This is to confirm that ANGELO A BONILLA, whose photo appears above, have taken and passed the written
portion of the Consolidated FSD for Hi-Rise - Business Office/Ilotel - Motel Z-50 Exam. This test is the first
part of the two-part requirement. In order to obtain full certification as a Fire Safety Director, an On-site test must
be taken and passed.
Your experience qualifications, as previously submitted, will now be reviewed. You will be notified within six (6)
to eight (8) weeks regarding this qualification review. If your qualifications are deemed acceptable, you will
receive a Certificate of Completion, which will allow you to schedule your On-site test. If your qualifications
are not accepted, you will receive a Letter of Non-Completion giving you a complete and detailed account of
your qualifications, and what do to submit additional information.
It is strongly suggested that you bring either notification to you employer's attention. It is your employer who is
expected to make the appointment for your On-site test or to submit additional information to the Fire
Department.
Please be aware that all notifications will be mailed to your home address. If you move or otherwise change
address, you must contact the Certificate of Fitness Unit in writing to New York City Fire Department, Bureau
of F'ire Prevention, C of F Unit, 9 MetroTech Centero Brooklyn, lYY 11201-3857, Attn: FSD Qualification
Review 1S-05K or email to shulerv@fdny.nyc.gov. All additional information should be mailed to this address
as well.
