  2. 2. VA’s Strategy to Eliminate Homelessness Among Veterans<br />VA’s goal is to ensure that all eligible homeless and at-risk Veterans have easy access to appropriate programs and services. Veterans in need of services, whether at a Medical Center, a Regional Office, or a Community Organization, will receive assistance in accessing, housing, supportive services and treatment. <br />Built upon 6 strategic elements: <br /> Outreach/Education, <br /> Treatment, <br /> Prevention,<br /> Housing/Supportive Services, <br /> Income/Employment/Benefits, and<br /> Community Partnerships.<br />

VHPD- Primary Responsibilities<br />Conduct outreach to locate eligible veterans<br />Determine eligibility based on minimum service requirement<br />Partner with Vet Center and CoC grantee<br />Provide information and assistance for Healthcare, benefits and financial assistance (pension and compensation)<br />Conduct or arrange for clinical assessments<br />21 July 2011<br />
