This document provides the preliminary program for a seminar on preventing unnecessary hospital emergency department transfers for older people. The two-day seminar will be held on May 5-6, 2016 in Melbourne, Victoria and feature multiple sessions and speakers each day addressing challenges around emergency care for older patients and innovations to provide more appropriate care outside of hospitals. Topics will include primary and secondary prevention of emergency department attendance, integrated care models for residents of aged care facilities, reducing re-presentations to emergency departments, and keeping residents healthy at home.
