Apparently, a survey and questionnaire are not one and the same thing. The
difference mostly exists in the fact that a survey is a research design while a
questionnaire is the tool used to make up that survey i.e. the questionnaire
is “part” of a survey.
A questionnaire can be both qualitative and quantitative, the latter which
can be achieved through methods like the Likert scale.
A survey is more research-like, like analyzing people’s attitudes toward a
product rather than the “tool” or “material” itself. Again, the tool of such a
survey would consist of questionnaires.
From Wikipedia:
A single survey is made of at least a sample (or full population in the case of
a census), a method of data collection (e.g., a questionnaire) and individual
3. questions or items that become data that can be analyzed statistically.
