Problem: Care givers of elderly patients with dementia face a number of barriers in providing care and assistance to loved ones. These barriers increase when the caregiver lives at a distance. Given the long term and worsening nature of dementia, care givers are often stretched to the breaking point trying to coordinate care for the patient while maintaining their own quality of life. Tools: In this data-driven era there are many sources of community and healthcare information that can expedite the care process and give hope to caregivers facing fatigue and burnout. Solution: Teams will be judged on their proof-of-concept data mashups that prioritize options for caregivers based on patient data, targeted community resources, geographical data and technological innovations (wearables, social platforms, crowdsourcing, etc). Objective: The prize winning data-visualization will provide personalized solutions that improve the quality of life of caregivers and promote patient care.
