Our workflow management, speech recognition
and digital dictation solution, ‘SpeechReport’ has
been developed in partnership with healthcare
organisations across the UK.
SpeechReport offers the latest innovations in
speech technology, provided by Recognosco and
powered by Microsoft. It is easy to deploy, with
a very low network footprint and has a host of

SpeechReport supports healthcare organisations
to create, edit and manage dictations and patient
information. The clinic work list allows you to
organise your work throughout the day, providing a
digital overview of appointments and tasks. A HL7
integration with your clinical system guarantees
that patient data is immediately at hand.
SPEECHREPORT CURSOR
Speech recognition for any text field including
Microsoft Word and Outlook
SpeechReport Cursor enables you to use advanced
speech recognition capabilities and dictate directly
into any clinical, non-clinical or Windows-based
application (including: EPR, MS Word, MS Outlook
or any other third-party solution).
A small dictation window collates the dictated
text and enables you to rewind and replay your
dictation. There are also auto-texts, voice guided
navigations and correction capabilities available
within the solution. Upon completion, you can
quickly and simply direct the dictation with just
one click on your dictation device or keyboard. The
auto-text features can assist with creating further
efficiencies by enabling you to create standard text
blocks with input fields, to minimise the amount of
dictation required. This is extremely helpful when
regularly repeating large sections of text.

Keep track of KPIs with our reporting tool
G2 Analytics is our reporting tool that provides
managers with insight into: backlogs, department
turnaround times and employee performance.
Enabling teams, hospitals and Trusts to monitor
where there are bottlenecks, training requirements

Introduction
Patients with mesothelioma say that having reliable information about the condition and
receiving good care at home, from family members alongside community nurses, can
make a huge difference to their experience of the illness. The diagnosis of mesothelioma
usually comes as a shock, but those affected by this illness are not alone: this handbook
has been developed in collaboration with people who have mesothelioma, family
members and healthcare professionals involved in their care and members of asbestos
victims support groups.
This handbook is written with the intention of providing straightforward and
comprehensive information. Professional knowledge of the condition is variable as it is
very uncommon in some areas. However, the information provided here is based on
considerable experience in the medical and nursing care of sufferers from mesothelioma
and in the support of their relatives. People with mesothelioma and their family carers
want different types and quantities of information. Some people with mesothelioma might
feel that they receive sufficient information from their doctors. Family carers are ‘partners
in care’ with community healthcare staff and, as such, should have access to as much
information as they want and need.
The term ‘family carer’ is used throughout the handbook for those close relatives or
friends who might be involved in the care of someone with mesothelioma, for example,
wife, husband, daughter, son, step-relative, partner or close acquaintance. Many close
relatives have said that they do not regard themselves as ‘carers’ as they are simply
‘doing what you naturally do in a family’. However, for convenience and brevity, the term
‘family carer’ is useful. It also distinguishes ‘informal’ carers ie family or friends, from
‘professional’ or ‘formal’ carers ie those who are healthcare professionals.
This handbook is not meant to be read cover to cover, you can chose how much or how
little you need to read, according to your situation. Particularly at the time of diagnosis
there is a lot to take in, and so you might prefer to dip in and out of the handbook. There
is a lot of information presented here because mesothelioma affects different people
in different ways. Every effort has been made to use straightforward language and to
explain any specialist terms. If you find some of the content rather difficult or technical
please discuss any queries with your Macmillan or lung cancer nurse, GP or specialist.
You can use the book to prompt questions that you might need to ask.
Information that is mainly for nurses and other healthcare professionals is placed
at the end of some sections and has a purple background and page heading. The
term ‘community nurses’ includes District Nurses, Practice nurses and Hospice at
Home nurses; other healthcare staff such as GPs, junior doctors, physiotherapists and
occupational therapists might also find this handbook useful.

Symptom control
Mesothelioma is associated with multiple symptoms as shown in the table below. Most
patients experience breathlessness and pain at some stage, to varying degrees.
Symptoms in pleural mesothelioma in the last year of life (Clayson H, 2007).

It is not sufficient to apply lung cancer care to pleural mesothelioma because,
although the incidence of breathlessness is about the same, pain is more common
in mesothelioma than in lung cancer (see table below), and sleep disturbances and
impairment of social role and functioning are also more common in mesothelioma
(Nowak et al, 2004).

Management of breathlessness
Most people with pleural mesothelioma will experience at least one episode of pleural
effusion. Pleurodesis is performed in order to prevent recurrence and is most successful
when performed early in the illness. If the mesothelioma has caused extensive thickening
of the pleura, is encroaching into the lung resulting in reduced lung volume, or causing
a fixed lung, then a pleurectomy/decortication (PD) procedure might be the preferred
surgical option intended to restore the free movement of the lung and thus relieve
breathlessness.

For many years, Defined Benefit workplace pension schemes were the norm,
providing an income for life based on your salary. Today, Defined Contribution
(DC) pension schemes have become the main way that UK employers help their
employees to be better off in later life.
Between 2007 and 2014 private
sector membership of occupational
Defined Benefit (salary-based)
schemes shrank from 2.7m to 1.6m.
At the same time membership of
occupational DC schemes grew from
0.9m to 3.2m. In addition, based on
HMRC’s Personal Pension Statistics,
there are around 6m employees

The money a member gets out of
their DC pension scheme depends on
many factors. These include which
provider and scheme the employer
chooses, how the scheme’s set up,

money’s invested.
Not all DC schemes are equal. Some

So, after 18 long years Scott finally said...
‘How about we go and get wed?’
Without further ado Rachael said ‘yes’,
then onto deciding the perfect white dress.
A big church wedding just wouldn’t be right,
thank God they agreed and avoided a fight.
So to Costa Rica it is but only those two,
and on the 12th November they will say ‘I do’.

Brooms Farm was the heart of the village in Baldesrby St James, creating
work for most of its occupants. I loved it and would spend most of my
childhood in the fields with the men picking potatoes in winter, by hand,
hoeing sugar beat in spring, and pulling wild oats in early summer, then
harvest. It was a cycle, a life cycle.
There were no corner shops or streets to hang out on. You worked. Whether it
was on the farm or at home. You worked. That’s just the way it was. I loved
it that much, that I stayed and worked on the farm until my late twenties
when I left to work in forestry. Old fashioned farming is in my blood!
The food we ate was simple but, my word, it was tasty. We ate fresh
vegetables from the garden, and meat mostly from the land; rabbit or game
pies being my absolute favourite. I can still feel the feathers tickling my
nose as a small boy as I would sit on the step plucking pheasants ready
for game pies.
When Dad had the cash to pay a trip to the butchers, he improvised using
cheap cuts of meat; chuck, shin, ham hock, heart and tripe. God I could never
get used to tripe!
Dad took care of six lads on his own on a shoestring, making do and mend
but we never went without. I’ve created Farm to try and recreate some of my
childhood memories; the food and design, a nostalgic feeling of warmth, well
being and security.

Indie - a 12 month old Border Collie
from Wigan - has recovered from a
life-threatening condition caused by a
congenital inability to absorb vitamin
B12 (termed Immuslund-Grasbeck
syndrome in people) thanks to the
timely intervention of ChesterGates
Veterinary Specialists, who correctly
diagnosed his condition.
Janet and David Royle took Indie on
as a puppy almost a year ago after
buying him from a local farmer. The
tri-colour pup was initially bouncy and
happy but did suffer from stomach
upsets.
The Royle’s changed his diet and
things improved for a while but then
followed a catalogue of infections
DOG SURVIVES A RARE ILLNESS RESULTING FROM
VITAMIN B12 DEFICIENCY
from conjunctivitis to kennel cough and
water infections. A severe infection later
resulted in Indie being hospitalised.
After another brief recovery, Indie took
a dramatic downturn and collapsed in
a state of horrific distress with diarrhoea
everywhere. The Royle’s rushed Indie to
a pet hospital in Manchester where he
was put on a drip in intensive care and
treated for gastroenteritis and sepsis.
Janet said:
We were told that he was unlikely
to survive the night, but thankfully he
was still with us by the next morning.

Staff were unable to reach a diagnosis
to explain why these infections kept
recurring so they referred Indie to
veterinary specialists, ChesterGates in
Chester.who immediately ran tests and
diagnosed his condition as sepsis with
low blood sugar and low calcium, in
turn related to the deficiency of vitamin
B12, caused in this case by a genetic
inability to absorb the vitamin.
Janet Royle said:
ChesterGates took him in and
ran tests on him. We were really
impressed that they had read
his notes and taken in all the
information we had provided.
They were really thorough and
prescribed 12 weeks of anti-biotics.
One of their tests - a genetic test
revealed that he had vitamin B12
deficiency.

THESE PETS SURVIVED AGAINST THE ODDS
AND WENT ON TO WIN A BRAVERY AWARD
ChesterGates’ Veterinary Surgeon, Ellie
Mardell, MRCVS, explains:
Inherited selective B12 deficiency
is caused by the genetic inability to
absorb the vitamin from the small
intestine. It is an uncommon but
recognised condition in certain breeds
of dog including Border Collies.
Usually, dogs present with vague
signs, including failure to thrive, or
neurological signs secondary to the
absence of B12.
Indie’s presentation was much more
unusual, and at the time, I had only
seen it in one other dog. We know that
low B12 levels often lead to diarrhoea
and poor gut function in dogs and
cats, and I suspect that Indie’s gut
had become so compromised, that
bacteria from the intestine were able
to enter the blood stream, causing
sepsis and secondary low glucose.
The low calcium I suspect was due
to sepsis, and also the poor gut
function, leading to reduced calcium
absorption. This case required some
detective work!

David said:
Ellie was fantastic, she knew almost
straight away what it was although
she has only ever seen one other
dog with the same condition during
her entire working career. Apparently,
it is a rare hereditary condition that
generally affects Border Collies, Giant
Schnauzers and Beagles.
Symptoms start to show when the
dogs are between five and nine
months old , which is what happened
with Indie and the infections get worse
and worse until the dogs are usually
euthanased.

Indie is now on weekly vitamin B12
injections and will remain on these for
the rest of his life.
About Vitamin B12 Deficiency, Ellie Mardell MA VetMB DSAM(Feline)
MRCVS Consultant in Canine & Feline Internal Medicine RCVS
Recognised Specialist in Feline Medicine,
explains:
Inherited, selective inability to absorb vitamin B12
(termed Imusland-Grasbeck syndrome in people) is
uncommon but has been reported in a few breeds
of dog including Border Collies, Beagles, Australian
Shepherd Dogs, and Giant Schnauzers.
It is caused by a slightly different gene mutation
identified in each of these breeds.
Vitamin B12 is vital to cellular function in all body systems and the effects of
deficiency are wide ranging, particularly affecting the intestine, liver, and bone
marrow. Typically, dogs present at a young age with vague signs such as failure
to thrive and poor appetite because of the effects on the intestine, and recurrent
minor infections because of poor white cell production from the bone marrow.
More dramatic presentations are also seen, including severe neurological signs
relating to liver dysfunction, and low blood glucose secondary to sepsis or poor
liver function. Such metabolic derangements can be fatal if they are acute and
severe, however patients can respond dramatically to supportive care, and vitamin
B12 injections.
Indie’s presentation was dramatic and without the rapid emergency care he
received, may have been fatal. Once stabilised, the genetic test result indicated
the cause of all of the historical and acute presenting factors, and confirmed the
necessary lifelong treatment required to prevent recurrence of the signs.
Hopefully Indie’s story will raise awareness of the acute signs of selective B12
malabsorption as well as the more chronic presentations, which will lead to rapid
identification and treatment of the condition in affected patients.

Shortly after Indie was discharged
from our care, his breeder was able
to put Indie’s owner in touch with
the owner of a sibling dog who was
hospitalised at a local vets with very
similar signs. Vitamin B12 levels were
duly assessed and found to be low,
and the dog responded well to B12
supplementation.
Ellie Mardell, commented:
It is heart warming to know that
because of this shared information,
a second dog has already benefitted
from our diagnosis and in addition
the breeder is aware not to use the
same sire-dam pairing again in the
future.

Janet said:
He is a happy, healthy dog now
and has put on weight and is really
enjoying his life. We are so grateful
to ChesterGates for saving him and
giving him a second chance.


  • 6. 4 5The Mesothelioma Handbook Introduction Patients with mesothelioma say that having reliable information about the condition and receiving good care at home, from family members alongside community nurses, can make a huge difference to their experience of the illness. The diagnosis of mesothelioma usually comes as a shock, but those affected by this illness are not alone: this handbook has been developed in collaboration with people who have mesothelioma, family members and healthcare professionals involved in their care and members of asbestos victims support groups. This handbook is written with the intention of providing straightforward and comprehensive information. Professional knowledge of the condition is variable as it is very uncommon in some areas. However, the information provided here is based on considerable experience in the medical and nursing care of sufferers from mesothelioma and in the support of their relatives. People with mesothelioma and their family carers want different types and quantities of information. Some people with mesothelioma might feel that they receive sufficient information from their doctors. Family carers are ‘partners in care’ with community healthcare staff and, as such, should have access to as much information as they want and need. The term ‘family carer’ is used throughout the handbook for those close relatives or friends who might be involved in the care of someone with mesothelioma, for example, wife, husband, daughter, son, step-relative, partner or close acquaintance. Many close relatives have said that they do not regard themselves as ‘carers’ as they are simply ‘doing what you naturally do in a family’. However, for convenience and brevity, the term ‘family carer’ is useful. It also distinguishes ‘informal’ carers ie family or friends, from ‘professional’ or ‘formal’ carers ie those who are healthcare professionals. This handbook is not meant to be read cover to cover, you can chose how much or how little you need to read, according to your situation. Particularly at the time of diagnosis there is a lot to take in, and so you might prefer to dip in and out of the handbook. There is a lot of information presented here because mesothelioma affects different people in different ways. Every effort has been made to use straightforward language and to explain any specialist terms. If you find some of the content rather difficult or technical please discuss any queries with your Macmillan or lung cancer nurse, GP or specialist. You can use the book to prompt questions that you might need to ask. Information that is mainly for nurses and other healthcare professionals is placed at the end of some sections and has a purple background and page heading. The term ‘community nurses’ includes District Nurses, Practice nurses and Hospice at Home nurses; other healthcare staff such as GPs, junior doctors, physiotherapists and occupational therapists might also find this handbook useful. Pleurodesis is performed in order to prevent recurrence and is most successful when performed early in the illness. If the mesothelioma has caused extensive thickening of the pleura, is encroaching into the lung resulting in reduced lung volume, or causing a fixed lung, then a pleurectomy/decortication (PD) procedure might be the preferred surgical option intended to restore the free movement of the lung and thus relieve breathlessness.   • 22. Brooms Farm was the heart of the village in Baldesrby St James, creating work for most of its occupants. I loved it and would spend most of my childhood in the fields with the men picking potatoes in winter, by hand, hoeing sugar beat in spring, and pulling wild oats in early summer, then harvest. It was a cycle, a life cycle. There were no corner shops or streets to hang out on. You worked. Whether it was on the farm or at home. You worked. That’s just the way it was. I loved it that much, that I stayed and worked on the farm until my late twenties when I left to work in forestry. Old fashioned farming is in my blood! The food we ate was simple but, my word, it was tasty. We ate fresh vegetables from the garden, and meat mostly from the land; rabbit or game pies being my absolute favourite. I can still feel the feathers tickling my nose as a small boy as I would sit on the step plucking pheasants ready for game pies. When Dad had the cash to pay a trip to the butchers, he improvised using cheap cuts of meat; chuck, shin, ham hock, heart and tripe. God I could never get used to tripe! Dad took care of six lads on his own on a shoestring, making do and mend but we never went without. I’ve created Farm to try and recreate some of my childhood memories; the food and design, a nostalgic feeling of warmth, well being and security. I hope you love it as we do. Janet and David Royle took Indie on as a puppy almost a year ago after buying him from a local farmer. The tri-colour pup was initially bouncy and happy but did suffer from stomach upsets. The Royle’s changed his diet and things improved for a while but then followed a catalogue of infections DOG SURVIVES A RARE ILLNESS RESULTING FROM VITAMIN B12 DEFICIENCY from conjunctivitis to kennel cough and water infections. A severe infection later resulted in Indie being hospitalised. After another brief recovery, Indie took a dramatic downturn and collapsed in a state of horrific distress with diarrhoea everywhere. The Royle’s rushed Indie to a pet hospital in Manchester where he was put on a drip in intensive care and treated for gastroenteritis and sepsis. Janet said: We were told that he was unlikely to survive the night, but thankfully he was still with us by the next morning.  Staff were unable to reach a diagnosis to explain why these infections kept recurring so they referred Indie to veterinary specialists, ChesterGates in Chester.who immediately ran tests and diagnosed his condition as sepsis with low blood sugar and low calcium, in turn related to the deficiency of vitamin B12, caused in this case by a genetic inability to absorb the vitamin. Janet Royle said: ChesterGates took him in and ran tests on him. We were really impressed that they had read his notes and taken in all the information we had provided. They were really thorough and prescribed 12 weeks of anti-biotics. One of their tests - a genetic test revealed that he had vitamin B12 deficiency.  THESE PETS SURVIVED AGAINST THE ODDS AND WENT ON TO WIN A BRAVERY AWARD ChesterGates’ Veterinary Surgeon, Ellie Mardell, MRCVS, explains: Inherited selective B12 deficiency is caused by the genetic inability to absorb the vitamin from the small intestine. It is an uncommon but recognised condition in certain breeds of dog including Border Collies. Usually, dogs present with vague signs, including failure to thrive, or neurological signs secondary to the absence of B12. Indie’s presentation was much more unusual, and at the time, I had only seen it in one other dog. We know that low B12 levels often lead to diarrhoea and poor gut function in dogs and cats, and I suspect that Indie’s gut had become so compromised, that bacteria from the intestine were able to enter the blood stream, causing sepsis and secondary low glucose. The low calcium I suspect was due to sepsis, and also the poor gut function, leading to reduced calcium absorption. This case required some detective work!  David said: Ellie was fantastic, she knew almost straight away what it was although she has only ever seen one other dog with the same condition during her entire working career. Apparently, it is a rare hereditary condition that generally affects Border Collies, Giant Schnauzers and Beagles. Symptoms start to show when the dogs are between five and nine months old , which is what happened with Indie and the infections get worse and worse until the dogs are usually euthanased.  Indie is now on weekly vitamin B12 injections and will remain on these for the rest of his life. About Vitamin B12 Deficiency, Ellie Mardell MA VetMB DSAM(Feline) MRCVS Consultant in Canine & Feline Internal Medicine RCVS Recognised Specialist in Feline Medicine, explains: Inherited, selective inability to absorb vitamin B12 (termed Imusland-Grasbeck syndrome in people) is uncommon but has been reported in a few breeds of dog including Border Collies, Beagles, Australian Shepherd Dogs, and Giant Schnauzers. It is caused by a slightly different gene mutation identified in each of these breeds. Vitamin B12 is vital to cellular function in all body systems and the effects of deficiency are wide ranging, particularly affecting the intestine, liver, and bone marrow. Typically, dogs present at a young age with vague signs such as failure to thrive and poor appetite because of the effects on the intestine, and recurrent minor infections because of poor white cell production from the bone marrow. More dramatic presentations are also seen, including severe neurological signs relating to liver dysfunction, and low blood glucose secondary to sepsis or poor liver function. Such metabolic derangements can be fatal if they are acute and severe, however patients can respond dramatically to supportive care, and vitamin B12 injections. Indie’s presentation was dramatic and without the rapid emergency care he received, may have been fatal. Once stabilised, the genetic test result indicated the cause of all of the historical and acute presenting factors, and confirmed the necessary lifelong treatment required to prevent recurrence of the signs. Hopefully Indie’s story will raise awareness of the acute signs of selective B12 malabsorption as well as the more chronic presentations, which will lead to rapid identification and treatment of the condition in affected patients. Ellie Mardell MA VetMB DSAM(Feline) MRCVS Shortly after Indie was discharged from our care, his breeder was able to put Indie’s owner in touch with the owner of a sibling dog who was hospitalised at a local vets with very similar signs. Vitamin B12 levels were duly assessed and found to be low, and the dog responded well to B12 supplementation. Ellie Mardell, commented: It is heart warming to know that because of this shared information, a second dog has already benefitted from our diagnosis and in addition the breeder is aware not to use the same sire-dam pairing again in the future.  Janet said: He is a happy, healthy dog now and has put on weight and is really enjoying his life.
