Patients given 'Ticket Home' after Healthwatch
recommendations

2. In June 2014, a team of
Healthwatch-trained volunteers visited Eccleshill Community Hospital's
intermediate care service, offered when patients can be discharged from
acute wards but are not quite ready to be at home.
The volunteers' overall impression was that Eccleshill is a good unit with
many positive features. They also recommended improvements; one was
to involve patients and carers more in making decisions about when they
are ready to go home.
As a result of Healthwatch's report, the hospital piloted a new scheme;
'Ticket Home' gives patients control of their own recovery goals, and
helps them work proactively with their care team. The 'Ticket Home' acts
as a visual aid to help patients work with medical staff to plan and record
their steps towards being ready for home. It typically includes goals for
treatment, mobility, and self-care.
The scheme was developed in the US and has been adapted for use in the
UK by Cardiff University where it was shown to significantly reduce the
time patients spent in hospital after hip-replacement surgery. After a
successful pilot, the scheme at Eccleshill is now being rolled out across
the other three intermediate care services in Bradford.
The Trust has also acted on other Healthwatch recommendations,
including developing better information for patients and carers.
