Anyone present at the photoshoot,
by tripping falling slipping. As the
floors is very hard head injuries
could become present.
Lennie as he is jumping in the air he
could pull a muscle when jumping
Lennie is jumping and if the floor is
wet he may slip causing head or
body harm and injuries
Camera is electrical and therefore
can be damaged easily by the rain
as not water resistant
Move all hazards and wear
appropriate shoes so no one slips
and also make sure to check the
basketball net and pole is secure.
Be careful when jumping
Don’t walk on slippery surfaces,
and make sure to keep eyes out for
others to make sure no one gets
hurt
Keep camera in bag when rain is
apparent, and keep under cover
when in use
Keep eyes out for anyone who is
slipping or near a hazard and
make them aware
Make sure that he stretches
before jumping so the risk of
pulling a muscle is much lower
Keep away for places where floor
can get wet
Do not take camera out of bag if
wet
a SEVERITY b LIKELIHOOD c FINAL SCORE RATING: what needs to be done
5 Dying or being permanently disabled 5 Will almost certainly happen 16-25 Stop! Do not start activity again until risk is

2. 4 Serious injury/long term illness 4 Highly likely to happen 10-15 High risk level, High priority. Take action
straight away to control the risk
3 Temporary disability/3 days off sick 3 Not so likely 6-9 Medium risk level. Tighten up controls and
make a plan to do something about risk
2 Will need medical attention 2 Even less likely 3-5 Fairly low risk level. Low priority but keep
possible action in mind
