This document describes a web application called the Patient Daily Diagnostics Portal that allows patients to register for diagnostic checkups, book appointments with doctors, get COVID-19 tests, and book ambulances for emergencies. The application was created using PHP, HTML, CSS, and JavaScript and allows patients to register with credentials and login to access services. It also details the requirements, proposed design including pages for registration, login, appointments and ambulance services, and work division among team members who developed it.
2. INTRODUCTION
Patient Daily Diagnostics Portal is a web application made using PHP,
HTML, CSS and Javascript. This is a portal where a patient can register
himself for diagnostics checkup, appoint an doctor, apply for covid-19
test book for a specialized health checkup. The application also
provides emergency ambulance booking. Overall it’s a all in one health
checkup and emergency service portal.
3. OBJECTIVE
The main goal of this project is to provide all in one Health Checking Portal
1. Here one can register himself for a full diagnostics check up. He/She can register
with his/her valid credentials and after that he/she can log in any time.
2. Here one can book for a Covid-19 test. Where he have to register with his valid
credentials.
3. Here someone can look for major health issue appointments and also can look for
the pricings. She/he can directly book an appointment with the preferred doctors.
4. Here someone can book an Ambulance for emergency purposes.

Home Page:
The page is the index or home page of the application. Here we can see the navigation bar and
quick access tabs. The page is made with HTML and designed with CSS. Major controls,
navigations and validations are done using PHP programming language. The responsive moving
images are made using JQuery and Javascript.
6. Registration Page:
This page is where someone can register himself to the web application. The page is
designed with HTML and CSS. The data inserted is stored in MYSQL database. The
connection with database is build using PHP.
7. Login Page:
This page is build using HTML and CSS. In this page the registered people can login using their
email and password. The credentials are linked with MYSQL database. The link is done using
PHP programming.
8. Appointment Page:
This page is made with HTML and designed using CSS. PHP is used in backed. Here one
can see the Doctor names and fees. By clicking book now the are redirect to
Appointment Register Page.
9. Ambulance Service Page:
This page made with HTML and CSS. Here one can book a ambulance for
emergency purposes. By clicking book now the page is redirected to Appointment
Register Page.

11. CONCLUSION
The project Patient Daily Diagnostics Check Up is really helpful for those who
lives in a far distance from Hospital and nursing home . By this project the
patients have access there daily checkup. They can book ambulance at
emergency situations. Overall the project will be very helpful if this is
implemented in real life.
