment and project as we continue to keep efficiency and quality at the heart of our operations. While these factors did impact our operations we still saw over 37,000 patients and can report positive net income for FY2014! We are encouraged about our work this past fiscal year and even more excited at the additional lives we will continue to touch through our efforts.   • 9. CESH (Center of Emotional and Social Health) Mental Health Services are just as important as physical well-being for total health. That’s why Total Health Care is committed to improving the mental health and quality of life for all members of the community. We provide high-quality services to those with physical barriers and behavior and mental health issues, working to improve the way they feel about everyday life. Our services comprise a variety of areas beginning with mental health screenings and including group and individual mental health counseling, nutritional assessment and therapy, and preventive health education. Outreach services identify and assist with navigation into mental healthcare treatment. Our team consists of licensed psychiatrists, psychologists, social workers, counselors, psychiatric nurses and public health administrators. Alcohol and Substance Abuse Treatment Services One invaluable program is Total Health Care’s alcohol and substance abuse treatment services. Overuse of alcohol, drugs or other substances is considered abuse, and Total Health Care has programs to help fight this community concern. The Total Health Care Substance Abuse Program is a state-certified, outpatient program that treats the whole person. This includes physically, psychologically, socially and spiritually. Our substance abuse case management services link on- site primary care, mental health, housing, and counseling services. HIV Medical and Mental Health Care Support group sessions provide additional support to patients with challenges to fully understanding their HIV condition by linking them to the full spectrum of HIV services available. Case managers help patients navigate services within and outside of Total Health Care so they can receive the total care they need to manage the disease and live productive and fulfilling lives. Behavioral Health Services What’s important to the community is important to us. Our standard core services are offered based on the community’s needs. Our patients depend on us at Total Health Care, and our foundation of care gives the community what is most needed. 9
  • 10. Patient-directed care is the core of Total Health Care. To help our patients direct their care, we developed our Patient-Centered Medical Home. It’s not a building, a house, or a hospital. It is a coordinated team of medical experts who make sure patients are their healthiest. We provide the team of experts and specialists to ensure the best care is available, and together with the patient, we develop a care plan based on that individual’s needs. The ultimate decisions are the patient’s. The care team gains access for patients to all needed services. This includes doctor visits, shots, testing, follow-up visits, home care services, medication reviews and discounts, and educational information like nutrition and self-care. This ensures patients are in control of their care. Patient Centered Medical Home 10
  • 11. Total Health Care’s Department of Pharmacy does more than dispense medications. We care for our patients, making sure that the medications they are prescribed are safe and the best choice to deliver maximum results. We monitor dosages and coordinate with the prescribing physician as well as our database to ensure the right medication is chosen based on the patient’s medical history. We provide patients with drug and disease information, medication therapy management programs and counseling services. In addition, our drug preparation and distribution services give the patient increased accessibility to customized medications. Patients choose from on-site retail pharmacy centers at six convenient Total Health Care locations, where they find the medications they need in name brand and generic forms. Licensed pharmacists are always on-site to provide prescriptions quickly, usually within 20 minutes, and counseling for patients who just want a question answered or a medication side effect explained. Our youngest patients often need the most attentive care. That’s why only board-certified pediatricians and family practitioners care for children at Total Health Care. Parents have peace of mind and children feel comfortable, whether they visit for flus and illnesses, asthma and allergies, skin diseases, sprains or strains. We also provide essential services to the community for minors like wellness physicals and vaccinations, confidential visits and contraceptive management. Our specialists have worked with children from birth through adolescence, and they become like family. Many children choose to remain with our pediatricians until their early 20s or as long as insurance permits. At Total Health Care, our pediatric services are provided in seven locations. Late hours, on-call services, same-day sick appointments as well as routine care are all available in our offices. Our specialty services include nutrition, diet and obesity, mental health and social work, sports physicals, asthma and allergy testing. It’s all here for our patients, in one place, at one time.   • 13. Happiness is best expressed with a smile, especially for children. Total Health Care is one of the city’s few community healthcare organizations committed to dental health for children. We provide pediatric dental screenings that include preventive care services and a focus on oral health education with dental literacy a priority for all ages. Diagnosis and treatment of cavities and dental conditions are handled at our centers and, if necessary, our dentists work in collaboration with our medical providers. Total Health Care hosts several dental screening events each year to improve the oral health of the community’s youngest members. Plans are underway to expand our efforts to open additional dental sites with a focus on our pediatric patients. Ensuring the Smallest Smiles 13
  • 14. Total Health Care continues to expand its services to provide comprehensive care for the community. In fall 2015, Total Health Care will open its ninth center, in the former Hebrew Orphan’s Asylum, located in Baltimore’s Rosemont neighborhood. The full- service center will offer enhanced pediatric and dental services and provide area residents with health education and preventive health programs to further improve the health of the community. The historic building’s foundation has been stabilized to improve safety and provide structural improvements. Total Health Care is pleased to operate within a gem of the community, which was built in 1874 as a Jewish orphanage. The mansion, which later housed West Baltimore General Hospital from 1923 through 1945 and the Lutheran Hospital of Maryland from 1945 to 1989, is listed on the National Register of Historic Places. At the City Center In Full Sight Responding to the community’s needs is paramount for Total Health Care. So it was only natural that optical services became a part of the health care organization’s expanded offerings for patients who reside or work in the area. Our board-certified optometrists and ophthalmologists work to make sure the eyes are just as healthy as the body. Services provided include comprehensive eye exams with and without refraction, glaucoma screenings and diabetic retinal evaluations. Our centers also offer a wide variety of eyeglass and sunglass frames that are fitted by our specially trained opticians. We offer lightweight, bi-focal, progressive, transition and digitally constructed lenses. We also provide custom-fitted contact lenses maximizing comfort for our patients.   • 17. The Health Enterprise Zone is a collaborative effort between local healthcare providers, Bon Secours Baltimore Health System, and West Baltimore Primary Care Access Collaborative. Partners in the Health Enterprise Zone are working to expand access to primary care for those living within it. This affords the creation of a community health infrastructure to serve residents in West Baltimore, a community facing some of the highest disease burden rates and worst social determinants of health care in Maryland. One of the program’s successes is the result of our Community Health Workers’ (CHWs) dedication — they are the experts that knit medical experience with patient lifestyle habits to create healthier living for individuals and families. Based upon need, available resources, and success, the outreach of the Health Enterprise Zone will expand to assist more persons. The ultimate goal is to foster self-responsibility for good health. Total Health Care is a member of the Maryland Maternity Access Coalition, which aims to create a Maryland Birth Injury Fund to compensate families and help care for children with permanent birth injuries. The benefits are to include nursing care, medical equipment and specialty care. The fund would be paid for by hospitals, OB/GYN physicians, and medical liability insurance companies to help families and protect access to quality maternity health care and eliminate the need for costly litigation.   • 19. All citizens have the right to receive healthcare services regardless of their ability to pay. Many community members do not know where to go or how to find healthcare services. To help, Total Health Care employs Community Health Workers to assist community members in gaining access to the care they need. Community health workers strive to match Baltimore residents with the specific healthcare services they need. They assist community members throughout the healthcare spectrum, from guiding them with healthcare applications to serving as navigators connecting them to case managers and clinicians, helping community members understand the need for healthcare prevention services and staffing health fairs to test for serious health conditions. Respect, professionalism and support are words community members use to describe their efforts. Moms Clubs at Total Health Care are clinic-based prenatal education support groups open to female patients at 34 weeks or less gestation. The program is designed to help pregnant women connect with important information and resources. Guided by the Baby Basics curriculum, which provides pregnant women with comprehensive and easy-to-read prenatal materials, the Moms Clubs empower them to effectively navigate the healthcare system and become better advocates for themselves and their families. Each participating woman receives a Baby Basics pregnancy guide and planner and agrees to attend six weekly sessions of the program at the health center. Participating women also receive one-on-one care coordination from THC’s Community HealthCorps members to address their individual care needs. Moms Club at Total Health Care was developed in partnership with Community HealthCorps Maryland, B’More for Healthy Babies Upton/Druid Heights and Promise Heights. In addition to clinic-based Moms Clubs, THC periodically facilitates school-based Young Moms Clubs for pregnant teens in partnership with Renaissance Academy and the Teen Pregnancy Prevention Initiative at the Baltimore City Health Department. Young Moms Club utilizes the Baby Basics curriculum, yet also incorporates family planning, career planning, and other age-appropriate topics.   • 21. A friend suggested Emmanuel Bartholomew contact Total Health Care for primary care services. Without insurance, he was hesitant about what he could pay. Emmanuel had diabetes; his sugar was a whopping 530 and his weight was up to 255 pounds. He could have been in a diabetic coma if it were not for his love of walking. Despite his initial hesitancy, Emmanuel followed through on his friend’s suggestion and connected with his local Total Health Care center. THC’s Dr. Samuel Williams got Emmanuel on medication, referred him for podiatry and ophthalmologic care, and put him on an exercise regimen. Emmanuel also met with a nutritionist, who helped him record his meals, watch his portions, and improve his diet. Total Health Care worked with Emmanuel to price his care based on what he could afford while simultaneously working to enroll him in insurance. Now with insurance, he can get his medication refill for just $14 and his TRU strips for $15. Everyone he’s worked with has been wonderful, he says. “Without them I don’t know where I’d be now,” says Emmanuel, whose blood sugar is down to 105 and his weight an improved 235. “I’m a patient for life.” PATIENT On Track to Good Health Emmanuel “I’m a patient for life.” 21
  • 22. Victor Parker first saw Dr. Arethusa Kirk when he was new to the area. He was just eight years old and needed a checkup. He was nervous about seeing a new doctor, but he soon realized there was no reason for concern. Dr. Kirk was friendly and made him feel at home. Today, at 18, he considers her a friend. They have a good relationship and he trusts her with his health. “She always makes me feel important,” he says. “Everything I have to say is important to her and she makes sure I understand everything we talk about.” Victor also credits the staff with making him feel comfortable. He laughs and jokes with them while waiting for Dr. Kirk. “I built something there I can continue going to,” he says.   • 23. The Gentleman’s Caregiver Tiffany Groover, M.D. PROVIDER “We truly care for everyone.” Tiffany Groover, M.D., saw a patient at the Men’s Health Center on North Avenue. He returned from the restroom looking pale, clammy and ill. His sugar level was through the roof, and his diastolic pressure was too low to measure. She felt like she was back in her residency training in the Intensive Care Unit. “He was close to cardiac arrest and dying and needed a defibrillator in his heart,” says Dr. Groover. “If I had let him walk away without being cautious, he probably would not have survived.” Dr. Groover has seen the patient frequently since and he’s doing much better. Getting patients the care they need is the main reward for Dr. Groover. Most are not that extreme, but they come in and tell her they’ve quit smoking or their medical condition is under control because they are taking their medications daily. Dr. Groover chose Total Health Care for her medical practice because of THC’s commitment to improving the health of the community. She trained to care for the underserved, and she thrives on caring for the Baltimore community. Her stellar reputation in the community for providing preventive care, education and care for chronic illnesses for men of all ages has seen her practice grow. “We truly care for everyone,” she says. “Everyone needs care.” 23
  • 24. Forever a Caregiver Teonya EMPLOYEE “…I realized this is where I need to be, this is where the need is.” She remembers being just eight years old, and her mother was quite ill. The young girl told her mom to lay down. She would take care of her. Since that moment, Teonya Perry-Barbour wanted to be a nurse. “I’ve always been the one to patch everyone up,” she says. That little girl who started out patching up dolls, and relatives, and playing nurse with her cousins grew up and never gave up on her goal. She started as a Geriatric Nursing Assistant then progressed up the career ladder to Certified Nursing Assistant then LPN in a float pool and ultimately earned her RN. “I’ve always loved nursing; it interested me to take care of people, nurture people,” she says. “It was my calling.” Teonya joined Total Health Care in 2011, and today serves as clinical coordinator at the Immediate Care Center on Division Street. She’s the friendly face who greets and triages patients, making sure to get them what’s needed. She manages the center and oversees an LPN and three medical assistants. Her impact is great and she can see the results of her labor, as a recent patient can attest. The woman presented with serious health issues including hypertension. With no job and no health insurance, she could no longer see her primary care physician. Teonya pulled her aside and the woman burst into tears. She calmed her down and talked to her. Teonya got her back to see the physician and the medications she needed. The woman was grateful. “That’s when I realized this is where I need to be, this is where the need is,” she says. 24
  • 25. For two years, Total Health Care has partnered with Promise Heights to care for the Upton/Druid Heights area of Baltimore. Promise Heights, a Promise Neighborhood administered by the U.S. Department of Education and operated by the University of Maryland School of Social Work, aims to improve the educational, social, health, and economic opportunities of children from birth to young adulthood in the West Baltimore neighborhood. Total Health Care is committed to improving the health of the area and is partnering with Promise Heights to build a pipeline of services for children from before birth through college. Among our offerings are prenatal services, health education and screenings, pediatric services, parenting workshops, moms clubs and physician-led classes. “They are a community institution and the people in the community really trust them,” says Bronwyn Mayden, executive director of Promise Heights. “They develop strong relationships with people in the community.
