7. Adoption certificate decree with a close relative's
affidavit (preferably the mother of the applicant) with
information about the date and place of birth,
parents' names, and the maiden name of the mother.

8. Certified prison and court records that include the
circumstances of the crime of which the applicant was
convicted and the nature of the case including the
fine, penalty, or sentence that was imposed.

9. Deportation documents must be provided by
applicants who have been deported or removed from
the U.S. by completing Form I-212 (Permission to
Reapply after Deportation) from the U.S. Citizenship
and Immigration Services or a U.S. Embassy or
Consulate.

10. Police records must be submitted for applicants 16
years or older that covers the entire period of his or
her residence in that area, and it must include all
arrests, the reasons for the arrests, and the
dispositions of each recorded case.

11. All military records must be submitted.

12. Marriage certificate must be submitted.

13. All custody documents must be submitted.
