Responsible for completing MDS Assessments for Medicare, HMO and all private insurance on six of the ten
units (264 beds) in order to maximize optimal reimbursement. Total responsibility for all phases of MDS from
completion of book to submission to NYSDOH. Oversight of all MDS completion and submission in
accordance with OBRA Regulations. Previously responsible for Full House completion and submission of all
PRI’s while maximizing the facility’s case mix index. Responsible for updating EQUIP Quality Indicator
Program with most recent MDS data.

and procedures. Utilizing input from all disciplines, identified quality issues and/or deficient practices, reviewed
and revised departmental policy and procedures manuals, as needed to restore compliance. Created QA audit
tool to monitor compliance with newly revised policies and procedures. Reviewed audit tools to compile data
to create a compliance threshold which is reported to the quality Assurance Committee, which then determines
the frequency of further audits/monitoring.
