Summary or Experience and qualifications:

I have been caring for a termanally ill, providing Hospice care, Mental Health care and all cares
of daily living much of last year and this year appoximately 6-7 days per week.

Worked at many different facilities i.e. ER, Day Surgery, Behavior Health in-patient, Chemical
Dependency units, i.e. CODA, Telecare, Providence Adult Care Mental Health Acute care
inpatient Providence inpatient medications as prescribed. I have charge RN experience in which I
was responsible for many patient in both long term care (senior and rehabilitating Facilities i.e
Prestige and Avamere) and psychiatric residential facilities I was successful in delegating other
RN's and LPN's, techs.and CNA's appropriately. In addition I was employed at Cedar Hills
Hospital because of my work through the Agency and started working there without applying for
the position and with no orientation by Cedar Hills Hospital for a FT position by COO, CON,
Supervisor and Manager of Nursing due to my outstanding work via working there via temp.
nursing. I have also been asked by several long term care units to become an employee as well,
but was unable to at the time.I also have worked with children at Schriner's, Dornbecker's, Perry
Center, Telecare, kerr Center for children. I have a stack of over 150 perfect evaluations that you

As Charge Nurse I made sure that patients were kept safe and evaluated their suicide ability
and other behaviors that were dangerous to themselves or others and notified their Providers and
others on the patients team as needed. I am knowledgeable in assessing patient using COWS and
CIWA's and medicate as order. I am also able to administer Suboxone and Subutex as ordered
and needed by assessment and MD's order. I report any changes in their behavior and evaluated
if medication was helpful by evaluating patient's CIWA'S or COWS after medication is given.
Attended Team meetings and shared my observed and documented information so the team was
able to provide the next step, how medication changes were helping (Both Psychotropic and
Medical Medications).Talked with patients families with regards to HIPPA regulations.
I also was successful in talking with patients on a 1:1 basis and able to calm them and in most
cases I was successful with this and I truely enjoyed helping patients in this manner.
Frequently was leader of groups and researched on my own time and got approval of many
topics for groups.

Worked on Neurologic, Neuro/ Trauma and Ear, Nose, Throat Unit
Unit involving pre-op, post-op, Chemotherapy,
Ventilator Cares including suctioning NT, oral and new laryngectomies from laryngeal or
esophageal cancers. Also maintained patients on ventilators, CPA P's, and respirators..
As Charge RN I was responsible for making assignments on days of work and make certain
that the assignment fit the RN's abilities and in addition to make certain that the patients were
taken care of professionally.
Talked with MD's, families, and other health care personnel to evaluate and treat as MD
ordered and attended discharge planning meetings an and discussed patients post care needs, etc.
Cared for comatose and semi comatose patients, requiring total care.
Paraplegic and Quadraiplegic patients. With experience in transferring the patient by self..Also,
