Defining Prevention <ul><li>To prevent, reduce or delay the need for health and social care interventions </li></ul><ul><li>With a clear focus on promoting independence through an enabling or re-abling approach </li></ul><ul><li>BUT not everything for everyone … </li></ul><ul><li>Targeting our approaches where they are likely to achieve the best outcomes and will have most impact on maintaining independence </li></ul><ul><ul><li>people at risk, but not yet eligible for social care support </li></ul></ul>

Our approach <ul><li>Building capacity – helping people make informed choices and take responsibility for themselves </li></ul><ul><li>Commissioning, or supporting access to short term support in response to life events – enabling people to get back to where they were before a crisis </li></ul><ul><li>Commissioning or supporting access to services to avoid, reduce or delay the need for long-term health or social care support </li></ul>

Targeting <ul><li>Using evidence and information: </li></ul><ul><ul><li>Demographic data; citizen and service user views; demand / needs analysis; current provision; evidence of what works </li></ul></ul><ul><li>Understanding the trigger points or life events when short-term support will help </li></ul><ul><li>Making better use of contact points and opportunities to identify risk or increasing frailty </li></ul>
