Dear Errol,
Congratulations! Avisena is pleased to extend a conditional offer of employment to you for the position of
Marketing Coordinator.
The following outlines the proposed terms of your relationship with our company:
Hire Date June 5, 2013, contingent upon successfully passing a pre-employment drug test and
background investigation.
Title Marketing Coordinator
Base Salary $2,083.33 semi-monthly, equivalent to an annual salary of $50,000
Incentive Bonus You will be eligible for an annual Incentive Bonus of up to ten percent (10%) of your Base
Salary tied directly to new sales revenue attainment goals. Your Incentive Bonus will be
prorated for 2013 based upon your Hire Date.
Benefits Standard benefits as offered by Avisena to all employees, as described in the Avisena
Employee Handbook and the Health Benefits Package, as well as 15 days of PTO
(Personal Time Off), accrued 5 hours per pay period.
Pursuant to the authorization you signed on the employment application and the consent form, we will conduct a
background investigation, and regardless of when you begin work, our offer is contingent upon the results of the
investigation. Because this is a conditional offer of employment, you should not give notice to your present
employer, sell real estate or incur any other expenses associated with acceptance of employment until you receive
notification that you have met the pre-employment conditions.
Please be reminded that Avisena does not want you to use or bring with you at any time, any confidential
information, trade secrets and/or proprietary information of any of your former employers.
