• Willingness to learn and enthusiasm. Communicates with management.
4. How would you rate his/her technical skill on a scale of (1-10)?
• Mr. Treat has limited experience with the unit we have (GE VCT) but is able to perform most
standard CT studies with and without IV contrast. Due to his experience level, I would rate
him a 7.
5. Would you say he/she was motivated to do the job required?
• Yes
6. Did he/she perform all of the job functions required for the position?
• Yes
7. Was there ever any (patient) complaints about him/her?
• No complaints from patients
8. Would you rehire him/her if they applied again?
