The outbreak of dengue in the city is taking a toll on diabetics and
heart patients. According to hospitals, those with comorbidities are in
panic mode since many of their prescription drugs tend to react
adversely with fever medications.
Maintaining fluid balance in such patients becomes a challenge for
doctors. “Dengue causes fever and increases metabolic rate, which can
lead to a rise in fluctuation in blood sugar levels. If it is not monitored
properly , there is high risk of such patients developing serious
complications,“ said Dr Randeep Guleria, professor of medicine at
AIIMS. He said heart patients take blood-thinning medicines, which
may have to be stopped to check bleeding due to dengue infection.

According to Dr Anoop Misra, chairman, Fortis CDoc Centre of Excellence for
Diabetes, Metabolic Diseases and Endocrinology , the number of diabetics and
heart patients requiring admission is lesser compared to the non-diabetics but their
complication rate is higher.“In many cases, such patients have to be admitted for a
longer duration to ensure that their vital parameters are stable,“ he said. This year,
Delhi has witnessed a sudden upsurge in dengue cases. Municipal corporations,
which maintain the data, state that two persons have died so far due to the
mosquito-borne disease.
“Timely medical intervention and monitoring can help save many lives. We advise
all our patients with history of diabetes or heart disease to report to the doctor if
they have high fever for more than 24 hours. Depending on the condition, the
dosage of medications may require to be altered,“ said Dr Misra. Doctors said
insulin therapy is considered safer for diabetic patients reporting dengue symptoms
because oral medications, metformin and DPP-4 inhibitors, may cause adverse
reaction.

For those with type-I and type-II diabetes with high blood sugar, urine
ketones should be checked to see if acid levels are up in body , said another

The mosquito-borne disease can cause congestion in the eyes and, in rare
cases, retinal haemorrh ages. Last year, TOI had reported how three dengue
patients partial or complete loss of vision at a hospital in Wadala, Mumbai.
AIIMS doctors said they have also come across sporadic cases of patients
suffering from loss of vision, partial or complete, due to dengue.
Research shows the time of onset of ocular symptoms can range from two
days to five months from the start of fever. But in maximum cases, the
damage to the eye is seen when platelet count drops severely .
