Naples, Florida, once a small, sleepy fishing
village, has quickly become an
internationally recognized vacation
destination and the definition of luxury living on the
Paradise Coast. The lifestyle, varied culture and
environmentally conscious attitudes of Neapolitans
have molded this Southwest Florida gem into one of
the most attractive locations in the world to vacation,
retire, have a second home or just live life to its
fullest.
In 1911, Barron G. Collier bought most of Naples
including over a million acres of swampland. Collier
was a visionary for the area and had grand plans to
bring the railroad to this part of the state. With
his own money he helped construct the Tamiami
Trail to connect Tampa to Miami forever changing
the landscape of Florida. With that pledge, the
state legislature created Collier County, the largest
county in the State of Florida. One hundred years
later, Naples has close to 1.5 million visitors per year.
Through innovation, Barron Collier succeeded in
changing the swampland into paradise.
With the recent U.S. Supreme Court Decision on
the Patient Protection and Affordable Care Act to
uphold the health care law, we look to the future
as we face the many challenges in implementing the
law and addressing the long-standing problems in
our healthcare delivery system. In order to survive
in these turbulent times, we will need to be much
like Barron Collier, innovative and bold, we must
demonstrate incredible leadership and go beyond the
ordinary to create a sustainable, value-based system
of care for the future of our country.
Join us in Naples, Florida where we will guide
you through the changing tides of Healthcare and
become visionaries of the healthcare delivery system
of the future as we transform it from a swampland
into paradise.

Welcome Conference Attendees:
On behalf of the 2012-2013 HFMA Florida Chapter Board and Leadership Team, I thank you for
attending the Region V Dixie Institute in beautiful Naples, Florida. A very special “sunny” welcome
to our friends from Alabama, Georgia, South Carolina, and Tennessee.
Our Dixie Institute Committee, chaired by Billie Jean Mounts and David Sierra, have put together an
exceptional curriculum addressing timely subjects such as the challenges and opportunities we all
face associated with the industry’s ongoing transformation from volume to value, the implementation
of the Affordable Care Act (ACA), and the drivers of change that will alter the delivery of
Medicaid. You are certain to be impressed with the caliber of our speakers and take away
valuable information to assist you in managing through these complex subjects going forward.
Since education and opportunities for career advancement are important to our members, we
implemented a scholarship program for this Dixie that truly demonstrates the benefits of becoming
an HFMA member. Thirty applicants applied for a full scholarship to the Dixie this year and after
an exhaustive evaluation of the applications, twelve members from Region V were awarded full
scholarships to attend the Dixie Institute.
No conference would be complete without an opportunity to network with colleagues, friends,
and new acquaintances. Don’t miss out on the chance to play some golf and enjoy the thrill
of experiencing an airboat ride through the Everglades on Sunday followed by trolley tours
in downtown Naples where you can sample the scrumptious cuisine of this fantastic town.
Compliments of our generous Corporate Sponsors and our creative Hospitality Committee, we
are hosting the Orange Blossom Cocktail Reception and 1927 Speakeasy Dinner / Dance on
Monday evening. Finally, on Tuesday evening, enjoy time on your own with friends and our
Corporate Sponsors exploring Naples and the surrounding areas.
Please be certain to visit the exhibit hall during the conference and thank our Corporate Sponsors
for making events like this possible for us. Many thanks to our volunteers who give up personal time
to prepare outstanding events like this.
I’m looking forward to spending time with each of you over the next few days. I truly hope you find
the Region V Dixie Institute a help to you and your organization as we work together to transform
healthcare through innovation and exceptional leadership.

BY INVITATION ONLY, AN INTERACTIVE PEER DISCUSSION WITH HFMA
Moody’s Industry outlook for 2013 for US Not for Profit Healthcare remains negative according to their January 22, 2013
report. Reasons for this include deceleration of revenue due to Federal cuts in healthcare spending, limited reimbursement from the
commercial sector and a slow economy that continues to impact patient volumes. Given the twin pressures of Health Reform and a
tight economy, it is critical for providers to lower healthcare service costs and create an efficient and effective revenue cycle.
This interactive discussion facilitated by Suzanne Lestina from HFMA, will allow all (invited??) attendees to interact with their peers,
and discuss the most pressing industry issues healthcare providers face. Attendees will be given topic ideas that are most challenging
to their organizations and working with their peers, discuss the impacts and barriers and identify solutions to these top challenges.

LEADING IN AN ERA OF CHANGE 10:45 AM
The second anniversary of the Patient Protection and Affordable Care Act has passed, and at no time in
recent history has the U.S. healthcare market been in such a state of flux. But through the pandemonium,
a consistent call to action has emerged. Efforts are underway to align the incentives of the government,

healthcare costs too much in the United States – the second-place country is not even close. Unfortunately, the
Affordable Care Act did not address America’s healthcare cost problem, so market forces are taking over.
AFTER THIS SESSION YOU WILL BE ABLE TO:
•	Discuss the healthcare industry transformation from volume based to value based reimbursement
methodologies.
•	Develop strategies to survive and thrive in the pay for performance model using cutting edge
collaborative payment arrangements.
•	Describe action steps for using innovation as a change agent to introduce new business models that
will require different skill sets and attitudes.

A REAL LIFE LESSON FROM THE FIELD: HOW A COMMUNITY BASED HEALTH SYSTEM RE-DEFINED
IT’S STRATEGY AND INITIATED CHANGE IN RESPONSE TO THE PARADIGM SHIFT, PART 1
NCH Healthcare is a private not for profit healthcare system in Naples Florida. NCH has 684 licensed beds which are split between two
hospitals NCH-Downtown Campus andNCH-North Naples Campus. NCH is rated as A2 by Moody’s and was the first hospital in Florida
to become a member of the Mayo Clinic Care Network. During its 54 year history prior to 2010, NCH employed very few physicians,
instead focusing on being the community’s leading provider of inpatient care. As a consequence in 2009 as a system, approximately
76% of NCH’s net revenue came from inpatient services and 24% came from outpatient services. Starting in the latter part of 2009, the
management team, recognizing that the healthcare system was not well positioned in the outpatient arena, launched a strategic initiative
that included the purchase of four cardiology practices, an outpatient catheterization lab, and the community’s largest multi-specialty
physician practice. This initiative when fully implemented resulted in the adding 67 physicians, 20 midlevel providers, 350 “support”
personnel and roughly $53M in new outpatient revenue. These transactions brought NCH’s outpatient revenue to 36% of total net patient
revenue.
AFTER THIS SESSION YOU WILL BE ABLE TO:
•	Discuss stakeholder perspectives on purchasing physician practices.
•	Develop strategies for merging cultures between the inpatient hospital team and the physician practice team.
•	Describe action steps for involving subject matter experts for legal, valuation and compensation arrangements and creating
transparency during the transaction.

PRESERVING AND MANAGING YOUR TAX EXEMPTION: AN OPERATIONAL PERSPECTIVE, PART I
With the changes to the law made by the Patient Protection and Affordable Care Act of 2010, legislative changes in state tax laws and
increased audit scrutiny by the IRS and state governmental agencies the tax-exempt status of hospitals are at risk. Illinois is setting the trend

crafting the 2012 tax exemption and charity legislation flowing from the landmark Provena Medical Center case. Lea McLaughlin, Baptist
Pensacola will review with us their recent 3 year IRS examination process and issues. Lea will discuss the issues the IRS challenged and
how they handled the examination; as well as discuss how they implemented new internal controls and processes around the tax function.
John Woodhull with Crowe Horwath will discuss the 501(r) regulations, best practice CHNA Reports and how you can use Form 990 as a
business case to support, preserve and manage your tax-exemption.
AFTER THIS SESSION YOU WILL BE ABLE TO:
•	Discuss specifics of the 2012 legislation specifically the 501(r).
•	Develop strategies, controls and processes for preparing for Government Audits.

EMERGING TRENDS AND THE FUTURE OF HEALTHCARE CAPITAL MARKETS
The healthcare capital markets have gone through drastic changes in the last several years. Bond
insurance and credit enhancement have all but disappeared, and alternatives like direct placements
have emerged with new benefits, but also new risks. As interest rates dipped to new lows last year, some
hospitals have refunded bonds and revisited how to pay for capital projects, but many providers are
still staying away from the debt markets out of concerns about what the future holds. To take advantage
of current market opportunities and maintain access to capital in an unpredictable environment, CFOs
must be current on their structural options and how markets determine terms and pricing. Moderated by
HFA Partners, an independent financial advisory firm, hospital CFOs from five states will discuss what’s
changed in the healthcare debt markets, what structures work, and how different organizations plan to
preserve their financial position and access to capital in the future.
AFTER THIS SESSION YOU WILL BE ABLE TO:
•	Discuss the pros and cons of various debt options in today’s environment.
•	Develop metrics and strategies for funding new projects and refunding existing debt.
•	Apply best practices to preserve and enhance access to capital with rating agencies, investors and
banks.
PREREQUISITES/PRE-WORK:
Basic understanding of tax-exempt bond financing
TOOLS AND TAKEAWAYS:
Lessons learned during the financing process, choosing between debt products, how to manage bond
ratings, when to start the refunding process, what to do with swap liability.
MODERATOR:
Pierre Bogacz, Managing Director, HFA Partners, LLC
PANELIST:
Susan Cornejo, Senior Vice President and Chief Financial Officer, Providence Hospital

IMPLEMENTING SECTION 501(R): BEST PRACTICES AND PITFALLS ON THE ROAD TO COMPLIANCE
Hospitals face a number of new disclosure and reporting requirements under Section 501(r), passed
as part of the Affordable Care Act. This session will discuss the Section 501(r) requirements regarding
hospitals’ financial assistance and emergency care policies, charge policies, and billing and collections
practices as set forth in the Proposed Regulations issued in June 2012. Presenters will share best practices
and challenges, as well as a practical discussion of implementation pitfalls.
AFTER THIS SESSION, YOU WILL BE ABLE TO:
•	Identify the requirements for adoption of a financial assistance policy and emergency medical care
policy.
•	Understand methods for determining amounts generally billed to patients and how these amounts
relate to charges to patients eligible for financial assistance.
•	Comprehend guidelines for implementing extraordinary collection actions that are compliant with
section 501(r).
PREREQUISITES/PRE-WORK:

TOOLS AND TAKEAWAYS:
501(r) implementation best practices

Don’t become a FRAUDian slip: Protect your organization’s bottom line and reduce vendor risk by taking
credentialing beyond the surface into a comprehensive systematic vetting process. Explore how a large
healthcare system identified vendor fraud and is proactively taking steps to manage threats in the future.
AFTER THIS SESSION YOU WILL BE ABLE TO:
•	Identify the need for vendor risk management.
•	Determine the vendor risk tolerance level at your organization.
•	Evaluate your vendor population through automated processes.
•	Learn how a more thorough vetting of your vendors helps protect your bottom line.
PREREQUISITES/PRE-WORK:
A basic understanding of risk management.
TOOLS AND TAKEAWAYS:
Strategic approaches to mitigating risks in the vendor selection and vendor retention processes.
SPEAKERS:
Matt Muhart, Senior Vice President Chief Financial Officer, Memorial Healthcare System and
Tom Green, Director, Vendor Risk Management, IMC, Inc.
In today’s environment the outsourcing of business office and revenue cycle management operations is a
strategy that is becoming a larger component of the master plans of many healthcare provider organizations.
The increased complexities and projected financial impact associated with the Affordable Care Act are
driving many providers towards outsourcing strategies that can help them to better navigate the changing
landscape, which includes striving to ensure compliance with the new regulatory guidelines and assisting with
the stabilization of the bottom line during times of reimbursement changes. The practice of outsourcing can
lead to improved results – reduction of Accounts Receivable days, increased cash collections, achievement
of workflow efficiencies, workforce optimization, specialized expertise, access to technological resources,
improved patient relations and the enhancement of overall financial performance. All of these benefits
support better business and healthcare decisions.
AFTER THIS SESSION YOU WILL BE ABLE TO:
•	Draw upon an innovative example utilizing a model that is broad in scope (addressing multiple areas
and disciplines of the provider organization) with longer-term objectives in mind.
•	Compare a dynamic example, narrow in scope and with more immediate objectives, illustrating how
a multi-faceted solution was created and implemented under extremely tight time constraints in order
to address a highly complex situation with millions of dollars of re-billable revenue at stake.
•	Contrast a more traditional business office outsourcing example, but with a different twist as the
outsourcing organization itself actually stems from a large healthcare provider organization, and
draws from its years of experience of supporting its internal facilities.
PREREQUISITES/PRE-WORK:
A basic knowledge of healthcare provider business office operations.
TOOLS AND TAKEAWAYS:
The panel discussion will focus on key drivers that each provider considered when moving to an out-
sourced model, the best practices learned from implementing the model and the outcome derived by
embracing the outsourced model.
MODERATOR:
Melinda Ramsdell, Senior Vice President, Healthcare and Institutions Division, Bank of America Merrill
Lynch
PANELISTS:
Joanne Aquilina, Vice President of Finance/CFO, Bethesda Health; Carmen Voelz, Vice President

How much will you lose
to take-backs this year?
You earned it.
Now stop losing it.
As hard as you work to submit clean and
compliant claims, your efforts to receive
reimbursement don’t end there. Payors can
still deny all or parts of a claim. Even worse,
when you undergo a RAC audit by your
region’s Medicare Recovery Auditor, you can
be asked to give back reimbursement you
received as far back as three years ago.
Enter Craneware’s Audit Revenue Recovery

manage payor denials and retain more cash
from RAC and other auditors – ensuring
that you collect and retain all the revenue to
which you are legitimately entitled.

The lifestyle, varied culture and environmentally conscious attitudes of Neapolitans have molded this Southwest Florida gem into one of the most attractive locations in the world to vacation, retire, have a second home or just live life to its fullest. In 1911, Barron G. Collier bought most of Naples including over a million acres of swampland. Collier was a visionary for the area and had grand plans to bring the railroad to this part of the state. With his own money he helped construct the Tamiami Trail to connect Tampa to Miami forever changing the landscape of Florida. With that pledge, the state legislature created Collier County, the largest county in the State of Florida. One hundred years later, Naples has close to 1.5 million visitors per year. Through innovation, Barron Collier succeeded in changing the swampland into paradise. A very special “sunny” welcome to our friends from Alabama, Georgia, South Carolina, and Tennessee. Our Dixie Institute Committee, chaired by Billie Jean Mounts and David Sierra, have put together an exceptional curriculum addressing timely subjects such as the challenges and opportunities we all face associated with the industry’s ongoing transformation from volume to value, the implementation of the Affordable Care Act (ACA), and the drivers of change that will alter the delivery of Medicaid. You are certain to be impressed with the caliber of our speakers and take away valuable information to assist you in managing through these complex subjects going forward. Since education and opportunities for career advancement are important to our members, we implemented a scholarship program for this Dixie that truly demonstrates the benefits of becoming an HFMA member. Thirty applicants applied for a full scholarship to the Dixie this year and after an exhaustive evaluation of the applications, twelve members from Region V were awarded full scholarships to attend the Dixie Institute. No conference would be complete without an opportunity to network with colleagues, friends, and new acquaintances. Don’t miss out on the chance to play some golf and enjoy the thrill of experiencing an airboat ride through the Everglades on Sunday followed by trolley tours in downtown Naples where you can sample the scrumptious cuisine of this fantastic town. Compliments of our generous Corporate Sponsors and our creative Hospitality Committee, we are hosting the Orange Blossom Cocktail Reception and 1927 Speakeasy Dinner / Dance on Monday evening. DIXIE INSTITUTE PROGRAM GUIDE 23DIXIE INSTITUTE 201322 CPE 1.0 Acacia 1 - 3 BY INVITATION ONLY, AN INTERACTIVE PEER DISCUSSION WITH HFMA Moody’s Industry outlook for 2013 for US Not for Profit Healthcare remains negative according to their January 22, 2013 report. Reasons for this include deceleration of revenue due to Federal cuts in healthcare spending, limited reimbursement from the commercial sector and a slow economy that continues to impact patient volumes. Given the twin pressures of Health Reform and a tight economy, it is critical for providers to lower healthcare service costs and create an efficient and effective revenue cycle. This interactive discussion facilitated by Suzanne Lestina from HFMA, will allow all (invited??) attendees to interact with their peers, and discuss the most pressing industry issues healthcare providers face. Attendees will be given topic ideas that are most challenging to their organizations and working with their peers, discuss the impacts and barriers and identify solutions to these top challenges. Thornton Kirby, JD, FACHE, President CEO, South Carolina Hospital Association MONDAY FEBRUARY 25 MONDAYMONDAY FEBRUARY 25THE VALUE JOURNEY 8:30 AM INNOVATION 8:30 AM LEADING IN AN ERA OF CHANGE 10:45 AM The second anniversary of the Patient Protection and Affordable Care Act has passed, and at no time in recent history has the U.S. healthcare market been in such a state of flux. But through the pandemonium, a consistent call to action has emerged. Efforts are underway to align the incentives of the government, hospitals, physicians, insurance companies, employers and patients to solve a ubiquitous problem: healthcare costs too much in the United States – the second-place country is not even close. Unfortunately, the Affordable Care Act did not address America’s healthcare cost problem, so market forces are taking over. NCH has 684 licensed beds which are split between two hospitals NCH-Downtown Campus andNCH-North Naples Campus. NCH is rated as A2 by Moody’s and was the first hospital in Florida to become a member of the Mayo Clinic Care Network. During its 54 year history prior to 2010, NCH employed very few physicians, instead focusing on being the community’s leading provider of inpatient care. As a consequence in 2009 as a system, approximately 76% of NCH’s net revenue came from inpatient services and 24% came from outpatient services. Starting in the latter part of 2009, the management team, recognizing that the healthcare system was not well positioned in the outpatient arena, launched a strategic initiative that included the purchase of four cardiology practices, an outpatient catheterization lab, and the community’s largest multi-specialty physician practice. This initiative when fully implemented resulted in the adding 67 physicians, 20 midlevel providers, 350 “support” personnel and roughly $53M in new outpatient revenue. These transactions brought NCH’s outpatient revenue to 36% of total net patient revenue. AFTER THIS SESSION YOU WILL BE ABLE TO: • Discuss stakeholder perspectives on purchasing physician practices. • Develop strategies for merging cultures between the inpatient hospital team and the physician practice team. • Describe action steps for involving subject matter experts for legal, valuation and compensation arrangements and creating transparency during the transaction. Bond insurance and credit enhancement have all but disappeared, and alternatives like direct placements have emerged with new benefits, but also new risks. As interest rates dipped to new lows last year, some hospitals have refunded bonds and revisited how to pay for capital projects, but many providers are still staying away from the debt markets out of concerns about what the future holds. To take advantage of current market opportunities and maintain access to capital in an unpredictable environment, CFOs must be current on their structural options and how markets determine terms and pricing. Moderated by HFA Partners, an independent financial advisory firm, hospital CFOs from five states will discuss what’s changed in the healthcare debt markets, what structures work, and how different organizations plan to preserve their financial position and access to capital in the future. AFTER THIS SESSION YOU WILL BE ABLE TO: • Discuss the pros and cons of various debt options in today’s environment. • Develop metrics and strategies for funding new projects and refunding existing debt. • Apply best practices to preserve and enhance access to capital with rating agencies, investors and banks. Explore how a large healthcare system identified vendor fraud and is proactively taking steps to manage threats in the future. AFTER THIS SESSION YOU WILL BE ABLE TO: • Identify the need for vendor risk management. • Determine the vendor risk tolerance level at your organization. • Evaluate your vendor population through automated processes. • Learn how a more thorough vetting of your vendors helps protect your bottom line. PREREQUISITES/PRE-WORK: A basic understanding of risk management. TOOLS AND TAKEAWAYS: Strategic approaches to mitigating risks in the vendor selection and vendor retention processes. SPEAKERS: Matt Muhart, Senior Vice President Chief Financial Officer, Memorial Healthcare System and Tom Green, Director, Vendor Risk Management, IMC, Inc. In today’s environment the outsourcing of business office and revenue cycle management operations is a strategy that is becoming a larger component of the master plans of many healthcare provider organizations. The increased complexities and projected financial impact associated with the Affordable Care Act are driving many providers towards outsourcing strategies that can help them to better navigate the changing landscape, which includes striving to ensure compliance with the new regulatory guidelines and assisting with the stabilization of the bottom line during times of reimbursement changes. The practice of outsourcing can lead to improved results – reduction of Accounts Receivable days, increased cash collections, achievement of workflow efficiencies, workforce optimization, specialized expertise, access to technological resources, improved patient relations and the enhancement of overall financial performance. All of these benefits support better business and healthcare decisions. AFTER THIS SESSION YOU WILL BE ABLE TO: • Draw upon an innovative example utilizing a model that is broad in scope (addressing multiple areas and disciplines of the provider organization) with longer-term objectives in mind. • Compare a dynamic example, narrow in scope and with more immediate objectives, illustrating how a multi-faceted solution was created and implemented under extremely tight time constraints in order to address a highly complex situation with millions of dollars of re-billable revenue at stake. • Contrast a more traditional business office outsourcing example, but with a different twist as the outsourcing organization itself actually stems from a large healthcare provider organization, and draws from its years of experience of supporting its internal facilities. PREREQUISITES/PRE-WORK: A basic knowledge of healthcare provider business office operations. TOOLS AND TAKEAWAYS: The panel discussion will focus on key drivers that each provider considered when moving to an out- sourced model, the best practices learned from implementing the model and the outcome derived by embracing the outsourced model.
