3. Rapid Sequence Intubation
• RSI គឺជាការផ្តល់ថ្ន ាំសណ្ត ាំ (induction agent) និង paralysis agent សាំរាប់បនថយកាំរ ិតស្មា រតី
និងចលនារបស់អ្នកជាំងឺ (Motor) ដដើម្បីដាក់បាំពង់ដដងហើម្សិបបនិម្ិតត (Intubation)។
• ការរកា O2 កន ុងដពលដាំដណ្ើរការ intubation គឺសាំខាន់បាំផ្ុត។

1. GCS< 8 ឬ​បាត់បង់ការការពារផ្ល ូវដដងហើម្
2. របួសម្ុខ ដដលពីបាកគ្គប់គ្គងផ្ល ូវដដងហើម្
3. ដ្លើងដ េះដដលដយើងគិតថ្អាចរបួសដល់ Aveoli
4. បញ្ហហ រសួ តធ្ងន់ធ្ងរ ដដដគ្បើ CPAP ម្ិនបានផ្ល

7. P1. Preparation
តាំដ ើងម៉ា សុីន Ventilator, Aspiration, Patient monitor, ថ្ន ាំ,
IV access (ដសរ៉ូម្), laryngoscopes, Endo tracheal tube(សុង intubation),
Stylet and Bougie,

8. P2. Pre-oxygenation
Type of patient

Sa 02 > 90%
Healthy 70kg adult 8 minutes
Moderate ill adult 5 minutes
10Kg child 4 minutes
Obese adult 3 minutes
Very ill patient <2minutes
Time taken to desaturate from 90% to zero is very short. In the
healthy adult it is 120 seconds, and in a child it is only 45 seconds.

9. P3. Pre-treatment
• Lidocaine
• Atropine
• Adrenaline
• Fluid bolus

10. P4. Paralysis with Induction
Induction agent

Fentanyl 2-3mcg/kg

Midazolam 0.2-0.3mg/kg

12. P6. Placement and Proof
• Intubation should be performed carefully and gently. The larynx is visualised
and the endotracheal tube placed. The stylet, if used, is then removed and
the cuff inflated.

13. P7. Post-intubation management
Standard post-intubation care:
• ECG,
• Blood pressure,
• SpO2, Capnograph,
• Urinary catheter,
• Naso/Oro Gastric tube,
• Chest Xray, ABG(GDS),
• Sedation long term.
