WHY was the HOPES plaAorm created?
HOPES	was	created	out	of	necessity	for	both	pa/ents	and	providers	so	they	
can	have	easy	access	to	ALL	of	the	needed	services	surrounding	the	
iden/fica/on	and	care	for	pa/ents	at	risk	of	hereditary	cancer.	
•  Providers	want	to	iden9fy	these	pa9ents.	They	feel	a	responsibility	to	offer	this	service	but	they	
just	don’t	have	/me	or	the	exper/se	to	counsel	them.	
•  Unfortunately,	the	lab	industry	has	pushed	us	to	take	the	decision	lightly	and	rush	through	
tes/ng	without	considering	everything	that	is	involved.	
•  Prior	PiZalls….	Pa/ent	have	been	tested:	
•  who	don’t	qualify.		
•  without	an	understanding	of	costs	or	access	to	organiza/ons	who	provide	assistance.	
•  by	providers	who	don’t	understand	WHICH	genes	even	need	to	be	tested	which	leads	to	overu/liza/on	
and	con/nued	denials	by	insurance	companies.	
•  by	providers	who	don’t	understand	the	results	or	how	to	support	the	pa/ent	in	developing	a	primary	
preven/on	plan.	
•  And	then	le^	stranded	b/c	the	lab	couldn’t	run	without	prior	authoriza/on	which	was	then	put	back	on	
the	provider	OR	gene/c	counseling	that	was	then	put	back	on	the	pa/ent.	
•  By	out-of-network	labs	who	get	denials	for	pre-auth.		but	then	fail	to	offer	other	solu/ons	for	the	pa/ent.	
•  HOPES	was	created	because	there	was	a	HUGE	void	in	the	con/nuity	in	pa/ent	care.

For	the	prac/ce…	
	
•  Pa/ent	receives	a	HOPES	Pa/ent	Ques/onnaire	at	Check-in	and	
returns	it	to	the	nurse	when	they	are	taken	to	the	room.	
•  If	one	of	the	boxes	are	checked	at	the	bo_om,	they	should	be	
referred	to	HOPES.	
•  Referral	involves	the	comple/on	and	faxing	of:	
•  A^er	the	pa/ent	has	completed	their	GC	appt.	and	prior	auth.	(when	
indicated),	HOPES	will	provide	the	informa/on	to	the	referring	
physician	and	they	have	the	op/on	to	proceed	with	ordering	any	
recommended	test(s).	
•  HOPES	will	facilitate	sending	the	pa/ent	a	test	kit	and	guiding	them	
through	taking	the	sample	and	submicng	it	to	the	lab	OR	the	pa/ent	
can	have	the	sample	taken	at	the	physician’s	office.

•  A^er	the	pa/ent	has	completed	their	GC	appt.	and	prior	auth.	(when	
indicated),	HOPES	will	reach	out	to	the	pa/ent’s	physician	to	execute	
the	ordering	of	the	test.	The	physician	will	receive	the	full	GC	
recommenda/on	and	Prior	Auth.	with	the	requisi/on	request	to	
demonstrate	all	appropriate	steps	have	been	taken	for	their	pa/ent.	
•  HOPES	will	take	this	opportunity	to	introduce	the	HOPES	program	to	
the	prac/ce	and	will	send	a	Welcome	Packet	in	the	mail	to	include	
marke/ng	tents	and	ques/onnaires.	
•  A^er	receiving	the	physician	signed	requisi/on,	HOPES	will	include	all	
paperwork	in	the	pa/ent’s	tes/ng	kit	and	mail	it	to	the	pa/ent	OR	
send	the	kit	to	the	physician’s	office	to	be	tested	during	an	in-person	
appt.	if	requested.	
•  If	the	test	kit	is	sent	to	the	pa/ent’s	home,	the	advocate	will	follow	
up	with	the	pa/ent	the	day	of	arrival	and	walk	them	through	
obtaining	the	sample.

HEREDITARY ONCOLOGY PREVENTION EMPOWERMENT SERVICES
Who should have a
Hereditary Cancer Risk Assessment?
• A person having two or more close relatives on the same side of the family with the same or
related cancers, especially if the cancer occurred before age 50.
• A person who either has cancer before the age of 50, who has more than one kind of cancer,
or who has a close family member with the above.
• A person from a family that is known to have a inherited cancer syndrome.
• A person with a rare cancer; for example, breast cancer in a man or a person with an adrenal
gland tumor, cancer of the uterus, stomach or thyriod.
• A person who is very anxious about their risk based on their family history.
