Answers:
1. Im DJSM, 22 years old, live in Tisa, Cebu City. My father was a retired AFP
officer and my mother is a Public teacher. I am the youngest of five children. I
graduated my highschool at USC-BHS and college in Velez College. Now Im
studying at CNU taking up my master’s degree majoring Medical-Surgical
Nursing.
2. Im good in singing, reporting, computer editing and some other staff, but I am
willing mam to learn anything just in the context of my value.
3. Actually this is my second time to apply for this position in your office mam, I
don’t know why I am not accepted in my first attempt maybe the conflict between
my schedule would be your major reason mam of not accepting, but this time
mam I hope that I will be accepted, and assure you mam that I will not miss up
and to my best to do my job. Applying for this position mam would give me an
hospital experience and give me a chance to develop further my skills in
performing different hospital procedures and providing nursing services to the
patient.
4. I just call the hospital mam, and ask if they are hiring for Volunteer Staff nurse
position or Staff nurse position and they said to me that they are hiring for
Volunteer staff nurses so I immediately pass my resume, TOR, and application
letter at your office.
5. I am unique mam in a way that I can be easily teach, I’m not hard headed, I am
friendly and most especially I am hardworking, and I am wiling to learn new

2. things. I am open minded and very optimistic on the way how I look up life and I
hope that I would bring this attitude into your institution and to the patients of this
hospital.
6. Off course mam, Im always punctual and very honest also. My integrity as a
person in very intact and I have a good value that I follow in my life.
7. I choose this hospital because it is near to our house, transportation is available. I
have friends also in this hospital. I have heard also that this hospital is a good
training ground for me as a new graduate nurse, and hopefully this hospital will
teach me more in this carrier I choose.
8. If you accept me mam, the good quality that I would bring in this institution is me
being a good teammate to my co-health workers. I have a value of a good nurse. I
can influence others through my action. I have the quality of being a good leader.
To tell you frankly mam, I don’t have any experience yet. Accepting me is my
first time to be in a job. I am not promising you anything mam, but I always do
my best to protect the good name of this institution.
9. Off course, If you like to change the nurses note to FDAR from SOAPIE, I am
willing to help you mam, it is my pleasure to lend my help to you. I am quit
familiar about FDAR, this is use by our school in writing our notes in the patient
charts.
10. I will use the competencies that suit to the needs of the patients. In dealing patient
safety, I will use safe and quality nursing care, where I can utilize the nursing
process in making assessment to my patient. Concerning about management of
resources, I will maintain a safe environment for the patient. For Health education
I can assess the learning needs of the patient and family. For Legal responsibility,
I will make sure that I always put into documents the care I rendered to m
patients. For Ethico Moral, I will always respect the rights of individual. For
quality improvement, I will go and participates in nursing audits and rounds every
shift. For Communication, I will make sure that I establish rapport to my patient
to gain their trust. For Collaboration and Teamwork, I will collaborate my plan of
care done to my patient with the other members of the health team.
11. In caring for our patients we always follow the nursing process even do now, we
have these protocols. And we make sure that the care we rendered to our patients
is based on competencies that we observed and followed.
12. I had attended a seminar on Basic Life support twice. I am also presently enrolled
in a Masteral class at CNU.
