CHALLENGE
Wraparound Services supports hundreds of children and teenagers
under the age of 21 who experience complex social, behavioral or
mental health challenges. The organization delivers wraparound
services in Northeast and Southcentral Louisiana, and Tennessee.
Founded in 2012, the organization identified a few challenges that
were impacting their ability to work in a system of care.
Wraparound Services needed a way to better support child and family
team collaboration in alignment with evidence-based practices for
high fidelity Wraparound. Before implementing an electronic health
record, this meant each Plan of Care needed to be printed and
physically signed off by every team member. Paper documentation
could get misplaced, and staff spent a lot of time visiting team
members to get their sign-offs. With two locations on opposite
ends of the state, the organization needed a centralized place to
keep their records and to track results. Wraparound Services was
looking for an electronic health record that would help them boost
efficiency and enable more team member participation - all within a
short implementation timeframe.

Streamlines Care Coordination
with FidelityEHR
SOLUTION
Wraparound Services took interest in FidelityEHR after seeing
how the platform directly aligns with the Wraparound care
coordination model. After considering other electronic health
record options, Wraparound Services implemented FidelityEHR
in the fall of 2015 and half of their facilitators in Northeast
Louisiana started using the system. They quickly saw the value
in using the electronic health record.
Implement once, expand indefinitely
FidelityEHR’s team led the implementation process and provided
training to the initial team of supervisors and care coordinators.
Based on the positive team feedback, Wraparound Services
rolled out FidelityEHR to all of their Child and Family teams in
southcentral Louisiana in early 2016. By late spring, they began
expanding their service network to southern Tennessee. After
the initial training in 2015, Wraparound Services was able to roll
out FidelityEHR in two more regions with minimal additional
training. FidelityEHR users reported that the platform’s ease of
use simplified the training process.

Remote Accessibility: By being able to access data quickly from the field or from home, Wraparound
facilitators are able to work more efficiently.
Quick team collaboration: Wraparound staff also saved time while designing and completing the
wraparound Plan of Care, scheduling team meetings through the EHR, and storing feedback and notes
from the family, youth and other team members.
More high level reporting: FidelityEHR users have been able to gauge the Plan of Care’s success and
impact on the youth and family through customized dashboards and assessments.
“It makes our work easier”: Care coordinators and supervisors report that FidelityEHR reduces intake
and everyday care coordination management time, and helps streamline their day-to-day wraparound
tasks.
