We have attached the following 4 files to this e-mail. These are Word documents. If you do not
have Word on your computer let us know immediately.
1. Fiancée Visa Questionnaire --- This document must be filled out by you. It is not a
government form. We fill out your US Government immigration forms based on the information
you give us in this questionnaire. You will only send this questionnaire to us one time.
2. Sample Letter of Intent --- You will need to write a draft of your letter showing your intent to
marry your fiancée within 90 days of your arrival in the U.S. You can use this one as a
template and add other information. Do not sign.
3. Color Photo Specifications --- This document has the requirements for taking the USA
passport-style photo, including the correct size, pose and background.
4. Visa Process---This document gives you an overview of the visa process so you know what
to expect.

B. The completed letter of intent.
C. A scanned copy of your birth certificate and all divorce, annulment or death
certificates, if applicable. If from the Philippines, documents must be issued by the
National Statistics Office.
2. D. Scanned copies of certified birth certificates for ALL your children, even if they are
not traveling with you.
E. If documents are not in English, scanned copies of certified English translations of
documents with information on the translation company.
WHAT WE DO
We complete your US government immigration forms based on the information you give us in
the questionnaire. When we complete the government forms we will e-mail them to you to
review.
After all forms have been reviewed and approved by you, we will give you instructions to sign
and date the appropriate forms and scan the signed forms for our approval. You will then
express-mail or FedEx to our office the signed forms and the other supporting documents.
Once we receive your forms & supporting documents and all of your fiance’s forms & supporting
documents we will do a final review and then mail the completed visa package to your fiancé to
file with USCIS.

Please note:
You (and children traveling) must have a valid passport at the time of your interview. If
you do not have a passport, please apply for one right away!
Do not obtain local police clearances until instructed by the US Embassy. These
documents are time sensitive.
Other countries police clearances: Applicants aged 16 years and older must also present
police certificates from other countries where they have lived for 6 months or more after
reaching the age of 16. In order to obtain these police clearances you must present a letter from
the US Embassy which you will receive after your visa has been approved. Please wait for
further instructions.
