1 Is there a Faculty Health & Safety Policy or code of practice ? Y There is an additional Health & Safety Policy other than the University one, which highlights
all the workshop areas in the Faculty, but does not have a code of practice.
2 Does the Policy assign Responsibility ? Y The policy does have responsibilities with Richard Trupp being the key focus for Health & Safety
as Head of Workshops, in conjunction with Technical Officers from each department, however the
ultimate responsibility and accountability for Safety, Helath and Welfare at Work within FADA
lies with the Dean and members of the Faculty Management Group, in particular heads of School.
3 Does the Policy / code of practice commit to providing training, Induction Y There are clear objectives for Induction to both student and staff from which stemmed from the
Commitment ? Health & Safety review for all workshops carried out by Independent Health & Safety
Consultant James Vetch Dip Grad, where he identified key areas of concern in the faculty
workshops and Inductions being one of them, there is now a written policy on Inductions in
some of the workshops, with 3D being the higher risk. All training is carried out as per the
Induction, where clear guidelines are given to students before working in the workshops of which
they have to read and sign the declaration.

5 Does any policy assign a responsibility for conducting audits or reviews ? Y This has been agreed in principal.
6 Is there any evidence of undergoing regular reviews ? Y/N A protocol for regular reviews within FADA has been put in place, to be carried out on a regular
basis.
7 Is the policy signed by the Dean ? Y This policy was put in place in June 2014 where upon it was submitted to the FMG for signing
by the Dean.
8 Is the policy discussed or endorsed by the Faculty Management Group ? Y This was put forward to an FMG meeting for endorsement in 2014.
9 Do the arrangements include protection to visitors / public ? Y The policy covered all parties including visitors and contractors.
10 Does the policy make reference to other documentation ? Y There is one key document to which the policy refers to and this is a document called the Risk
and Control Measures document, which highlights all the risks and control measures in each
workshop of FADA.

documented evidence of which I have seen, to suggest this is being carried out.

assessments are concerned.

read the Risk & Control document for each departmental workshop within the faculty, all the
documents have the same emphasis, however have different risk categories.
13 Are checks made and recorded to ensure staff comply with safe systems Y Richard Trupp as the head of workshops is responsible for making sure that there are safe systems
of work and working procedures ? and procedures are in place and that staff sin the various workshops are complying to them.

chemicals and substances are concerned, to bring the risks down to the lowest possible where
reasonably practicable.
7 Has suitable and sufficient training been given to staff ? Y Yes all staff with have suitable and sufficient Instruction, Information and where practicable
training.
8 Are repeat assessments scheduled ? Y Where there is a requirement to update exisiting risk assesments on machinery due to modifications
on equipment, this will then be re assessed otherewise new risk assessments will be wriiten for all
new equipment.
9 Is new equipment assessed prior to purchase ? Y All new equipment will be assessed prior to being brought to make sure they have suitable and
sufficient safety standards such as interlocking switches and cut off buttons.
10 Are risks made known to staff? If so can they explain how ? Y All risks will be explained and documented to identify to staff.
11 If work changes are reassessments carried out ? Y Where work changes so does the risk assessments and this will be carried out when required.

2 Can the alarms be activated from all parts of the building ? Y The building is old, but in terms of fire and fire alarms, these can be clearly heard through all
areas of the faculty.

5 Are the exit signs clearly marked and illuminated ? Y/N One of the recommendations from the Health & Safety review in FADA was to look at signage
and to make sure it was suitable and sufficient in all areas, as there were key places of which had
been identified where this was not the case, and new signs were purchased, however there are
issues wi th the new signs which were sticky and in the heat have become in effective and basicaly
peeled of, making them not fit for purpose.

are new students joining the sessions on a daily basis.

case, and more often or not there are wedged fire doors.

1 Are there sufficient first-aid boxes and are they adequate for the number of staff ? Y There are first aid boxes on all floors of all the FADA, and outside buildings.
2 Are the boxes labelled and identified ? Y There are adequate number of first-aid boxes which are sufficiently stocked and labelled.
3 Are staff made aware of first aid arrangements ? Y Clearly marked on all floors of FADA and outside buildings, with suitable and sufficient signage.

person is and where they are located.

1 Have Safe systems of work been devised and are they scheduled for review ? Y In the Health & safety review in 2014 safe systems of work were looked at by James Vetch Dip
Grad - Independent Health & Safety Consultant who was carrying out the review, and a review
of the practices within the workshops was recommended on a regular basis, to make sure that
there was compliance.
2 Are workplace inspections carried out and how often ? Y These are carried out by Richard Trupp and Kat Horswill before LSG meetings, so that relevant
information is brought to the table on the day, unless of course there are unknown hazards in any of
workshops which need to be brought to his attention for investigation, these need to be carried out
on a more regular basis, as highlighted in the smmary for the faculty.
3 Are Safety arrangements in place for all work including overtime and when lone Y So far as lone working is concerned, this has been significantly reduced by putting in place time

there is always the risk of slips, trips and falls, and although there are controls in place to reduce
the risk of accidents occurring in the workplace it is not possible to control all risks relating to

14 Are windows in a safe condition ? Y All windows in FADA are in good condition, however where the print making workshop is
concerned there is not enough ventilation in terms of fresh air, as there are not enough opening
windows.
15 Are there adequate washing facilities ? Y There are suitable and sufficient toilets and washing facilities on all floors of FADA, and in the
avionics building.
16 Is there an adequate supply of drinking water ? Y Yes there are water fountains in various locations of FADA, however there are some issues on
the fourth floor where water fountains and coolers are not available and there is not adequate
drinking water, which in summer because a hazard for both students and staff.
17 Is protective clothing provided where required under the PPE Regulations 1992 ? Y In the workshop areas of FADA PPE is worn for example staff are advised to wear Toe Tec
footwear to EN 345 standard in 3D workshop, Moving Image, and Print Making workshops.

All students are given a green apron upon induction into the 3D workshop.

students health, that these are reported immediately so that remedial action can be taken where

service for the workshops in FADA.

where the area is hot, there is undue stress by staff and students, therefore the risk of accidents and
incidents increases, therefore important to stabilize the temperature in the departments and
workshops where there are variant temperatures.

1 Are there 'special' wastes which need to be disposed of ? Y / N In the 3D Workshop there are paints and hazardous substances which need to be controlled, and
to include safe disposal of waste products.

kinds of waste products such as Plastic, Cardboard.

the weight of disposable waste, and therefore this meant there were additional hazards in terms of

now procedures to make sure that all loads to be lifted in the various workshops undergo a
suitable and sufficient Manual Handling risk Assessments.

projects, however there are places where light is not so good, and therefore have the potential to
cause accidents or incidents, through poor lighting, these Ares would need to be identified by means

I have seen no documented evidence to support this statement in as much as a data sheet or a
COSHH Risk Assessment.

and sufficient control measures, however this is not always the case, and aerosols with the potential
to ignite are left in areas of high risk, this was something to which was looked at and addressed in
the Health & Safety review of 2014 under bad practice, and although it is better there is room for
much improvement.

Y There is an additional Health & Safety Policy other than the University one, which highlights all the workshop areas in the Faculty, but does not have a code of practice. 2 Does the Policy assign Responsibility ? Y The policy does have responsibilities with Richard Trupp being the key focus for Health & Safety as Head of Workshops, in conjunction with Technical Officers from each department, however the ultimate responsibility and accountability for Safety, Helath and Welfare at Work within FADA lies with the Dean and members of the Faculty Management Group, in particular heads of School. 3 Does the Policy / code of practice commit to providing training, Induction Y There are clear objectives for Induction to both student and staff from which stemmed from the Commitment ? Health & Safety review for all workshops carried out by Independent Health & Safety Consultant James Vetch Dip Grad, where he identified key areas of concern in the faculty workshops and Inductions being one of them, there is now a written policy on Inductions in some of the workshops, with 3D being the higher risk. All training is carried out as per the Induction, where clear guidelines are given to students before working in the workshops of which they have to read and sign the declaration. 4 Is a Senior Manager assigned responsibility ? Y Richard Trupp Head of Workshops. 5 Does any policy assign a responsibility for conducting audits or reviews ? Y This has been agreed in principal. 6 Is there any evidence of undergoing regular reviews ? Y/N A protocol for regular reviews within FADA has been put in place, to be carried out on a regular basis. 7 Is the policy signed by the Dean ? Y This policy was put in place in June 2014 where upon it was submitted to the FMG for signing by the Dean. 8 Is the policy discussed or endorsed by the Faculty Management Group ? Y This was put forward to an FMG meeting for endorsement in 2014. 9 Do the arrangements include protection to visitors / public ? Y I would like to think that where there are hazards in the workplace that these are reported either by the correct Safety coordinators of each workshop or by staff, however there is no documented evidence of which I have seen, to suggest this is being carried out. 7 Are staff informed of emergency procedures ? Y This will be covered in their Induction. 8 Are staff aware of their duties to themselves and others ? Y At all times and all courses staff are made aware of their duties to themselves and others, 9 Is there written information provided to staff on their duties to maintain safety Y Yes there are clear guidelines on how staff are to maintain good Health & Safety practice, it is when working in the Faculty ? just a question of being able to sustain such good practices due to time. 10 Is a record kept of information and instruction provided to staff ? Y Since the review there are good records kept in the various workshops so far as Induction and assessments are concerned. 11 Is the approved HSE Poster displayed ? Y Provided centrally by the Health & Safety Office. 12 Are staff informed of the hazards and the required precautions ? Y These are explained to all staff in their various workshops, and also staff are now required to read the Risk & Control document for each departmental workshop within the faculty, all the documents have the same emphasis, however have different risk categories. 13 Are checks made and recorded to ensure staff comply with safe systems Y Richard Trupp as the head of workshops is responsible for making sure that there are safe systems of work and working procedures ? and procedures are in place and that staff sin the various workshops are complying to them. 14 Are there staff with the responsibility of managing first aid provision Y First aid is available on all floors of FADA, and there are clear signs for persons who need first of managing first aid provision ? aid as to who is on duty and where they are based.. 15 Are there regular Health & Safety meetings of the local Health & Safety Y There are now local Health & Safety meetings held at FADA with minutes, of which will be held group with minutes taken and available ? every 6 months. 16 If a member of staff misses training is there a system that will ensure the information Y Where training has been missed by a member of staff, and depending on the trading required they is provided at a later time ? will undergo this at the next convenient time where r4easonbly practicable. 17 Are there any nominated local safety coordinators or officers to monitor Health & Y Apart from the Safety Champion Richard Trupp there are localised staff who look at Health & Safety ? There is a Risk Register for each workshop in a document called Risk & Control Measures, this can be accessed in each workshop, through the individual workshop managers.

Y Since the review the Risk Assessment for each workshop identifies control measures in the risk assessment especially where machinery and equipment is concerned. 5 Are the hazards for any new task undertaken assessed for hazards such as Y Under the new review all hazardous risks are assessed and taken into account, and especially chemical exposure or noise ? where chemicals are concerned. 6 Are recommendations made to reduce the risk to the lowest practicable level ? Y At all times since the Health & Safety review there is a policy to review safer options so far as chemicals and substances are concerned, to bring the risks down to the lowest possible where reasonably practicable. 7 Has suitable and sufficient training been given to staff ? Y Yes all staff with have suitable and sufficient Instruction, Information and where practicable training. 8 Are repeat assessments scheduled ? Y Where there is a requirement to update exisiting risk assesments on machinery due to modifications on equipment, this will then be re assessed otherewise new risk assessments will be wriiten for all new equipment. 9 Is new equipment assessed prior to purchase ? Y All new equipment will be assessed prior to being brought to make sure they have suitable and sufficient safety standards such as interlocking switches and cut off buttons. 10 Are risks made known to staff? If so can they explain how ? Y All risks will be explained and documented to identify to staff. 11 If work changes are reassessments carried out ? Y Where work changes so does the risk assessments and this will be carried out when required. 12 Are Risk assessment actions recorded ? Y Since the review all risk assessments will be recorded for the future. 13 Are staff comments noted and acted upon ? Y Where staff work on a particular type of equipment their views will be sort when looking at new equipment for the faculty. 14 Are there Risk Assessments showing how risks are managed especially where Y Where there is a risk and COSHH is concerned this will be manages correctly through the Risk COSHH is concerned ? Y The building is old, but in terms of fire and fire alarms, these can be clearly heard through all areas of the faculty. 3 Can the fire alarm be heard in all parts of the building ? Y At all times. 4 Was the last Fire evacuation within the last 6 months ? Y Date to be advised. 5 Are the exit signs clearly marked and illuminated ? Y/N One of the recommendations from the Health & Safety review in FADA was to look at signage and to make sure it was suitable and sufficient in all areas, as there were key places of which had been identified where this was not the case, and new signs were purchased, however there are issues wi th the new signs which were sticky and in the heat have become in effective and basicaly peeled of, making them not fit for purpose. 6 Do students / staff and visitors know where to go in the event of a fire evacuation ? Y At the beginning of each teaching session with students the Fire evacuation is explained as there are new students joining the sessions on a daily basis. 7 Are there suitable and sufficient arrangements for evacuation of disabled wheelchair Y There is an evac chair in FADA. users ? 8 Are the results of evacuations recorded and reviewed ? Y KUSCO. 9 do staff know what actions to take in an emergency ? Y 10 Are emergency exits kept free from obstruction at all times in all buildings ? Y Good housekeeping to be looked at on a daily basis as there were times om the Health & Safety review where there were blocked, wedged Fire doors, which was not good Health & Safety practice. 11 Is there suitable fire fighting equipment ? Y In all areas of FADA there is good fire fighting equipment with now fully trained fire marshals who can fight a fire where reasonably practicable. 12 Are staff told not to use fire extinguishers as door steps ? Y Staff are told not to use fire extinguishers as door stops but unfortunately this is not always the case, and more often or not there are wedged fire doors. 13 Are fire drills / equipment and fire training recorded ? Y These are recorded by KUSCO 14 Is security on the premises adequate ? Y As good as any other faculty, open access policy. Comments

Y One on each floor of FADA there are suitable and sufficient first-aiders and also in the Avionics building and the gallery there is adequate provision, as well as fully trained security personnel. 6 Are arrangements made for prompt attendance at first-aid incidents?
