無薦髂關節炎、HLA-B27陰性，會ALL連續鈣化超過四節，椎間腔相對完
整基本上，雖然乍看很像AS 但一個老杯杯進來，之前沒有你突然給人
家診斷AS不是很奇怪嗎？！要猜也猜DISH～
• Syndesmophytes而言大部分都會第一想到 Ankylosing spondylitis(AS).
• 但這題是......Diffuse Idiopathic Skeletal Hyperostosis(DISH)
• 瀰漫性特發性骨肥厚(diffuse idiopathic skeletal hyperostosis DISH)，是一種未明原因的以脊
柱前外側、骶結節、髂腰韌帶髖臼旁等部位瀰漫性鈣化及骨贅增生，是以脊柱前緣骨化為特
徵的特殊類型的強直性骨肥厚為主要表現的骨病。也稱為肥厚性脊柱關節病、Foresti-病.是一
種常見的隨年齡增長而逐漸增多的疾病.
• X光表現上 AS 跟 DISH 之間是有些差異性的.
• DISH而言...較常侵犯cervical and thoracic (particularly T7-115 )spine.以及Iliac crests
• Ischial tuberosities Iliolumbar ligaments
• Lesser trochanter等...AS則會侵犯到S-I joint.
• Ossifications而言AS會比較薄一點.我們比較AS(黃色箭頭)及DISH(紅色箭頭)就能大約看出端倪.
