1. OEDEMA [NEPHROTIC AND NEPHRITIC SYNDROME]

HISTORY OF PRESENT ILLNESS
The patient came with generalized swelling of the body for the past _____ days,
it is the ____th
episode over the past 6months/ 12 months which appeared after ___wks of “completion of the earlier
treatment / past episode” or after cessation of the “daily regimen / complete treatment” ( in cases of
relapse)
the swelling first appeared in the face followed by the Limbs and then lastly involving the abdomen,
it was gradual in onset progressive in nature.
Swelling is bilaterally distributed usually appearing in the periorbital region during the early
morning hours which gradually reduces in the daytime and is present throughout the day
and there is also bilateral symmetrical swelling of the legs more prominent during the evening hours,
recently the patient developed distension of abdomen over the last ____ days,
there is also associated scrotal/vulval swelling ( Nephritic ruled out)
His/her caregiver also complaints of reduced urine output and
dark red colored urine( Suggestive of Nephritic)
negative h/O-
There is no history of any sore throat or recent skin infection( if present Suggestive of nephritic~ PSGN),
no feeding problems (PEM ruled out)
There is no associated history of Dyspnoea, PND (CHF ruled out, in CHF legs involved first followed by abdomen, if
Dyspnoea present nephritic can be suspected ) or
yellowish discoloration of urine and sclera( Hepatitis ruled out, abdominal swelling appears first).
There is no history of recent IV fluid administration prior to the hospital admission (overhydration ruled out),
there is no history of any urticaria, any food allergy or any history of insect bite (angioedema ruled out)
The pt belongs to a non endemic zone of filariasis
HISTORY OF PAST ILLNESS
There is no such similar history in the past or
There is a history of similar 2 episodes in the last 6 months, the 2nd
episode appeared after stopping the intake of
medication given during the 1st
episode / after shifting to alternative day regimen from daily day regimen / after
____weeks of the 1st episode
No h/o of TB, DM ,HTN ,Filariasis ,URTI ,Malaria etc
