3. • Political: Many elderly individuals have federal
insurance (Medicare) dictating what their access to
Alzheimer's care looks like, regardless of patient
knowledge
• Economic: Long-term care facilities focusing on
dementia care are very expensive so out of pocket
expenses effect medical choices
• Social: Elderly individuals and caregivers face isolation
due to limited access to community, either
transportation or supports
• Technology: Elderly individuals, especially those with
memory and cognitive issues, face challenges in a
technology focused world, possibly causing more
isolation

4. Client Culture
and Diversity
• Majority of our small, midwestern community is 65+,
Caucasian, with a history in agriculture business
• However, families and support systems do reflect the
changing Midwest demographics
• To encourage help for all demographic, educational
services would be accessible in other languages –
especially Spanish- to support individuals where
English is not their first language

• For safety: volunteer names, times and locations
with residents, resident names
• For growth: monthly depression screenings on
patients

found helpful to provided more focused information

monthly to keep presented information up-to-date
and relevant to participants
• Socialization services will review depression
screenings monthly, with a goal of seeing
decreased scores with increased socialization
• Locations of services will also be reviewed monthly
to brainstorm increasing use of community

• Elderly adults and family/support systems face an unknown and undereducated world
when facing Alzheimer's Dementia
• Elderly adults and their caregivers can face increased social isolation, related to higher
rates of depression and lower overall wellbeing
• Without support, these populations may face declining health outcomes and quality of life
throughout their journeys
• Interventions provided by Wiser Years aim to prevent negative health outcomes for both
patients and their support system
• Education and socialization are used as tools to empower these family units in their
Alzheimer's journeys for educated care choices and promoting mental health
