3. Case Details
• Right sided abdominal lump for 3 months
• Right sided vague abdominal pain for 15 days
• Abdominal fullness and anorexia present
• Normal bowel and bladder habbit
• Known diabetic and hypertensive
• Metformin, Linagliptin and glimepiride
• Losartan and atorvastatin
• Chews tobacco left few months back
4. On Examination
• Vitals = stable
• General condition = good
• No palor no icterus no edema
• P/A: lump of size 20*20cm at right
lumbar region, soft to firm in
consistency, globular, mobile laterally
but fixed with posterior surface, non
painful, no features of IVC
obstruction
• DRE: normal
• Other systemic examination normal
9. • Diagnosis : Retroperitoneal mass ?liposarcoma or AML
• Plan : exploratory laparotomy and excision of mass
10. OT findings
• About 20*20cm retroperitoneal mass ,with
irregular margins, solid to cystic , semi
mobile , inside cocoon along with small
and large intestine causing pressure effects
• Adherent to IVC, right gonadal vessels,
right ureter and right kidney without
invasion
• On cut section : lipomatous lesion with
central fat necrosis surrounded by thick
irregular capsule
