DEFINITION OF SLEEP
ī‚ĸ Sleep is a naturally occurring altered state of
consciousness characterized by decreases in
awareness and responsiveness to stimuli.
PHYSIOLOGY OF SLEEP
ī‚ĸ Controlled by recticular-activating system and
bulbar synchronizing system.
ī‚ĸ Wakefulness occurs when the reticular system is
activated.
ī‚ĸ The hypothalamus has control centres for several
involuntary activities of the body, one of which
concerns sleeping and waking.
ī‚ĸ Injury to the hypothalamus may cause
a person to sleep for abnormally long
periods.
CIRCADIAN RHYTHMS
ī‚ĸ Biological rhythms that follow a cycle of about 24
hours are termed circadian rhythms
ī‚ĸ circa means ―about and dies means ―day
ī‚ĸ Ciracdian synchronization exists when an individual
sleep-wake pattern follows an inner biological clock.
ī‚ĸ when physiologic and psychological rhythms are
high or most active, the person is awake and when
these rhythms are low, the person will sleep
PHYSIOLOGIC FUNCTION
ī‚ĸ Electro- physiologic Approach
electro-physiologic changes in brain waves, eye
movements, and muscles show five sleep stages.
ī‚ĸ Neurotransmitter Balance:
ī‚§ Involves the reticular activating system (RAS) and a
dynamic interaction of neurotransmitters.
ī‚§ Serotonin - decrease the activity of the
RAS, thereby inducing and sustaining sleep
ī‚§ acetylcholine and nor-epinephrine appear to be
required for the REM sleep cycle

ī‚ĸ fast theta waves on the EEG.
ī‚ĸ Muscles relax.
ī‚ĸ respirations become even.
ī‚ĸ pulse rate decreases.
ī‚ĸ This stage usually lasts only a few minutes and if
awakened the person may say he or she was
not asleep.
ī‚ĸStage 2:
ī‚ĸ Bursts of sleep spindles appear on the EEG
ī‚ĸ Rolling eye movements continue and snoring.
ī‚ĸ Body functions continue to slow.
ī‚ĸStage 3 and stage 4:
ī‚ĸ delta sleep seen on the EEG.
ī‚ĸ the muscles are relaxed but muscles tone is
maintained.
ī‚ĸ respirations are even
ī‚ĸ Vital signs, urine formation and oxygen
consumption by muscle decrease.
ī‚ĸ In these stages snoring, sleepwalking and bed
wetting are most likely to occur.
Rapid Eye Movement:
ī‚ĸ REM sleep closely resembles wakefulness
except for very low muscle tone, indicated
by a reduction in amplitude of the EMG.
ī‚ĸ Blood pressure and pulse rate show wide
variations and may fluctuate rapidly.
ī‚ĸ Respirations are irregular and oxygen
consumption increases.
ī‚ĸ Vaginal secretions increases in women and
erections may occur in men.

A sleep diary is a daily account of sleeping and
walking activities. The client or personnel compile the
information in a sleep disorder clinic.

1 = slight chance of dozing or sleeping

hour intervals

ī‚ĸ Rested person take a time of atleast 15 mts for
sleep
Actigraphy:
ī‚ĸ small, wrist mounted device records activity
plotted against time, usually 1-3 weeks. there is a
correlation between the rest/activity recorded by
actigraph and wake/sleep pattern determined by
polysomnography

ī‚ĸRestless leg syndrome.
PRIMARY INSOMNIA
ī‚ĸ is troubling or difficulty in falling
asleep
ī‚ĸ Idiopathic insomnia
decreased feeling of wellbeing during the day, a
deterioration of mood and motivation, decreased
attention span, low levels of energy and
concentration and increased fatigue.
ī‚ĸ Psycho physiological insomnia
usually not sleepy during the day but function
poorly in terms of cognitive skills and also report
fatigue.
NARCOLEPSY
ī‚ĸ Narcolepsy is a condition characterized by an
uncontrollable desire to sleep
features
ī‚ĸ fall asleep while standing up,
driving a car or while swimming.
ī‚ĸ Cataplexy.
ī‚ĸ Hallucinations.
ī‚ĸ Sleep paralysis.
ī‚ĸ Disrupted night time sleep.
NARCOLEPSY
Diagnosis:
ī‚ĸ Polysomnography
ī‚ĸ Multiple sleep latency test.
Treatment:
ī‚ĸ Stimulant medications such as
methylphenidate, methamphetamine, dextro
amphetamine, and modafinil are generally
used. Dependency is usually common.
HYPERSOMNIA:
ī‚ĸHypersomnia is a condition
characterized by excessive
sleep, particularly during the day.
ī‚ĸIn some cases sleep drunkenness seen.
ī‚ĸKleine-Levin syndrome
ī‚ĸtwo to three days of sleeping 18-20
hours per day, hypersexual
behaviour, compulsive eating, and
irritability
SLEEP APNOEA SYNDROME
ī‚ĸ Sleep apnoea refers to periods of no breathing
between snoring intervals.
ī‚ĸ Obstructive sleep apnoea
ī‚ĸ Central sleep apnoea syndrome
ī‚ĸ Mixed-type sleep apnoea syndrome
ī‚ĸ there is a drop in the oxygen level of the
blood, the pulse irregular and the BP increases.
The accumulation of carbon dioxide and the fall
in oxygen cause brief periods of awakening
throughout night.
PERIODIC LIMB MOVEMENT
DISORDER
ī‚ĸ it is also called nocturnal myoclonus. In
this syndrome, sleep is disturbed by
repetitive jerky flexion movements of the
limbs which occurs in the early stages of
sleep.
ī‚ĸ Treatment includes small doses of
levodopa 100-200 mg a
night time or a dopamine
agonist.
RESTLESS LEG SYNDROME
ī‚ĸEkborn’s syndrome.
ī‚ĸUnpleasant sensations in the legs that
are ameliorated by moving the legs
occur when patient tired in the evenings
and at the onset of sleep
ī‚ĸTreatment: clonazepam 0.5 to 2 mg,
small doses of levodopa 100-200 mg or
dopamine agonists at night
EXTRINSIC SLEEP DISORDER
ī‚ĸsecondary insomnia
īļadjustment insomnia.
īļinadequate sleep hygeine.
īļinsomnia associated with psychiatric
conditions.
īļinsomnia caused by a medical
condition.
īļinsomnia caused by a drug or
substance.
Clinical features of insomnia:
ī‚ĸ Complain about inability to sleep long or well
enough to awaken feeling rested or restored.
ī‚ĸ Daytime consequences like feeling tired or
fatigued , trouble concentrating.
Diagnosis:
ī‚ĸ Sleep diaries.
ī‚ĸ Actigraphy.
Treatment:
ī‚ĸBehavioural therapy
ī‚— Stimulus control therapy
ī‚— Sleep restriction therapy:
ī‚— Relaxation therapy
ī‚— Cognitive therapy
ī‚— Sleep hygiene education
ī‚ĸmedications

PARASOMNIAS
ī‚ĸ Parasomnias are conditions associated with
activities that cause arousal or partial arousal
usually during transitions in NREM periods of
sleep.
Arousal disorders
ī‚ĸ Somnambulism:
carry an automatic motor activities
that range from simple to complex.
ī‚ĸ Sleep terrors
The child screams, exhibiting autonomic
arousal with sweating, tachycardia and
hyperventilation
ī‚ĸ Sleep-wake transition disorder
sudden jerking movements of the legs often
occurs as a person is falling asleep.
ī‚ĸ Parasomnias usually associated with
REM sleep
Nightmares are frightening dreams that arise
in REM sleep and are often vividly recalled on
awakening
Other Parasomnias
ī‚ĸ Sleep bruxism:
Bruxism is an involuntary, forceful grinding of
teeth during sleeping . treated by biofeedback
mechanism, providing rubber tooth to protect tooth.
ī‚ĸ Sleep enuresis:
Bedwetting is uncontrolled passage of urine who
have previously continent for 6-12 months.
Treatment consists of bladder training exercises
and behaviour therapy,desmopressin 0.2 mg
HS, oxybutynin chloride 5-10 mg HS or
imipramine 10-50 mg HS.

SLEEP DEPRIVATION:
Sleep deprivation refers to a decrease in the
amount, consistency and quality of sleep.
ī‚ĸ The manifestations progress from irritability and
impaired mental abilities to a total disintegration
of personality. Partial sleep deprivation may
result in loss of concentration and pose serious
safety risks. The strange environment of the
hospital, physical discomfort and pain, the effects
of medications and the need for 24 hour nursing
care may all contribute to sleep deprivation in the
hospitalized client.

Preventive strategies

ī‚ĸ Pharmacologic approaches:
ī‚§ Discontinue agents with potential to cause drug
induced sleep disturbances when possible.
ī‚§ If unable to discontinue potentially causative
agents:
*change time of administration to earlier in the day.
*reduce dose to decrease symptoms
TREATMENT OF SLEEP DISORDERS
Medications
ī‚ĸ Sedative or hypnotic medications
ī‚ĸ Benzodiazapines bind with GABA-A receptors and
modulate the effect of GABA.
ī‚ĸ Temazepam and estazolam
ī‚ĸ Diazepam is a long acting one
ī‚ĸ safer hypnotic agents are lorazepam, temazepam,
and zolpidem.
ī‚ĸ Side effects include (REM sleep rebound, daytime
memory impairment respiratory depression in patients
with pulmonary disease and may lose sleep-inducing
efficacy with prolonged use
Other Sedating Agents
ī‚ĸ In patients with chronic insomnia, 22% report using
ethanol as a hypnotic.
ī‚ĸ Over-the-counter sleeping pills contain sedating
antihistamines, usually diphenhydramine
ī‚ĸ Chloral hydrate
Anidepressants
ī‚ĸ Sedating antidepressants include the tricyclics
(amitriptyline, imipramine, nortriptyline, etc.), trazo
done, and the newer agents mirtazapine and
nefazodone.
Stimulants
ī‚ĸ Narcolepsy is treated with stimulants such as
dextroamphetamine sulfate or methylphenidate.
ī‚ĸ Psychotherapy.
ī‚ĸ Sleep education
ī‚ĸ Lifestyle changes

  • 2. DEFINITION OF SLEEP ī‚ĸ Sleep is a naturally occurring altered state of consciousness characterized by decreases in awareness and responsiveness to stimuli.
  • 3. PHYSIOLOGY OF SLEEP ī‚ĸ Controlled by recticular-activating system and bulbar synchronizing system. ī‚ĸ Wakefulness occurs when the reticular system is activated. ī‚ĸ The hypothalamus has control centres for several involuntary activities of the body, one of which concerns sleeping and waking. ī‚ĸ Injury to the hypothalamus may cause a person to sleep for abnormally long periods.
  • 4. CIRCADIAN RHYTHMS ī‚ĸ Biological rhythms that follow a cycle of about 24 hours are termed circadian rhythms ī‚ĸ circa means ―about and dies means ―day ī‚ĸ Ciracdian synchronization exists when an individual sleep-wake pattern follows an inner biological clock. ī‚ĸ when physiologic and psychological rhythms are high or most active, the person is awake and when these rhythms are low, the person will sleep

PHYSIOLOGIC FUNCTION ī‚ĸ Electro- physiologic Approach electro-physiologic changes in brain waves, eye movements, and muscles show five sleep stages. ī‚ĸ Neurotransmitter Balance: ī‚§ Involves the reticular activating system (RAS) and a dynamic interaction of neurotransmitters. ī‚§ Serotonin - decrease the activity of the RAS, thereby inducing and sustaining sleep ī‚§ acetylcholine and nor-epinephrine appear to be required for the REM sleep cycle
  • 6. SLEEP CYCLE: NREM NREM NREM STAGE 1 STAGE 2 STAGE 3 REM SLEEP 90-100 NREM MNTS STAGE 4 NREM NREM STAGE 2 STAGE 3
  • 7. ī‚ĸStage 1: ī‚ĸ fast theta waves on the EEG. ī‚ĸ Muscles relax. ī‚ĸ respirations become even. ī‚ĸ pulse rate decreases. ī‚ĸ This stage usually lasts only a few minutes and if awakened the person may say he or she was not asleep. ī‚ĸStage 2: ī‚ĸ Bursts of sleep spindles appear on the EEG ī‚ĸ Rolling eye movements continue and snoring. ī‚ĸ Body functions continue to slow.
