Introduction / Background
The Statement of Work (SOW) is a document which describes the scope of work
required to complete the project. The SOW will contain an appropriate level of detail
so all parties clearly understand what work is required, the duration of the work
involved, what the deliverables are, and what is acceptable. This section provides a
general description of the project as well as highlight the project’s background and
what is to be gained by the project.
The information in this Statement of Work (SOW) may not be disclosed outside of North
Corp Consulting and may not be duplicated, used or disclosed in whole or in part
forany purpose other than to evaluate the SOW, provided that if a contract is awarded
toNorth Corp Consulting as a result of or in connection with the submission of this
proposal, Patanjali Ayurveda Hospital will have the right to duplicate, use or disclose
the information to the extentprovided by the contract. North Corp Consulting retains
ownership of this SOW.

2015[“Agreement”], executed between Patanjali Ayurveda Hospitaland North Corp. Consulting.

Note: Patient will carry appointment slip to the doctor.
Process B – Diagnosis Process (At Doctor’s Desk)
B(I) Patient Queue:
 Basis on the appointment number doctor will create a case history for
Patient.
 Prescribe the patient
 Recommend either the test(s) if required and/or basis on the reports
collected from pathology (post-test) suggest medicine or directly suggest
the medicines basis on diagnosis done.

B(II) Patient Communication Queue:
 Doctor will communicate with the patient for the treatment follow up.
 After follow up status will be updated by the doctor.
B(III)Alerts Manual:
 Doctor will send an SMS to the patient (in case unable to reach over call) for
communication follow up.
Note: Doctor will scan and upload the prescription and pathology reports (if any) to
local DB for each patient. Also will generate “Digital Prescription” based on Aushadhi
Darshan book.
Process C – Pharmacy
 Capture the patient’s prescribed medicine details
 Medicines prescribed for how many days
 Patient purchased medicine for how many days
 Payment Details
 Update the follow up status to Doctor
Note: Pharmacist will update the medicine purchase details to local DB for each patient.

Mobile App
Mobile App will be developed for Patient support.
 From SMSC we will find out which cell phone customer is carrying. (Smart
phone or ordinary phone).
 Smart phone holder patient should get a WAP link to download the Mobile
App.
 Now patient will be able to perform following tasks:
 Book an appointment
 Track the treatment history
 View prescriptions by doctor
 Order medicines by consulting doctor
 Can raise a query to physician and vice versa

Introduction
The purpose of this document is to identify the solutions, deliverables, schedules and
Project management expectations for developing and implementing this project. This
document will cover the part that deals with specific requirements of the project.
Purpose of this document is to record the agreed scope of work and deliverables with
the user function and obtain the sign off from the Key user and Business Sponsor as an
acknowledgement of the agreed scope and deliverables.
Executive Summary
The proposed solution is a step by step approach for managing and maintaining
the complete OPD (Out-Patient-Department) functionalities in an IT based
solution, The solution is specifically designed to meet the requirements of client for
managing the individual and centralized hospital management systems. Right
from the patient registration, physician’s consultation and up to the patient follow
up with respect to the treatment provided and the feedback evaluation all
sections will be handled and managed by this solution.

Objectives
The objective of this project is to provide crystalized view of the activities
being taken out as a part of OPD at individual and centralized hospital(s). It
also provides a way to connect with patient beyond the conventional
methods of treatment by connecting with patient more harmoniously.
Keys to Success
The access will also be available through the Mobile Apps developed for
the software. This will provide a lightning fast access to the hospital and the
services provided by them. This will speed up the patient registration
process and also provide other additional benefits to the patients.

Project Significance
The project aims to provide a platform for PATANJALI AYURVEDA HOSPITAL
to have an organized environment for OPD record management and
follow up over treatment supplied. With this a detailed vision can be
brought out basis on the data feed. This will be a useful input to the
forthcoming implementations and innovations in medical.
Project Scope
The scope of the project covers the high-level shared vision of the
architecture and scope of the proposed IT system. Also to communicate all
stakeholders a conceptual understanding of the target IT system. This also
includes to describe the overall architecture to provide a conceptual
overview of major components of the system and interactions among
these components in an architecture.

NORTH CORP CONSULTING’s SOW
This Statement of Work describes the work to be undertaken by NORTH CORP
CONSULTING under the terms and conditions of this project for PATANJALI
AYURVEDA HOSPITAL. The tasks to be performed by NORTH CORP CONSULTING
and the Deliverable Materials are defined and an Estimated Schedule is
provided. In addition, the responsibilities of client are listed. Changes to this
Statement of Work will be processed in accordance with the procedure of
Project Change Control.
The investigation and the implementation of changes may result in modifications
to the Estimated Schedule, Charges, and other terms of this Statement of Work
and/or the Agreement.
