A Trigger Point (TrP) is a palpable nodule in the taut bands of the skeletal
muscles' fascia. Direct compression or muscle contraction can elicit jump
sign(individuals are frequently startled by the intense pain), local tenderness,
local twitch response and referred pain which usually responds with a pain
pattern distant from the spot.
The following approach to patient care is designed to help to focus in on the
problem at hand and its resolution:
1. Clearly define the areas of pain and restrictions of movement that the patient is
experiencing.
2. Determine the various muscles that might be the source of your patient’s pain
and restriction.
3. Palpate for constrictions and taut bands in the individual muscles.
4. Palpate associated regions for additional constrictions and taut bands. It is
important to palpate the entire body, anterior and posterior, to determine
associated constrictions.

5. Locate individual taut bands in the involved muscles. “Capture” the band with
precise palpation and compression.
6. Apply treatment to the trigger points. Once the bands and trigger points are
captured, maintain pressure through ischemic compression until softening of the
point occurs. Palpation of the region after treatment will provide feedback as to
whether or not there has been a complete release of the muscle.
7. Apply moist heat(to increase blood circulation),stretching, strengthing exercise

STERNOCLEIDOMASTOID
Proximal attachment: Mastoid process and the lateral
half of the superior nuchal line of the occipital bone.
Distal attachment: Clavicular head: superior border of
the anterior surface of the medial one-third of the
clavicle. Sternal head: anterior surface of the
manubrium, medial and more superficial than the
clavicular head.
Action: Acting unilaterally: rotation of the face to the
opposite side and lifting
the chin. Acting bilaterally: flexion of the head and neck

Stretch exercises:
1. Clavicular head: Bend head and neck backward, rotating the face to one side.
The muscle will be stretched at the clavicular head on the opposite side.
2. Sternal head: Turn head to one side, then at full rotation tilt the chin toward the
shoulder. The muscle will be stretched at the sternal head on the same side.

Proximal attachment: Transverse processes of C2–C7.
Distal attachment: Anterior and medius: first rib. Posterior: second rib.
Action: Acting unilaterally: lateral flexion of the cervical spine. Acting
bilaterally: stabilization of the cervical spine against lateral movement;
elevation of the first and second ribs to assist in inspiration.

TRAPEZIUS
Proximal attachment: External occipital protuberance, nuchal
ligament, spinous processes of C1–T12.
Distal attachment: Spine of the scapula, acromion, lateral one-third
of the clavicle.
Action: Upper fibers: flexion of the head and neck to the same side;
elevation of the shoulder acting on the clavicle and the acromion.
Middle fibers: retraction of the scapula. Lower fibers: depression of
the scapula, rotation of the glenoid fossa upward.

Proximal attachment: Spinous processes of C7–T5.
Distal attachment: Medial border of the scapula.
Action: Elevation and retraction of the scapula.

SUPRASPINATUS
Proximal attachment: Supraspinatus fossa of the scapula.
Distal attachment: Upper part of the greater tubercle of the
humerus.
Action: Assists the deltoid in abduction of the humerus by
pulling the head of the humerus inward toward the glenoid
fossa. Aids in stabilizing the head of the humerus in the
glenoid fossa.

INFRASPINATUS
Proximal attachment: Medial two-thirds of the infraspinatus
fossa, which lies distal to the spine of the scapula.
Distal attachment: Posterior aspect of the greater tuberosity
of the humerus, distal to the attachment of supraspinatus.
Action: External rotation of the arm; aids in stabilizing the
head of the humerus in the glenoid fossa during upward
movement of the arm, assisted by teres minor.

LATISSIMUS DORSI
Proximal attachment: Latissimus dorsi fuses with teres
major to attach at the medial edge of the bicipital groove, on
the anterior aspect of the humerus.
Distal attachment: Spinous processes of T6–T12, L1–L5 and
the sacrum, iliac crest via the thoracolumbar aponeurosis,
lower 3–4 ribs, inferior angle of the scapula.
Action: Extension, adduction, and internal rotation of the arm
at the shoulder.

Proximal attachment: Long head: supraglenoid tubercle of
the scapula.
Short head: coracoid process of the scapula.
Distal attachment: Tuberosity of the radius.
Action: Flexion of the forearm at the elbow; assists in flexion
of the arm at the shoulder. Aids in supination of the forearm
against resistance when the elbow is flexed.

Proximal attachment: Coracoid process of the scapula.
Distal attachment: Anterior chest wall, on the upper
surfaces of ribs 3, 4, and 5.
Action: Pulls the scapula and shoulder down and forward;

Proximal attachment: Medial two-thirds of the clavicle; sternum;
costal cartilages of ribs 2–7; superficial aponeuroses of the external
oblique and rectus abdominis.
Distal attachment: Lateral lip of the bicipital groove of the
humerus.
Action: Adduction and medial rotation of the arm. Clavicular
fibers: flexion of the arm, as in touching the lobe of the opposite
ear.

Proximal attachment: Anterior bodies and intervertebral
discs of T12–L5; upper two-thirds of the iliac fossa.
Distal attachment: Lesser trochanter of the femur.
Action: Flexion of the thigh at the hip when the torso is
fixed; flexion of the hip on the thigh when the thigh is fixed;
assists in extension of the lumbar spine (increasing lordosis)
in the standing position.

GLUTEUS MAXIMUS
Proximal attachment: Posterior iliac crest, lateral sacrum,
and coccyx.
Distal attachment: Iliotibial band of the fasciae latae and
the gluteal tuberosity of the femur.
Action: Powerful extension of the thigh at the hip during
strenuous activities such as running, jumping, stair
climbing, and in rising from a seated position;
helps maintain an erect posture; assists in lateral rotation
of the hip. Upper fibers: abduction of the thigh. Lower
fibers: adduction of the thigh.

TENSOR FASCIAE LATAE
Proximal attachment: Anterior iliac crest, just posterior to
the anterior superior iliac spine (ASIS).
Distal attachment: Through the iliotibial band to the lateral
condyle of the tibia.
Action: Assists flexion, abduction, and internal rotation of
the thigh; helps stabilize the knee. Aids gluteus medius and
gluteus minimus in stabilizing the pelvis during walking.

Proximal attachment: Anterior surface of the sacrum.
Distal attachment: Through the sciatic foramen,
attaching to the greater trochanter of the femur.
Action: External rotation of the thigh; acts in abduction

Biceps femoris Semitendinosus Semimembranosus
HAMSTRINGS
Proximal attachment: Biceps femoris, long head:
ischial tuberosity short head: linea aspera of the femur
Semitendinosus: on the ischial tuberosity, by a common
tendon with the long head of biceps femoris.
Semimembranosus: on the ischial tuberosity, lateral and
deep to the common tendon of biceps femoris and
semitendinosus.
Distal attachment: Biceps femoris: both long and short
heads attach by a common tendon to the posterior and
lateral aspects of the head of the fibula. Semitendinosus:
medial side of the superior part of the tibia, medial.
Semimembranosus: posterior aspect of the medial
condyle of the tibia, deep to semitendinosus.
Action: Biceps femoris, long head: extension of the
thigh. Biceps femoris, both heads: flexion of the leg,
external rotation of the flexed leg. Semitendinosus and
semimembranosus: extension of the thigh and flexion of
the leg; assists in internal rotation of the leg at the knee.

Proximal attachment: Rectus femoris: via two tendons, one at the anterior
superior iliac spine (ASIS) and the other at the ilium, superior to the acetabulum.
Vastus lateralis: lateral side of the upper three-quarters of the posterior femur
and the linea aspera on the posterior femur.
Vastus medialis: posteromedial aspect of the shaft of the femur.
Vastus intermedius: anterior and lateral surfaces of the upper two-thirds of the
shaft of the femur.
Distal attachment: All quadriceps attach to the patella by means of a common
tendon via the patellar ligament to the tibial tuberosity.
Action: Rectus femoris: extension of the leg; flexion of the thigh on the pelvis
when the pelvis is fixed; flexion of the pelvis on the thigh when the thigh is fixed.
Vastus lateralis, vastus medialis, and vastus intermedius: extension of the leg at
the knee.
Vastus lateralis and vastus medialis working together help to maintain normal

GASTROCNEMIUS
Proximal attachment: By two heads, the medial head
attaching to the medial epicondyle of the femur, the lateral
head attaching to the lateral epicondyle of the femur.
Distal attachment: Via the tendocalcaneus (Achilles tendon)
to the posterior surface of the calcaneus, with soleus.
Action: Plantar flexion; aids in flexion of the knee when the
leg is not bearing weight.

Proximal attachment: Posterior aspect of the head of
the fibula and the proximal one-third of the posterior
fibula and middle one-third of the tibia.
Distal attachment: Via the tendocalcaneus (Achilles
tendon) to the posterior surface of the calcaneus, with
gastrocnemius.
Action: Plantar flexion of the foot; assists inversion;
contributes to knee stability; provides ankle stability.

  • 2. A Trigger Point (TrP) is a palpable nodule in the taut bands of the skeletal muscles' fascia. Direct compression or muscle contraction can elicit jump sign(individuals are frequently startled by the intense pain), local tenderness, local twitch response and referred pain which usually responds with a pain pattern distant from the spot. The following approach to patient care is designed to help to focus in on the problem at hand and its resolution: 1. Clearly define the areas of pain and restrictions of movement that the patient is experiencing. 2. Determine the various muscles that might be the source of your patient’s pain and restriction. 3. Palpate for constrictions and taut bands in the individual muscles. 4. Palpate associated regions for additional constrictions and taut bands. It is important to palpate the entire body, anterior and posterior, to determine associated constrictions. 6/13/2023 trigger points 2
  • 3. 5. Locate individual taut bands in the involved muscles. “Capture” the band with precise palpation and compression. 6. Apply treatment to the trigger points. Once the bands and trigger points are captured, maintain pressure through ischemic compression until softening of the point occurs. Palpation of the region after treatment will provide feedback as to whether or not there has been a complete release of the muscle. 7.   • 4. STERNOCLEIDOMASTOID Proximal attachment: Mastoid process and the lateral half of the superior nuchal line of the occipital bone. Distal attachment: Clavicular head: superior border of the anterior surface of the medial one-third of the clavicle. Sternal head: anterior surface of the manubrium, medial and more superficial than the clavicular head. Action: Acting unilaterally: rotation of the face to the opposite side and lifting the chin. Action: Upper fibers: flexion of the head and neck to the same side; elevation of the shoulder acting on the clavicle and the acromion. Middle fibers: retraction of the scapula.
