There are several forms used for shipping Dangerous goods internationally,
here are two:

4. What are the chances of being cited for a Violation and Fine?
The number of violations each year is low, but the fines are
high. We’ve seen civil cases where a penalty exceeds $50,000
dollars for an undeclared air shipment.
Violations for ground shipments are also significant. The new
statutory maximum is $75,000 per violation, per day.
