The Psychiatric Review Technique Form
Psychological impairments such as anxiety,
depression, schizophrenia, and post-traumatic
stress disorder (PTSD) have increasingly become
a cause for concern for many Americans.
Approximately one in four adults (or 61.5 million
Americans) lives with a serious mental illness.
We understand as disability lawyers in St. Louis,
MO, that many people who suffer from such a
mental illness find that it causes difficulty in their
ability to work or possibly even prevents them
from working at all. Social Security uses a
special form called the “psychiatric review
technique” form (PRTF) when evaluating these
types of conditions.
The PRTF allows a doctor (usually a psychiatrist) to review your medical
records and list his conclusions concerning whether you meet the
requirements to be approved for disability.
It consists of four parts:
Medical Summary, Documentation of Factors that Evidence the Disorder,
Rating of Functional Limitations, and Consultant Notes. The Medical
Summary allows the doctor to indicate whether you even have a mental
condition, and whether your condition is severe, non-severe, meets or
equals a listing, is severe but not expected to last 12 months, or if there is
insufficient information to evaluate your mental condition. The doctor will
then document the findings that show the existence of your impairment.
Once that is completed, the doctor will rate your functional limitations in four
areas: activities of daily living, social functioning, concentration, persistence,
