- A vehicle collision report was submitted regarding two accidents involving vehicle GW-962 on January 2, 2023. - The first accident occurred when the vehicle hit an elderly woman crossing the road, fracturing her hand. The second occurred shortly after when another vehicle rear-ended GW-962. - An investigation committee was formed that reviewed witness statements, vehicle inspections, and accident scenes. It determined that both drivers were at fault - the first for not avoiding the pedestrian and the second for speeding and not maintaining a safe distance. - The committee recommended forming another committee to repair GW-962 and withholding pay from the first driver for two trips for failing to avoid the accident.

3. WHEREAS, vehicle GW 962 was on official duty drive by Mr.Zahir khan, driver along with
Project Accountant Mr.Sadiq Shah from PMDC SALT MINES WARCHA to MAINWALI to receive
external auditors. The Project Accountant PMDC Salt Mines Warcha was also travelling in the
vehicle GW-962.However, shortly making his journey back from Mainwali to PMDC Salt Mines,
Warcha Guest House. The twice accidents reported of vehicle GW 962.The impact of accident
was so substantial that it damage of official vehicle from rear and front side and private aulto
car as well as fracture the hand of aged-women ,on which the report has been prepared.
AND WHEREAS, on receipt of note submitted to Project Manager PMDC salt Mines Warcha initiated
proceedings under PMDC rules by appointing Dy. Manager (Admin) Mr. Tariq Aziz Shah & Engr.Faisal
Nawaz Asstt Manager (Electrical) vide officer order no:-SMW/Dis-Action/2022/2779 Dated:
03.01.2023 as inquiry officer’s.
AND WHEREAS, inquiry officer’s conducted the inquiry by inspection, impact of collision and
following eye witness of the accidents, recorded their statements.

AND WHEREAS; the brief fact of the accidents are that the dated 02.01.2023 Mr. Zahir Khan was on
official duty along with Project Accountant to receive external auditors who was on way from
Rawalpindi Bus Stand to Mainwali. Around 2:30 when the vehicle was on way back to project at wah
bachra an old aged women name Naseem Bibi with his daughter suddenly cross the road. The daughter
of women cross the road however Naseem Bibi started crossing the road while looking other side of
road. The driver in order to save from collusion applied the brakes instantly in result of which, however
vehicle hit the women which results in the old age women fell down and injured. The front bumper of
vehicle cracked near the right lights.in order to provide medical treatment they left with the women to
nearest Rural Health Center Wah Bachra. The Naseem Bibi was provides with First aid at RHC but due to
fracture on his right hand RHC referred to DHQ hospital. During this Naseem Bibi son Shabeer also come

4. in the scene at RHC wahn bachra hospital and insisted that the he will get further treatment of his
mother from local practitioner of eastern medicine (HAKEEM). After leaving RHC, when the vehicle was
on Mainwali-Sargodha road near Shadia with victim of accident Naseem Bibi with Shabeer and daughter.
Mr.Zahir Khan Vehicle applies brakes to slow down to for right turn to reach opposite side of road. An
Aulto car (BTX 550 SINDH) driver hit the GB 962 from back thus giving damage to vehicle from rear side.
1. Tail gate of vehicle got dents along with rear bumper.2. The right side lights broke.3. The rear right
side tail body also dent.

FINDINGS OF COMMITEE
After going through all the committee concluded the following
1. The committee members find that the road cross by women along with his daughter was
unlawful. In fact it can be seen in the following picture that the overpass bridge for
pedestrian were constructed/installed by the government. Furthermore it was discovered
from the external auditors that the women intended to cross the road by looking at the
opposite side of road.

6. 2. The speed of Vehicle GW 962 driving by Mr.Zahir Khan was normal as it is crossed verified
by statement of witnesses and also the impact of accident which result in crack of front
bumper was not so significant that it only damage the part of bumper.
3. Whereas Mr.Zahir Khan is permanent driver and drive different project vehicles from time
to time. However when the daughter of women intended to cross the road. He shall
deemed/realized the accident. It was responsibility of driver to avoid danger of accident
Thus he breach the RULES FOR THE USE OF STAFF CAR 1980” AND punishment/pelanties
under West Pakistan industrial and commercial employment (standing orders ordinance
1968) stands correct on him.
4. The impact of second accident near Shadia when the other vehicle collide with GW-962 was
so substantial that it damage the both the vehicle.as per eye witnesses who was sitting at
rear side of GW 962 near the tail gate that the driver was absent and focus on mobile.
Furthermore the statement provided by PROJECT ACCOUNTANT was that he tries to tell the
private driver by hand gesture and despite of this that the driver signal for the right turn
before turn point. Moreover as per accident impact analysis the speed of private vehicle was
above the speed limit allowed.
5. As evident from statement and impact of collision that the private driver fails to maintain
the safe distance and soon after the collision between the two vehicle. The crowd gathers
and due to severity of injury of women the private driver find his way to escape.

7. 6. After inspection the braking and tyre of vehicle of found satisfactory despite of tyres
completed the life as per record.as per eye witnesses the speed of vehicle in whole trip was
normal and in accordance with regulations. Moreover after the accident, Mr.Zahir Khan
perform official duties in GW-962 and also the vehicle performs duties with other driver of
this project as recorded in moment register of GW-962.
7. Tentative estimate on repair of vehicle obtained from M/S Sarso Auto Care & Enterprises of
Rs.35,000.00
RECOMMENDATIONS
NOW THEREFORE, we committee members in this instant report, after having considered all the
aspects material related to the report, statement and answer sheet submitted by Mr.Zahir Khan Driver,
Statement provided by Mr.Sadiq Shah ( Project accountant ), External Auditors and accident impact
hereby recommends that
1. A further committee shall be constituted for the repair/replacement of damage parts of vehicle
GW-962 with the representation of Mechanical and Accounts of this project.
2. The committee suggests with holding of two future TA of Mr.Zahir Khan of
Lahore/Rawalpindi/Islamabad for not avoiding accident and collective failure for the escape of
private driver.
