Dear Hiring Manager,
I, Anthony Cerimele, am the lead medical radiographer at Paramount Surgery Center. I had the
privilege of working with Rachel Garrett for a year and a half. Rachel was hired as the OR
tech/CNA in the surgical aspect of Paramount Surgery Center, Mesa for over a year.
I was initially impressed with Rachel’s abilities to understand medical terminology and her
thorough comprehension of the human body. Rachel was attending nursing school at Eastern
Arizona College but despite the heavy load she was fully trained to manage four operating rooms
and a pain room within one week of hire. Rachel was persistent to help and determined to learn.
Rachel was timely with her shift and always greeted everyone with a smile. She was seen helping
the staff daily with tasks within her scope of practice. She was quick to respond to the room turn-
over’s and could take down and set up the operating room under 15 minutes by herself. Although
most of Rachel’s tasks were done independently she enjoyed assisting the nurses and surgical techs
with a team approach.
Rachel is honest and full of integrity. Before the end of the day she would take extra time after her
shift to help stock and organize the operating rooms to prepare for the next day. She responded to
each patient with kindness by offering them warm blankets before and after their surgery. Rachel
would seek out answers when she wasn’t sure and was careful to maintain accurate information.
The surgery center became cleaner and more organized as Rachel continued to work with us,
maintaining Paramount’s high surgical standards. She instigated strict cleaning of door handles,
light switches, keyboards, and telephones at our facility with approved surgical antibacterial wipes.
On behalf of the staff at Paramount we all saw her as having a bright future in the Nursing field.
I highly recommend Rachel Garrett RN for your medical facility. She is an exemplary candidate
with a determination for success and a passion for helping others.
Best regards,
Anthony Cerimele

To whom it may concern:
Rachel Garrett worked under my supervision as a certified nursing assistant (CNA). She readily responded
to supervision with minimal instructions given. Rachel could perform her duties, responsibilities and tasks
with thoroughness and with little to no supervision required. She had excellent job attendance. Once
given her assignment, she would reliably and efficiently perform. She had the ability to follow
instructions that were given once and did not need repeated instructions. She is congenial and pleasant
to work with and respectful in her interactions amongst team members. Rachel is kind and patient in her
dealings with staff members and patients alike. Rachel would be a positive asset to any organization.
With best regards,

To Whom It May Concern,
I am happy and honored to write a letter of recommendation for Rachel Garrett. She was a nursing
student for Med/Surg whom I taught. She graduated at the top of her class, from Eastern Arizona
College in Thatcher, Arizona. Rachel traveled from out of town each week to attend classes and
then traveled to the clinical sites to complete the clinical components while working in a hospital
surgical area setting during this time. She was the student who consistently studied to achieve
understanding. She was respected by her peers and lower classmen for her nursing knowledge and
skills. Her positive attitude and desire to learn made the clinical experience positive for assigned
patients and herself. Out of town clinical experiences at the large hospitals in the Phoenix area
also sent positive reports which were very complimentary from the clinical staff and the Clinical
Instructors. She has a very compassionate personality.
She was an excellent student, compassionate to all and an honor to have known. She will be an
excellent employee and nurse to provide care for any patient. Teaching Rachel was an honor for
me, getting to know her made me appreciate how much young people can teach us as well. She
will always have my support in any endeavors she chooses.
Any questions or concerns may be directed to me at 1-928-322-7655.
Respectfully,
Linda Lopez, RN, CDE
