Resident Assessment Coordinator
Responsible for completing MDS Assessments for Medicare, HMO and all private insurance on six of the ten
units (264 beds) in order to maximize optimal reimbursement. Total responsibility for all phases of MDS from
completion of book to submission to NYSDOH. Oversight of all MDS completion and submission in
accordance with OBRA Regulations. Previously responsible for Full House completion and submission of all
PRI’s while maximizing the facility’s case mix index. Responsible for updating EQUIP Quality Indicator
Program with most recent MDS data.

Home Care Coordinator
Responsible for all nursing care for in-patient home care clients, as well as Quality Assurance, Inservice
Education and Evaluations, Infection Control and Risk Management. Completed all admissions assessments,
including the coordination of all marketing efforts. Reported directly to the Facility Director.

Responsible for the full spectrum of Nursing Services including, but not limited to, policy and procedure
creation and implementation, master staffing patterns and all aspects of regulatory compliance. Involved in all
pre-opening planning (Park Avenue Extended Care), including the configuration of the Nursing Department
within the framework of the facility’s overall structure. Established Quality Assurance and Assessment
Program. Set up and maintained sub-acute programs, including Wound Care, Ventilator and Short Term and
Cardiac Rehab. (including all policies and procedures). Created CQI and Nursing Performance Appraisal policy
and procedures. Utilizing input from all disciplines, identified quality issues and/or deficient practices, reviewed
and revised departmental policy and procedures manuals, as needed to restore compliance. Created QA audit
tool to monitor compliance with newly revised policies and procedures. Reviewed audit tools to compile data
to create a compliance threshold which is reported to the quality Assurance Committee, which then determines
the frequency of further audits/monitoring.
