Avatar Based Health Assessment
Goal
 This app aims at engaging the citizens in a
health oriented game while at the same
  time tracking their health information.
 The app will function based on
personalized avatars that are set up after
the app is installed. The avatar will be set
up through an initial survey to personalize
the graphic of the avatar and establish an
  information collection system.
Goal
 Once the avatar is set-up (having
completed the initial questionnaire), it will
be released into the “Avatar World” that
functions in terms of badges to collect
  health information.
 Each badge will also unlock a mini-game.
 All the information they provide will be
  collected into the RwJ databse.
Software Description
 Our software is going to be created using Unity game
    engine.
 we can create more realistic avatars and also giving the
users an illusion that this software is more of a game than
    a questionnaire.
Data Generation Description
 Each question on the survey will be a given a unique value
and based on either the user will answer positively or
negatively to that certain question, the value would
    change.
 That value would be uploaded to our server and by using
an automated script we convert those values to an Excel
script that would be added to National County Health
Rankings data( 2012) that is uploaded in our server.
 There would be an automated script that would make a
copy of the file every Hour, in case anything goes wrong
    with the actual file.
Community Deployment
Approach
 The initial community relation will be
located through the “location” in the first
  survey
 We should get doctors to do this first and
recommend this to their patients as a
good health tracking application that can
totally be used to suggest diets, exercises,
  medicine information etc.
