Application
• Basically this form is used to alert the community that you
will be applying for a liquor license
• In NYC you must notify the local community board at
least 30 days BEFORE submitting an application to the
State Liquor Authority
• Outside of NYC you must notify the city clerk in the town
where the proposed premises is located also 30 days

5. • Right to Premises Where Alcohol Will Be Sold - you need to
know if there are any pending disciplinary actions at the
proposed location.
• Actual or Estimated Expenses - anyone investing money into
your business (and any joint account holders) will need to fill
out a questionnaire. It’s a good idea to be very clear about
where money is coming from and where it will be spent.
• 500 Foot Rule - if there are 3 or more existing establishments
with on premise liquor licenses within 500 ft of your location
then you will need to provide a statement of how granting of
this license would be in the public’s interest
• 200 Foot Rule - is there a school, church or place of worship
located with 200 ft of your location?

6. A questionnaire needs to be filled out by:
• All sole proprietors
• Each partner in a partnership
• Corporations or LLCs, see further instructions for line 10
• Managers
• Individual lenders, donors, or giftors
• Co-signers of loans
• Spouses who aid in management of premises, will be
employed at premises or who hold joint accounts with
applicant
• Anyone holding a joint with someone who is providing
funds for this venture
