3. SUSPENDED SENTENCE WORKFLOW INVESTIGATION Time of Filing Application filed at any time after Court finds accused guilty, but before it promulgates judgment of conviction. Who may apply The offender, his parent, or Guardian, may file an application

SUSPENDED SENTENCE WORKFLOW INVESTIGATION Form of application; supporting documents Application is in writing, duly verified, and supported by the following documents: * Certified true copy of Court’s decision * Original copy of birth certificate * Two (2) 2” x 2” colored photographs taken within 30 days from date of application * Original copy of NBI clearance and clearance from the Clerk of Court of law having jurisdiction over the case If offender is user, the following shall be attached * Result of drug confirmatory test of offender * Certificate/Clearance from the DOH that offender has not been previously confined in a rehabilitation and treatment center and if there is no c=such center, has been placed under the care of a DOH-accredited physician in the region or province where the offender resides.

SUSPENDED SENTENCE WORKFLOW INVESTIGATION COURT Referral of application to Executive Director The Court shall refer the application to the Executive Director for investigation, comment and recommendation. EXECUTIVE DIRECTOR Referral of application to PPA Refers application for suspended sentence for investigation to PPA, by virtue of Board Resolution No. 5 and MOA CITY OR PROVINCIAL PPA OFFICE

7. SUSPENDED SENTENCE WORKFLOW INVESTIGATION COURT EXECUTIVE DIRECTOR CITY OR PROVINCIAL PPA OFFICE Investigation Report Probation Officer submits his/her report and recommendation on the application within 30 days from receipt of the referral from the Executive Director of the DDB.

SUSPENDED SENTENCE WORKFLOW INVESTIGATION COURT Refers application for investigation to Executive Director, Dangerous Drugs Board EXECUTIVE DIRECTOR Refers application for suspended sentence for Investigation to PPA, by virtue of Board Resolution No. 5 and MOA CITY OR PROVINCIAL PPA OFFICE Conducts investigation and submit report and recommendation within 30 days to the DDB Executive Director, copy furnished PPA Central Office
