The WGC Prepared Card is the Philippines' premier microinsurance card to be used for emergencies such as emergency hospitalization (emergency room only), fire, earthquake and others. The WGC PreparedCard is owned and managed by Western Guaranty Corporation and made available online through Heartshaper Asia Inc.'s e-commerce facility.

Membership Eligibility: 5 years old to 65 years old
An individual is allowed to enroll and activate only 1 card at a time,
up to a maximum of 3 cards within a period of 1 year.
“One-time availment“ provision of the WGC PreparedCARD will
pertain only to the Emergency Room Availment.
In case of availment of the Emergency Room Benefit, the rest of the
unavailed insurance benefits will remain in force. A certificate of
cover will be issued to that effect.
In the event that the same cardholder purchases and activates
another card, the insurance coverage (ADD, Fire, Earthquake) will
accumulate.

7. HOW TO ENROLL

1.Scratch off
to get the PIN number.
3.Successful SMS membership enrolment if all fields are
valid and completed, and if you have replied “Yes” that
you agree to accept the Company’s terms and
conditions contained in the master Policy found in our
website, otherwise, follow SMS instruction for invalid
registration.

Once activated, your card will be valid for one (1) year. The coverage of this card shall
be terminated upon expiry (one (1) year from date of activation), except for the “Use
of E.R. for Emergency Cases” Coverage, which is good for one-time use only.
Cards not activated after 90 days from date of purchase shall be considered expired.

9. Surrender your WGC PreparedCARD, together with a valid ID
at the Emergency Room of any Cocolife Healthcare accredited hospital.

FOR NON-ACCREDITED HOSPITALS:
Settle the bill and secure documents (receipts, Medical Certificate, WGC
PreparedCARD together with a valid ID) for submission to
Western Guaranty Corp.

10. Any sudden or unexpected onset of illness or injury
Reasonably appears to have the potential of causing disability
or death at the time of occurrence
Requires immediate alleviation of severe pain and discomfort
Requires urgent medical or surgical care right after or no later
than 24 hours after onset

11. Ambulance service to transport a non-ambulatory patient
the nearest medical facility covered up to Php.5,000
(inclusive of coverage limit)
Emergency room fees
Professional services of accredited physicians, surgeons,
and specialists
Medicines used for immediate relief and treatment
Intravenous (IV) fluids and whole blood and human blood
products
Dressings, plastic casts, sutures and other supplies
X-ray, laboratory examinations and diagnostic procedures
Anti-tetanus equine serum (ATS), tetanus toxoid, anti-
rabies injection, and anti-venom vaccine for immediate
treatment covered up to Php.10,000 (inclusive of
coverage limit)

shall be made available for Non-Accredited, through
reimbursement

is packaged by a folded flyer.
Do not accept if plastic wrapper is
opened, or if PIN number scratch area is
