As part of the Building Health Partnerships programme SS&SP are launching a social prescribing pilot. This presentation was given as part of an action learning day on social prescribing hosted by Swindon Building Health Partnerships group. 1. Overall aims:
 To increase the CCG’s awareness of the VCSE sector and its value and
contributions to health outcomes
 To work in partnership to develop a model of commissioning that takes a
holistic approach to the needs of the individual and embeds social value
 To develop a joined-up, cross-sector approach that links statutory health
provision with community based VCSE providers

2. SESSP CCG’s Priority Areas:

3. Stage 1:Mapping VCSE offer
 Desk research to map all services/support available in key priority areas
 Identify what is happening already and gaps in data
 Consider use relevance and value of existing case studies
 Research quick update mechanism for datasets

4. Stage 2: NEW Health Improvement Partnership
 Aimed at VCSE groups operating in the SESSP CCG area and providing services
and/or activities to older, vulnerable people and those with long term health
conditions.
 Offers these organisations the opportunity to be closely involved in testing a new
model for meeting local community health needs and the commissioning of new
health related services.
 Identifies volunteers who can hold one-to-one conversations with
beneficiaries/service users/members about their isolation and health issues.
 Volunteers are invited to a half day training session to ensure they capture the
information needed.
 The impact of the project on participating volunteers will be assessed to measure
additional social value

5. Stage 3: Identifying Need
 From the conversations we will be able to identify local health needs and the
best way to meet those needs either from existing services or by
commissioned pieces of work.
 These targeted interventions will reduce social isolation for the beneficiaries
 A reliable data-set will result that will shape future service provision in the
CCG area and pilot a new approach to outcomes commissioning of health
services for particular groups working within particular themes.
