52. CANCER DE VEJIGA DETECCION
Painless gross hematuria occurs in 85% of patients with
bladder cancer and requires a complete evaluation that
includes cystoscopy, urine cytology, CT scan, and a PSA
blood test.
Patients with microscopic hematuria require a full evaluation,
but low-risk patients do not require repeat evaluations.
High-risk individuals primarily are those with a smoking
history and should be evaluated every 6 months.
lWhite light cystoscopy with random bladder biopsies is the
gold standard for tumor detection, but blue light cystoscopy
may be an adjunct.
There are various urine markers that evaluate secreted proteins
or shed cells in the hope of noninvasively detecting
bladder cancer.To date, none of these markers have a high
enough sensitivity or specificity to replace office
cystoscopy.

l Intravesical chemotherapy has a clear impact on tumor

Immunotherapy
l Intravesical BCG has higher efficacy than intravesical
chemotherapy.
l BCG should be used cautiously for patients with low-risk
disease because of concern about side effects.
l BCG is the only agent shown to delay or reduce high-grade
tumor progression.
lThe optimum dosage and the treatment schedule for BCG
are undetermined, but results are better with maintenance
therapy, if tolerated.
l BCG is contraindicated in the setting of a disrupted urothelium

Management of Refractory Disease
l Patients who fail to respond to an initial course of intravesical
therapy afterTURBT are at high risk of recurrence or
progression.
l Failure after initial chemotherapy or BCG is most appropriately
treated with a subsequent course of BCG because its
efficacy in this setting is significantly greater than that of
chemotherapy.
l Patients at high risk for progression should be considered
for cystectomy.
l Failure to respond to an initial course of intravesical therapy
is occasion to reconsider cystectomy. Failure to respond to
a second course is an indication for immediate cystectomy
unless contraindicated or the patient chooses to pursue
clinical trials.

l Cystoscopy is the hallmark of surveillance.The optimum
schedule is undefined but may be individualized on the

non-muscle invasive recurrences is critical since a delay in
diagnosis and therapy threatens a patient’s life.
b.Tumour recurrence in the low-risk group is nearly always
low stage and low grade. Small, non-invasive (Ta), lowgrade
papillary recurrences do not present an immediate
danger to the patient and their early detection is not essential
for successful therapy.

92. Recommendations for follow-up cystoscopy
• Patients with tumours at low risk of recurrence and
progression should have a cystoscopy at 3 months.
If negative, the following cystoscopy is advised at 9
months and consequently yearly for 5 years. (Grade of
recommendation: C)
• Patients with tumours at high risk of progression should
have a cystoscopy and urinary cytology at 3 months.
If negative, the following cystoscopies and cytologies
should be repeated every 3 months for a period of 2
years, every 4 months in the third year, every 6 months
thereafter until 5 years, and yearly thereafter.
A yearly exploration of the upper tract is recommended.

• Patients with intermediate-risk of progression (about
one-third of all patients) should have an in-between follow-
up scheme using cystoscopy and cytology, adapted

LAS PROBABILIDADES EN LA VIDA DE DESARROLLAR UN CANCER POR SITIO U ORGANO, EN VIAS URINARIAS Y GENITALES MASCULINOS ES DE UNO EN SEIS PARA LA PROSTATA , DE CANCER EN LA VEJIGA EXISTE UNA POSIBILIDAD EN 26, RIÑÓN DE UNO EN 55 Y EN EL TESTICULO ES 1 EN 500. EN UN ESTUDIO EUROPEO DEL AÑO 2001 SE REVISO LA INCIDENCIA DE NUEVOS DIAGNOSTICOS DE TUMORES GENITOURINARIOS ENCONTRANDOSE QUE EL MAS COMUNMENTE DTRECTADO FUE EL CANCER DE PROSTATA, El cáncer vesical constituyó el 44,89% de los nuevos diagnósticos (519/1159), el cáncer renal el 5,7% (66 casos), el testicular el 2,5% (29 casos) y por último, el cáncer de pene el 0,77% (9 casos). Las Tablas 1 y 2reflejan los resultados de esta parte del estudio.
LAS DIEZ PRINCIPALES CAUSAS DE CONSULTA DE PRIMERA VEZ EN EN EL INCAN EL AÑO PASADO EL TUMOR GENITOURINARIO MAS COMUNMENTE CONSULTADO ES EL DE TESTICULO (212 CASOS) DESPUES DE LA MAMA Y EL CUELLO UTERINO, SIGUIENDO EN ORDEN DECRECIENTE DE FRECUENCIA EL CANCER DE OVARIO, ESTOMAGO, MALIGNIDADES DE LA PIEL Y PROSTATA CON 193 CASOS. CANCER DE COLON SE ENCONTRÓ EN DÉCIMO LUGAR.
ENTRE TODAS LAS MALIGNIDADES, EXISTEN DIFERENCIAS DE TASAS DE MORTALIDAD INEGI, 2006-2007). EN MEXICO COMPARATIVAMENTE ENTRE AMBOS SEXOS: CANCER DE VIAS URINARIAS CON CIFRAS MAS ALTAS EN EL HOMBRE, 3.5% QUE EN LA MUJER DE 2.2%, CANCER DE PROSTATA EN UN 16.5 POR CIENTO EN PROMEDIO (2007) CONTRA EL CA DE OVARIO OVARIO DEL 5% Y LIGERAMENTE MAS ELEVADA QUE EL EL DE MAMA EN LA MUJER (14%)
DENTRO DE LAS INCIDENCIAS DE LOS TUMORES GENITOURINARIOS, EL CANCER DE PROSTATA ENCABEZA LA LISTA COMO LO MENICONÉ EN LA PRIMERA DIAPOSITIVA. LE SIGUE LA VEJIGA, RIÑÓN Y TESTICULO.
EN CAMBIO LA MORTALIDAD POR ENFERMEDAD AVANZADA ES PROPORCIONALMENTE INVERSA EN TESTICULO, RIÑON, VEJIGA Y PROSTATA Y PENE CON LOS MAS PEQUEÑOS PORCENTAJES DE MORTALIDAD
ESTA incidencia de tumores malignos ha ido aumentando en todo el mundo y se espera que mas pacientes van a morir por enfermedad avanzada, no en etapas iniciales. Las consecuencia será que se necesitará mas y mas tratamientos principalmente medicamentos. MENCIÓN APRTE, SE CREE QUE EL 30 AL 40 POR CIENTO DE LOS TUMORES AVANZADOS SERÁN TRATADOS POR QUIMIOTERAPIA
ESTO NOS LLEVA A DEFINIR BIEN EL PAPEL DE UROLOGO EN EL MANEJO DE ESTAS ENFERMEDADES MALIGNAS ES CIERTO QUE EL CIEN POR CIENTO de los tumores benignos, LO MANEJAMOS LOS UROLOGOS
Nosotros mismos hemos tratado las enfermedades benignas con nuestros tx habituales.
LA ENFERMEDAD MALIGNA las atendemos con quimioterAPIA Intravesical o inmunoterapia con BCG, antiandrogenos, analogos ó aun los bifosfonatos y algunos citotóxicos y terapias blanco.
EN OTRAS NO POCAS OCACIONES nos vemos limitados para manejar algunas enfermedades malignas localizadas, localmente avanzadas y otras recurrentes, en la cuales es necesario contar con procedimientos radio ó quimioterapeuticos con el equipo y personal adecuado principalmente en tratamientos medicamentosos adyuvantes o neoadyuvantes de ca de vejiga, no se diga en el el manejo paliativo de las enfermedad avanzada. CONCLUYO QUE LA META DE TODOS LOS MEDICOS QUE ATENDEMOS EL CANCER ES DETECTAR Y ESTADIFICAR LA ENFERMEDAD DESDE EL INCIO PARA DECIDIR EL PRIMER TRATAMIENTO Y ABATIR LA MORTALIDAD Y MORBILIDAD DE TRATAMIENTOS AGRESIVOS DE LA ENFERMEDAD AVANZADA

Detection of Urothelial Cancer
l Painless gross hematuria occurs in 85% of patients with
bladder cancer and requires a complete evaluation that
includes cystoscopy, urine cytology, CT scan, and a PSA
blood test.
l Patients with microscopic hematuria require a full evaluation,
but low-risk patients do not require repeat evaluations.
High-risk individuals primarily are those with a smoking
history and should be evaluated every 6 months.
l White light cystoscopy with random bladder biopsies is the
gold standard for tumor detection, but blue light cystoscopy
may be an adjunct.
l There are various urine markers that evaluate secreted proteins
or shed cells in the hope of noninvasively detecting
bladder cancer. To date, none of these markers have a high
enough sensitivity or specificity to replace office
cystoscopy.

Se ha demostrado el riesgo significativo de tumor residual despues de la RTU inicial de lesiones
TaT1 En el 33 %‑53 % de los pacientes se observa una enfermedad persistente tras la reseccion de tumores T1
Ademas, es posible que la reseccion inicial clasifique el tumor por debajo de su estadio real. La
probabilidad de que un tumor de alto grado TaT1 se haya clasificado por debajo de su estadio
y que, por tanto, presente invasion muscular es del 10 % Dado que el tratamiento de un
tumor de alto grado TaT1 y un tumor T2 es completamente diferente, es importante una estadificacion
correcta.
Ha de contemplarse una segunda RTU cuando la reseccion inicial haya sido incompleta, por
ejemplo, cuando hay tumores multiples o grandes, o cuando el anatomopatologo indique que la
muestra no contenia tejido muscular. Ademas, ha de practicarse una segunda RTU cuando en la
inicial se detecte un tumor de alto grado sin invasion muscular o un tumor T1.
Se ha demostrado que una segunda RTU aumenta la supervivencia sin recidivas. No hay unanimidad con respecto a la estrategia y el momento de
la segunda RTU. La mayoria de los autores recomiendan la reseccion al cabo de 2‑6 semanas de
la RTU inicial. La intervencion debe incluir la reseccion del foco tumoral primario.
EL PASO ESENCIAL EN EL DIAGNOSTICO DE CA DE VEJIGA ES EL EXAMEN ANATOMOPATOLOGICO DE LA O LAS PIEZAS OBTENIDAS POR RTU O BIOPSIA. EL MEDICO PATOLOGO DEBE ESPECIFICAR EL GRADO DE LA LESIÓN Y LA PROFUNDIDAD DE LA LESIÓN TUMORAL, OBTENER INFORMACIÓN ACERCA DE SI HAY LAMINA PROPIA Y MUSCULO PRESENTES
LA FORMA CLASICA DE CLASIFICAR A LOS PACIENTES CON TUMORES T1 Y Ta CONSISTE EN ESTRATIFICARLOS EN GRUPOS DE ALTO Y BAJO RIESGO SEGÚN LOS FACTORES CLINICOS Y ATOMOPATOLOGICOS DERIVADOS DE ANALISIS MULTIFACTORIALES CON EL FIN DE PREDECIR LOS RIESGOS A CORTO Y LARGO PLAZO DE RECIDIVA O RECURRENCIA DEL TUMOR Y LA PROGRESION
Prognostic Factors and Adjuvant Treatment
Since there is considerable risk for recurrence and/or progression
of tumours after transurethral resection, adjuvant intravesical
therapy is recommended for all stages (Ta, T1 and Tis).
All patients should receive one immediate post operative
instillation of chemotherapy within 6 hours after TUR. The
immediate instillation is considered as standard, the choice of
drug (mitomycin C, epirubicin or doxorubicine) is optional.
The choice of further intravesical adjuvant therapy depends on
the patient’s risk of recurrence and/or progression which can
be assessed using the EORTC scoring system (Table 3) and
risk tables (Table 4).

within the normal-appearing urothelium.
l Recurrent urothelial tumors occur by activation of nascent
normal cells that have some genetic instability by environmental

l Accumulation of genetic changes leads to cellular proliferation,
loss of cell–cell adhesion, and invasion.
l The depth of invasion and grade of the tumor are the best
prognostic determinants of urothelial cancer, but molecular
assays are likely to be incorporated into future staging
schemas.

Intravesical Chemotherapy
l Intravesical chemotherapy has a clear impact on tumor
recurrence when immediately instilled after TURBT and in
the adjuvant setting. There is no clear evidence of an impact
on progression.
l Combinations of various chemotherapeutic agents and chemotherapy
combined with BCG have not demonstrated
major benefit combined with single-agent treatment, with
the exception of interferon.
l In general, side effects of chemotherapy tend to be less
common and less severe than those for BCG, but BCG is
more efficacious.
Immunotherapy
l Intravesical BCG has higher efficacy than intravesical
chemotherapy.
l BCG should be used cautiously for patients with low-risk
disease because of concern about side effects.
l BCG is the only agent shown to delay or reduce high-grade
tumor progression.
l The optimum dosage and the treatment schedule for BCG
are undetermined, but results are better with maintenance
therapy, if tolerated.
l BCG is contraindicated in the setting of a disrupted urothelium
because of the risk of intravasation and septic death.
l Interferon-α has not been shown to have benefit compared
with BCG for primary treatment but appears to work well
in combination with low-dose BCG, especially for salvage
CISTECTOMÍA PARA EL CÁNCER DE VEJIGA SIN INVASIÓN MUSCULAR
Muchos expertos consideran razonable proponer una cistectomia inmediata a los pacientes con un
tumor sin invasion muscular que corren un riesgo elevado de progresion. Segun las tablas de riesgo
de la EORTC (veanse las tablas 3 y 4), se trata de:
• múltiples tumores de alto grado recurrentes
• tumores T1 de alto grado
• tumores de alto grado con CIS concomitante.
En los pacientes sin invasion muscular con fracaso del BCG se recomienda una cistectomia tal
como se ha mencionado anteriormente. El aplazamiento de la cistectomia en estos pacientes puede
conllevar una disminucion de la supervivencia especifica de la enfermedad (73).
Recommendations for Cistoscopia de seguimiento
• pacientes con Riesgo bajo de recidiva y progresión –a los 3 meses. Si esta sale
NEGATIVA-la cistoscopia se realiza a los 9 meses y, posteriormente, una vez al ano durante 5 anos. (Grado
de recomendacion: C)
• Los pacientes con tumores con un riesgo alto de progresión deben someterse a una cistoscopia
y una citologia urinaria al cabo de 3 meses. En caso de ser negativas, las siguientes
cistoscopias y citologias han de repetirse cada 3 meses durante un periodo de 2 anos, cada
4 meses en el tercer ano, luego cada 6 meses hasta los 5 anos y anualmente a partir de
entonces. Se recomienda una exploracion anual de las vias superiores. (Grado de recomendacion:
C)
• Los pacientes con un riesgo intermedio de progresión (en torno a un tercio de los pacientes)
deben seguir un esquema de seguimiento intermedio con cistoscopia y citologia, adaptado
en funcion de factores personales y subjetivos
Surveillance and Prevention
l Cystoscopy is the hallmark of surveillance. The optimum
schedule is undefined but may be individualized on the
basis of risk.
l Table 81–8 demonstrates reasonable surveillance protocols
based on clinical scenarios. Guidelines for management are
shown in Table 81–9.
l A number of tumor markers have shown the ability to
improve upon the sensitivity of cytology, but specificity is
lower for most.
l Increased fluids, smoking cessation, and a low-fat diet are
recommended.
Histological Diagnosis
The diagnosis mainly depends on the cystoscopic examination
of the bladder, biopsy and urine cytology. To date, molecular
urinary markers have not improved the combination of cystoscopy
and cytology.
The standard therapy for Ta and T1 papillary bladder tumours
is complete macroscopic eradication by transurethral resection
(TUR) including a part of the underlying muscle. A second
TUR should be considered if there is a suspicion that the initial
resection was incomplete, e.g. when multiple or large tumours
are present or when the pathologist reported no muscle tissue
in the specimen or when a high-grade tumour or a T1
tumour was detected.
Tis cannot be eradicated by transurethral resection.
The diagnosis of Tis is made by multiple biopsies from the bladder wall
in conjunction with urine cytology. Fluorescence cystoscopy
improves the detection rate of Tis.
PACIENTES CON GRANDES Y MULTIPLES TUMORES (MAS DE 3 CMS) Y ALTAMENTE RECURRENTES (MAS DE UNA RECURRENCIA POR AÑO) TIENEN MAS RIESGO DE RECURRENCIA MIENTRAS QUE PACIENTES CON TUMORES ESTADIO T1, ALTO GRADO Y Cis , TIENEN EL MAYOR RIESGO DE PRORESIÓN.
Patients with multiple tumours, large
tumours (> 3 cm), and highly recurrent tumours (> 1 recurrence/
year) are at the highest risk of recurrence while patients
with stage T1 tumours, high grade tumours, and CIS have the
highest risk of progression.
Follow-up for non-muscle invasive bladder tumours
Because of the risk of recurrence and progression, patients
with non-muscle invasive bladder tumours need to be followed;
however, the frequency and duration of cystoscopies
should reflect the individual patient’s degree of risk. Using risk
tables (see Tables 3 and 4), we are able to predict the shortterm
and long-term risks of both recurrence and progression
in individual patients and can adapt the follow-up schedule
accordingly:
a. The prompt detection of muscle invasive and high-grade
non-muscle invasive recurrences is critical since a delay in
diagnosis and therapy threatens a patient’s life.
b. Tumour recurrence in the low-risk group is nearly always
low stage and low grade. Small, non-invasive (Ta), lowgrade
papillary recurrences do not present an immediate
danger to the patient and their early detection is not essential
for successful therapy.
c. The result of the first cystoscopy after TUR at 3 months is
a very important prognostic factor for recurrence and for
progression. The first cystoscopy should thus always be
performed 3 months after TUR in all patients with nonmuscle
invasive bladder tumour.
Recommendations for follow-up cystoscopy
• Patients with tumours at low risk of recurrence and
progression should have a cystoscopy at 3 months.
If negative, the following cystoscopy is advised at 9
months and consequently yearly for 5 years. (Grade of
recommendation: C)
• Patients with tumours at high risk of progression should
have a cystoscopy and urinary cytology at 3 months.
If negative, the following cystoscopies and cytologies
should be repeated every 3 months for a period of 2
years, every 4 months in the third year, every 6 months
thereafter until 5 years, and yearly thereafter.
A yearly exploration of the upper tract is recommended.
(Grade of recommendation: C)
• Patients with intermediate-risk of progression (about
one-third of all patients) should have an in-between follow-
up scheme using cystoscopy and cytology, adapted
according to personal and subjective factors. (Grade of
recommendation: C)
l Muscle-invasive bladder cancer leads to death in a significant
proportion of patients despite aggressive therapy.
EN FORMA GENERAL ESTOS SON LOS FACTORES RESPONSABLES DEL CANCER DE VEJIGA
Characteristics of Stages Ta, T1 and Tis
Stage Ta tumours are confined to the urothelium, have a papil-
lary configuration of their exophytic part and do not penetrate
from the urothelium into the lamina propria or detrusor muscle.
Stage T1 tumours generate from the urothelium but penetrate
the basement membrane which separates the urothelium from
the deeper layers. T1 tumours invade into the lamina propria,
but not so deep that they reach the detrusor muscle.
Carcinoma in situ (Tis) is a high-grade (anaplastic) carcinoma
confined to the urothelium, but with a flat non-papillary configuration.
Tis can be local or diffuse and it can be concomitant
with papillary tumours. Unlike a papillary tumour, Tis appears
as reddened and velvety mucosa and is slightly elevated but
sometimes not visible.
ESTUDIO DE SANGRE EN ORINA
SE ACEPTA GLOBALMENTE QUE LA HEMATURIA ES EL HALLAZGO MAS FRECUENTE (85%) EN LOS TUMORES VESICALES Ta Y T1. NO CAUSA DOLOR Y RARA VEZ CURSAN CON IRRITACIÓN VESICAL, DISURIA O TENESMO. ESTOS SINTOMAS DE IRRITACIÓN DEBE AUMENTAR LA SOSPECHA DE UN CA DE ALTO GRADO O UN CA IN SITU
SE HA DETERMINADO UNA MEDIA DE RETRASO DE HASTA 177 DIAS CASI SEIS MES, EN EL DIAGNOSTICO DESDE QUE SE SOLICITA LA CONSULTA a mediana de retraso desde que se solicitó la interconsulta con urología desde atención primaria hasta el diagnóstico fue de 77 días, siendo la media 174 días y la media de 43.
Un 20% (9/46) de los tumores uroteliales analizados no se presentaron con hematuria macroscópica. Los más frecuentes de este grupo fueron el carcinoma de vejiga (8 casos) y los del parénquima renal (1 caso). El 95% de las neoplasias se diagnosticaron en varones, siendo fumadores 1 de cada 4 casos.
