Objective
To obtain a position as a licensed physical therapist in an outpatient clinical setting. I wish to utilize my
knowledge and passion for this profession to improve my patient’s function and overall quality of life. It is my

Providing outpatient orthopedic rehabilitation evaluations and interventions for various musculoskeletal
diagnoses for patients of all ages.

Primarily provided physical therapy interventions in the Medical ICU setting while also spending time
in inpatient rehabilitation, post-op orthopedics, pediatrics, post-natal, and oncology settings.
Initially assisted Clinical Instructor with patient care eventually leading to independently handling a full
patient load for the entire second half of the rotation.
Collaborated with inter-professional team in order to optimize patient care and decrease overall patient
stay.
Gave personal opinion on patient placement status at discharge via observation and use of specific
outcome measures.

2. Performance Rehab ▪ 8 weeks (January 2015 – March 2015) ▪ Overland Park KS.
Initially assisted Clinical Instructor in the evaluation, examination and assessment portions of patient
care that eventually progressed to my handling of a full patient load independently.
Provided an in-service presentation regarding osteochondritis dissecans of the radio-humeral joint for
the purpose of gaining a more in-depth knowledge to apply to a complex patient of ours.
Independently completed documentation via WebPT using CPT, ICD-9, and Medicare billing codes.
