2. Near drowning in fresh water
 Hypotonic load to alveoli
 Water absorbed into circulation
 Surfactant washout
 Alveolar cell damage
 Chemical pneumonitis, pulmonary
edema
 Hypervolaemia
 Hyponatremia
 Haemodilution
 Haemolysis
Near drowning in Salt water
 Hypertonic load to alveoli
 Protein rich effusion into alveoli
 Surfactant damage, alveolar
basement membrane damage
 Alveolar cell damage
 Chemical pneumonitis,
pulmonary edema
 Hypovolaemia
 Hypernatremia
 Haemoconcentration
Near drowning is the survival of a drowning event and can lead to
serious secondary complications including death.
Complication of Near Drowning:
Hypoxemia & Metabolic Acidosis are Inevitable features.
Others: ALI, Dehydration, Haemoptysis, Rhabdomyolysis,
Renal Failure, Cardiac dysrhythmias, may Develop ARDS.

 If the Pt is Normothermic Upon arrival in the ED & in
Cardiopulmonary arrest or Asystole, serious thought
should be given to discontinuing resuscitation efforts
because recovery without profound neurologic
complications is rare.
