This document provides information about locations, times, and partners for health empowerment sessions and services provided by a Faith Community Nurse Practitioner. Locations listed include New Bethel Missionary Baptist Church, St. Ann Place Homeless Outreach Center, and True Fast Outreach Ministries. Community partners that play a key role in providing healthcare include Catholic Charities, Clinics Can Help, FAU Diabetes Clinic, and others. A variety of services are offered ranging from health screenings, education, referrals, and follow-up to ensure access to resources for ongoing care.

NURSE CONSULTATION
Answers questions about medical conditions,
medications, and follow-up.
HOME VISITS
To provide care coordination, medication
therapy management support, and screening
services to elderly and disabled persons.
CARE COORDINATION
To give you access to the resources you need
in order to improve your health. You will be
linked with individuals that can help you
obtain SNAP benefits, healthcare, mental
health services, and dental services.
BASIC TRIAGE SERVICES
Provides basic First Aid for scrapes, cuts,
and minor injuries as well as wound care.
SERVICES AVAILABLE:
The Faith Community Nurse Practitioner offers a variety of services ranging from
health screenings, education, referrals, and follow-up visits or calls to ensure you
received the services you needed. She is able to link you with the resources you need
in order to obtain health insurance, a primary care provider, dental services, and
mental health services so that you can continue to receive on-going care.
HEALTH SCREENINGS
Evaluate Blood Pressure, temperature, pulse, and
respirations. Blood glucose screening.
Depression screening. Rapid HIV screening
through Genesis Community Health Center. STI
screenings and Well woman exam referral.
Prostate and Mammogram screening referrals.
Basic wellness screenings related to general
health, including weight and vision testing.
HEALTH EDUCATION CLASSES
Basic wellness education at each site during visit
with clients. Health empowerment sessions
where special monthly topics are presented.
Topics include cervical health, heart health,
colorectal health, prostate health, physical fitness,
mental health, home safety awareness,
immunizations, glaucoma, breast cancer, diabetes,
and HIV/AIDS. Contact Dr. Kinlaw for the

COMMUNITYHEALTH AMBASSADOR
TRAINING
A health ambassador is any person who wishes to
promote holistic health within our community
through health screening, health education and
health referrals. You do not have to be a health
care professional in order to be a community health
ambassador, however you do need to be able to
commit to serving the community at least once a
month. For more information on what is required
and the roles and responsibilities of becoming a
health ambassador, please contact Dr. Kinlaw.
CPR/AED/FIRST AID CLASSES
This class is designed to teach adult/child/infant
CPR, AED, and First Aid Training.
MENTAL HEALTH FIRST AID
TRAINING
This training is designed to de-stigmatize mental
illness by training persons how to identify and
support adults experiencing a mental health crisis
and link them to the appropriate health services.
